on financial and financial reports on the performance of family benefits tasks
On the basis of art. 33 (1) 3 of the Act of 28 November 2003. on family benefits (Dz. U. 2006 r. Nr 139, pos. 992, with late. 1. 2) ) The following shall be managed:
§ 1. The Regulation sets out the models and the way in which the financial statements for the performance of family benefits tasks, hereinafter referred to as 'reports', are drawn up and the time limits and presentation of the reports.
§ 2. 1. The municipality shall submit the wojewater, in accordance with the provisions issued on the basis of art. 23 (1) 6 of the Act of 28 November 2003. on family benefits, hereinafter referred to as 'the Act', in electronic form, within a period of up to 15. on the day of the month following the end of each quarter, a quarterly report containing the information in kind on the implementation of the law and information on the family benefits paid and the costs of their handling, contributions to the pension insurance scheme (s), social security and health insurance premiums, drawn up on the basis of the current records of the family benefits paid, the social security contributions paid and the social security contributions and the contributions to the health insurance, as well as on the basis of service cost records.
2. The Wojewoda shall submit a quarterly summary report containing the data from the reports referred to in paragraph 1. 1, the minister competent for family affairs, in accordance with the provisions issued on the basis of art. 23 (1) 6 of the Act, in electronic form, by the date until the last day of the month following the end of each quarter.
3. Model of the reports referred to in paragraph 1 and 2, specifies Annex 1 to the Regulation.
4. The Marshal of the voivodship shall submit to the voivodship, in accordance with the provisions issued on the basis of art. 23 (1) 6 of the Act, in electronic form, by the deadline of 15. on the day of the month following the end of each quarter, a quarterly report containing information on the implementation of the tasks relating to the coordination of social security systems relating to family benefits, including the costs of carrying out the task ordered from the scope of government administration.
5. The wojewoda shall submit a quarterly report containing the data of the report referred to in paragraph 1. 4, the minister competent for family affairs, in accordance with the provisions issued on the basis of art. 23 (1) 6 of the Act, in electronic form, by the date until the last day of the month following the end of each quarter.
6. Model of the reports referred to in paragraph 4 and 5, specifies Annex 2 to the Regulation.
§ 3. 1. The provisions of the Regulation shall apply to the drawing up of reports, starting with the reports for the 1st quarter of 2013.
2. The Municipality and the Marshal of the voivodship shall submit the report for the 1st quarter of 2013. by 30 April 2013 at the latest.
3. The Wojewoda shall submit to the Minister responsible for the family of the report for the 1st quarter of 2013. by 15 May 2013.
§ 4. This Regulation shall enter into force on the day following that of the announcement. 3)

3) This Regulation was preceded by the Regulation of the Council of Ministers of 23 May 2006. on the mode of transfer of funds for the payment of family benefits and the manner in which the financial and financial statements are drawn up (Dz. U. Nr. 95, pos. 661 and 2007 Nr 250, pos. 1869), which is repealed with effect from the date of entry into force of this Regulation on the basis of art. 14 of the Act of 7 December 2012. to amend the Act on Family Benefits and certain other laws (Dz. U. Entry 1548).
