No 81 26) order in which social services are registered with social services and excluded from it Issued under the social services and social assistance Act, article 17, the second part of rule 1 defines the order in which the social services (hereinafter services) providers of social services are recorded in the register (hereinafter register) and excluded from it.
2. the registry administrator is subordinated to Ministry of welfare in the administration of social services (hereinafter referred to as the Government).
3. the register shall register the individuals, State and local authorities and their institutions, branches and departments, which provide social services and social assistance statutory social services.
4. When you register a provider, the Board shall issue a certificate of registration (annex 1). The registration certificate shall be issued for an unlimited duration.
5. If the service provider provides various types of services, the provider shall issue a separate certificate of registration for each service.
6. If there is more of the actual addresses of the provision of services (service provider departments or branches in different locations), the service provider shall issue a separate certificate of registration corresponding to each service location and recorded as a separate service provider.
7. The register shall include the particulars of the service provider and the service provided is freely available. If you need additional information, administration after receipt of a written request providing the free State and local authorities, but for a fee, to individuals.
8. To register a service provider, service provider, driver, or his authorized person before the commencement of the provision of the service, but not later than within three months after the service launched, lodge Administration: 8.1 submission (annex 2);

12. the registry entry: 12.1. details of service provider and services: 12.1.1. number and date of registration;
12.1.2. the name of the service provider;

12.1.6. service provider unified registration number, but, if the service provider is the Department or branch, its individuals or public authorities, local governments or institutions of the unified registration number, which created a divisional or branch;
12.1.7. social service form;
12.1.8. service to be provided or the services;
12.1.9. customers by age and sex;
12.1.10. customer group to which the services are provided;
12.1.11. General information about the service provider;
12.1.12. service provider's supervisor's name and title;
12.1.13. If the service provider is the Department or branch, its individuals or public authorities, municipalities, or the head of the institution name and position that created it;
12.2. the service provider registration code assigned. Each registration code is structured according to the following requirements: 12.2.1. the first part is in accordance with the administrative territory of the Republic of Latvia and territorial units (EUROPEAN TREATIES: LOOKING BACK) the administrative and territorial unit for the territory assigned code (seven characters);
12.2.2. the second part is in accordance with the statistical classification of products by activity (CPA) in the code given;
12.2.3. the third part is the service provider code according to this provision, annex 3 (six characters);
12.3. Administration dates of checks;
12.4. the message corresponding to the last test results on the service provider's compliance with the requirements set out in the law on social service providers;
12.5. Notes on exclusion from the register, the date of exclusion and the exclusion grounds.
13. the service provider receives the certificate of registration after you have submitted the payment document certifying payment of the State fees for the issue of the registration certificate.
14. The Board adopted the decision on the suspension of the registration if: 14.1. these provisions have been submitted to all the documents referred to in point 8;
14.2. the documents submitted are incomplete or submitted statutes (regulations) does not indicate that the applicant provide the services mentioned in the application;
14.3. the Board reasonable doubts as to the veracity of the information provided.
15. the decision on the suspension of the registration authority adopted 14 working days following this provision, the document referred to in paragraph 8 and in writing, inform the service provider. The head of the service provider or his authorized person 14 working days of receipt of the decision, shall submit the decision on the suspension of the registration of the documents and particulars listed or justified request to extend the deadline for the submission of documents.
16. The Board shall adopt a decision on the refusal to register the provider if: 16.1. service does not conform to social services and social welfare services listed in the Act;
16.2.14 working days following this provision in paragraph 15 of the decision of suspension of registration have been submitted that decision contains documents and messages, or substantiated request to extend the deadline for the submission of documents;
16.3. provide false information;
16.4. the service provider does not comply with the requirements of legislation for the social service providers.

17. The registered service provider for the driver or an authorized person, but in cases where the service provider is the Department or branch – the individuals, public bodies, authorities or institutions, which created it, is obliged within 10 working days to inform the Administration about the suspension, termination or changes in the organisation and provision of services, by changes in the supporting documents.
18. The Board examined this provision in paragraph 17 of these documents and make changes in the registry or 14 working days, decide on change tracking and the new registration certificate is issued or the service provider's exclusion from the register.
19. If you have changed the service provider legal address, phone or fax number, electronic mail address, website address (if any) or the provider's supervisor, the Board made the appropriate changes to the registry, do not change the service provider's certificate of registration.
20. If the Board receives other information about the registered service provider it five working days, but not less frequently than once a month to update the information in the registry.
21. If a registration certificate has been lost or damaged, the Government issued the duplicate registration certificate (if the registration certificate issued to the electronic form of a document, the original), based on the application service provider. Administration prepares a duplicate within two weeks after receipt of the application. 22. The Board adopted the decision on the service provider or the service provided from the registry if: 22.1 the service provider received a written application for removal from the registry;
22.2. the service provider has terminated the activities;
22.3. the service provider does not comply with the requirements of legislation for the social service providers, are administratively punished for failure to comply with the requirements and do not resolve the shortcomings identified in the regulatory deadline;
22.4. the entry into force of the Court decision on the service provider operating the suspension, termination or insolvency;
22.5. the Board notes that the service provider terminates service provision.
23. If the service provider or the service supplied is excluded from the registry, the service provider for the driver or an authorized person, but in cases where the service provider is the Department or branch – the individuals, public bodies, authorities or institutions, which created it, is obliged to transfer the licence previously issued.
24. the Government established the list of service providers, as well as the list of service providers who are excluded from the register shall be published in the internet home page of the administration. The list indicates: 24.1. registered service providers: 24.1.1. service provider's name, date of registration and registration number;
24.1.2. service provider's legal status;
24.1.3. the form of the provision of services;
24.1.4. Services;
24.1.5. customers (age and sex) that services are provided;
24.1.6. customer group that services are provided;
24.1.7. service provider registered and actual address, if different from the registered office;
24.1.8. service provider's phone and fax numbers, electronic mail address, website address (if any);
24.2. excluded from the registry services provider or individual services: 24.2.1. service provider name and the registration number;
24.2.2. service provider's legal status;
24.2.3. service provider or service provided, the date and grounds for exclusion.
25. the service provider who has just launched a service prior to the entry into force of these regulations and is not registered in the registry, the application for registration shall be submitted within 12 months of the entry into force of these regulations.
26. the registration certificate issued to the date of entry into force of the provisions are valid until their expiration date. If your service provider has decided to continue the service after registration certificate expiration date, service provider, driver, or his authorized person makes administration of these rules 8.1 application referred to reregister the service provider and to issue a certificate of registration as well as the transfer of previously issued a certificate of registration.
27. Be declared unenforceable for Cabinet 1 February 2005 rules no. 78 "order in which social services are registered with social services and off of it" (Latvian journal, 2005, nr. 20; 2006, 68 no).
