on the form of medical treatment of pregnant women giving entitlement to an allowance for the birth of a child and a model certificate confirming that the child is in custody

On the basis of art. 9 ust. 8 of the Act of 28 November 2003. on family benefits (Dz. U. 2006 r. Nr 139, pos. 992, with late. 1. ²⁾ ) The following shall be managed:

§ 1. The Regulation shall specify:

1) the form of medical care exercised over a woman no later than 10. a week of pregnancy in childbirth, entitling to the child's birth allowance;

2. a specimen of a medical certificate or issued by a midwife confirming the woman's stay in the medical care referred to in point 1.

§ 2. 1. The forma of medical care referred to in § 1 point 1 shall be the health benefits provided in connection with the pregnancy within:

1) primary health care;

2. ambulatory specialist benefits;

3) hospital treatment.

2. The health benefits referred to in paragraph 2. 1, they are awarded to a woman at least once in each trimester of pregnancy.

3. The model of the certificate referred to in paragraph 1 (2) shall be as set out in the Annex to the Regulation.

§ 3. This Regulation shall enter into force on 1 January 2012. ³⁾

U. No 163, pos. 1305), which expired on 31 March 2010. in connection with the entry into force of the Act of 5 March 2010. amending the Act on Family Benefits Act and the Act on Health Care Benefits financed from public funds (Dz. U. Nr 50, poz. 301).
