on the model of filing of a set of data for registration to the General Inspector for the Protection of Personal Data

On the basis of art. 46a of the Act of 29 August 1997. on the protection of personal data (Dz. U. of 2002. No. 101, pos. 926, with late. 1. ²⁾ ) The following shall be managed:

§ 1. A model for the notification of a set of data to the General Inspector of the Protection of Personal Data, which is annexed to the Regulation, shall be specified.

§ 2. The Regulation of the Minister of Internal Affairs and Administration of 29 April 2004 is hereby repealed. on the model of filing of a set of data for registration to the General Inspector of the Protection of Personal Data (Dz. U. No 100, pos. 1025).

§ 3. The Regulation shall enter into force six weeks after the date of the announcement.
