on the definition of models and the manner in which official forms of pleadings are made available in civil proceedings
On the basis of art. 125 § 3 of the Act of 17 November 1964. -Code of Civil Procedure (Dz. U. of 2014 items 101, of late. (d) the following shall be managed:
§ 1. [ Specimens of official form pleadings] The following models of official forms of pleadings shall be established:
1) the design of the application form marked with the symbol P-for the lawsuit in the simplified procedure and the lawsuit in the cases referred to in art. 187 1 the Act of 17 November 1964. -Code of Civil Procedure, hereinafter referred to as 'the Code', which is set out in Annex 1 to the Regulation;
2. a model form of the annex to the pleadings of the pleadings with the reference to the pages marked with the symbol DS, in which the subsequent pages or additional data of the parties already indicated are indicated as Annex No 2 to the Regulation;
3) a specimen of the letter containing the evidentiary conclusions marked with the symbol of the WD-for the pleading in the simplified procedure, in which the evidence is submitted not indicated in other letters, constituting Annex 3 to the Regulation;
4) the form of the response form on the basis marked with the symbol of the IP, for the pleadings of the pleadings in the simplified procedure, which is annexed to the Regulation;
(5) a model form of the application form, marked by the CI symbol, for the mutual action in the simplified procedure and the counterpart in the cases referred to in Article 3 (2) of the EC-law. 187 1 the Code, which is Annex No 5 to the Regulation;
6) a model of the form of opposition or allegations marked with the symbol of the SP-for opposition from the judgment of the court, allegations or opposition from the order for payment in simplified proceedings and the pleas or objections from the order for payment in the cases referred to in Art. 187 1 of the Code, which is Annex No 6 to the Regulation.
§ 2. [ Specimens of official forms of application forms used in the district courts of the perpetual books in the IT system] The following models of the official application forms used in the district courts in the IT system shall be established as follows:
1) the model of the application form for the establishment of a land-book marked with the KW-ZAL symbol, constituting Annex No 7 to the Regulation;
2) the model of the application form for the entry in the perpetual register marked with the KW-WPIS symbol, which is Annex No 8 to the Regulation;
3) a model form of the annex containing the request for an entry in the perpetual register marked with the KW-ZAD symbol, which is Annex No 9 to the Regulation;
4) a model form of the annex containing the marking of the register parcel marked with the KW-OZN symbol, which is Annex No 10 to the Regulation;
5) a model form of the annex containing the applicant's or participant's data marked by the KW-WU symbol, which is Annex No 11 to the Regulation;
6) model form of the annex containing the data of the representative or statutory representative marked with the KW-PP symbol, constituting Annex No. 12 to the Regulation.
§ 3. [ Official Forms] 1. The official forms made in accordance with the formulae constituting Annexes No 1-6 to the Regulation shall be printed in a shade of grey colour with the symbol PANTONE Cool Gray 9.
2. The official form, made in accordance with the model of Annex 7 to the Regulation, shall be printed in the shade of blue colour with PANTONE symbol 280 U.
3. The official form, made in accordance with the model of Annex 8 to the Regulation, shall be printed in the shade of blue colour, with the symbol PANTONE 279 U.
4. The official forms made in accordance with the formulae constituting Annexes No 9 to 12 to the Regulation shall be printed in a shade of brown colour with the symbol PANTONE 2695 U.
§ 4. [ Official forms of pleadings] 1. The official forms of written pleadings listed in § 1 shall be made available free of charge in the buildings of all district and district courts.
2. The Authority's application forms and annexes to the applications referred to in § 2 shall be made available free of charge in the departments of the perpetual courts of the district courts and the Central Information of the Perpetual Books.
3. The official forms shall be made available free of charge on the websites of the Ministry of Justice and the general courts.
4. In the case referred to in paragraph. 3, the official forms shall be made available in a form which permits the editing of their content.
§ 5. [ Printing of the official computer form] 1. The official forms, their printouts and photocopies shall be of the A4 format.
2. The printing of the official computer form should reflect the structure of the design of the form.
3. The printout of official computer forms does not apply the requirements of § 3.
§ 6. [ Application of official rules] The proceedings brought before the date of entry into force of this Regulation shall be subject to the provisions of the existing provisions.
§ 7. [ Entry into force] This Regulation shall enter into force on 3 May 2012. 1)

1) This Regulation was preceded by the Regulation of the Minister of Justice of 29 May 2002. on the determination of the formulae and the manner of making available to the parties the official forms of pleadings in civil proceedings (Dz. U. Nr 80, poz. 728 and 2005 Nr 80, pos. 704) and the Regulation of the Minister of Justice of 14 August 2003. on the determination of the models and the manner of making available the official application forms used in the district courts leading the perpetual accounts in the IT system (Dz. U. No 156, pos. 1527, 2009 Nr 29, pos. 189 and of 2011. Nr 85, pos. 462), which have lost power on the date of entry into force of the Act of 16 September 2011. on the amendment of the Act-Code of Civil Procedure and some other laws (Dz. U. No. 233, item. 1381).
