If the operator fails to submit the requested additional information from the Ministry or from the information provided by economic operators shows that electricity is produced from renewable energy sources or high-efficiency cogeneration in the process, the Ministry shall take a decision on the refusal to issue a certificate of origin. 8. The Ministry's decision on the refusal to issue a certificate of origin, the manufacturer may appeal to the administrative procedure law. 9. Proof of origin indicates the following: 9.1. energy, from which was derived the electricity and production start and end dates; 9.2. the unit name, location, type and capacity of heat and electricity, which the energy produced; 9.3. the date of issue of the certificate of origin, country and a unique identification number; 9.4. the amount of the State aid and the aid scheme, if the energy production unit has been making public aid; 9.5. date of commencement of operation of the plant. 10. The proof of origin of high efficiency cogeneration of electricity produced in the process in addition to the provisions referred to in paragraph 9 shall bear the following information: 10.1. electricity use fuel lower calorific; 10.2. the electricity for heat and use; 10.3. industrial installations for nominal electricity and heat efficiency; 10.4. electricity produced from high-efficiency cogeneration, the subject of proof; 10.5. the primary energy savings calculated in accordance with the order in which the calculated cogeneration stations produced primary energy savings. 11.
