Cabinet of Ministers Regulations No. 587 in Riga 2014 (30 September. No 51 13) national social policy monitoring information system of rules Issued in accordance with the social services and social assistance law 14. the third paragraph of article i. General questions 1. determines: 1.1. State social policy monitoring information system (hereinafter referred to as the information system) structure; 1.2. information system data to be included; 1.3. the data contained in the information system processing (submission, escalation, and the storage, application of service) rules and procedures; 1.4. the terms of cooperation between the institutions. 2. the information systems manager is the Ministry of welfare (hereinafter referred to as the Ministry). Information system data from municipal information systems (these rules 9, 10 and 13, the data referred to in paragraph) is inserted after the decision of the State (administrative act) at the date of adoption and for the quality of their data and the answer changes in those data at the request of the data subject in the municipality in question. 3. to improve social services and social assistance, coordination and monitoring, information system shall contain data on social services and social assistance to individuals who requested them. 4. the information system maintained and the data it contains is treated with savietotāj of the national information system or through a direct interface with other national information systems and municipal information systems.
II. the information systems structure 5. Information System consists of the following sections: 5.1. social service provider register; 5.2. register of the persons who requested that, receive or reject social services or social assistance (hereinafter referred to as the register of persons); 5.3. social services; 5.4. the statements and reports. 6. Social services providers include the data in the register in accordance with the laws and regulations on social order in which the service provider is registered in the register of social services and excluded from it. 7. the register shall include the following: 7.1 the personally identifiable data; 7.2. data on the person's assets; 7.3. the data about the person assigned to the municipal social benefits and social services; 7.4. data on the person assigned to the State social services. 8. the following shall be included in the register of persons personally identifiable data: 8.1 the word (words); 8.2. the last name; 8.3. the code; 8.4. date of birth; 8.5. the sex; 8.6. nationality and its type; 8.7. the news of the death of the person, including the date of death; 8.8. details of a person's legal capacity and capacity to limit revisions; 8.9. details of person's guardians, the guardians or foster family; 8.10. news about the legal status of persons in the population register; 8.11. identity document data: 8.11.1. document type; 8.11.2. the number of the document; 8.11.3. the date of issue of the document; 8.11.4. period of validity of the document; 8.11.5. for the issuing State, the issuing authority; 8.11.6. the current status of the document to the State information system "invalid document register"; 8.12. the person, registered or declared in the specified home address; 8.13. data on a person's disability: 8.13.1. disability determination date; 8.13.2. disability group; 8.13.3. date by which a particular disability; 8.13.4. disability certificate number; 8.13.5. indication of movement disorders; 8.13.6. indication of visual impairments; 8.13.7. indication of the hearing impaired; 8.13.8. indication of severe mental disabilities; 8.13.9. the reference to the existence of other functional disorders; 8.13.10. disability cause; 8.13.11. opinion on the need for particular care due to severe functional impairment (or not); 8.13.12. date issued an opinion on the need for particular care. 9. the register shall include the following data on personal assets: 9.1. data on the poor or needy person: 9.1.1. status – poor or indigent; 9.1.2. the municipality, in which status is determined; 9.1.3. the decision granting the status date and number; 9.1.4. the status start date of the period; 9.1.5. status end date of the period; 9.2. data on the livelihoods of the declarations and the persons referred to in the Declaration: 9.2.1. name of the applicant (name), surname, identity code; 9.2.2. the date of filing; 9.2.3. the municipal authority which submitted the Declaration; 9.2.4. the applicant and the person who is living with the applicant, the average declared income per month; 9.2.5. the Declaration contains the name of the person: 9.2.5.1. (name), surname, identity code; 9.2.5.2. relationships to the applicant; 9.2.5.3. employment status; 9.2.5.4. average income; 9.2.5.5. income; 9.2.5.6. real estate; 9.2.5.7. movable property; 9.2.5.8. maintaining contract concluded; 9.3. data on the residence of survey: surveyed in 9.3.1. address; 9.3.2. the date of the survey; 9.3.3. those surveying – name, title; 9.3.4. the reason for the survey. 10. the register shall include the following data for municipal social benefits and social services: 10.1. date of filing of the application; 10.2. the authority or authorities, which filed the application; 10.3. application type – a social benefit or social services; 10.4. social benefit or social service status-granted, deferred, rejected, additional information, and an indication of its receipt; 10.5. the date when the social allowances or social service is granted or refused; 10.6. social benefits feature – income tested or untested benefit; 10.7. the municipal social benefits type: 10.7.1. guaranteed minimum income level benefit; 10.7.2. housing allowance; 10.7.3. unrivaled benefits in emergency situations; 10.7.4. other laws or regulations of the Cabinet of Ministers established the municipal social benefits, including social guarantees for orphans and audžuģimen; 10.7.5. local binding rules laid down by other social assistance benefits for families (individuals) the provision of basic needs; 10.7.6. local support by voluntary initiatives; 10.8. the municipal social services provided: 10.8.1. social service provided by the service provider; 10.8.2. social service provider, your local authorities, other authorities, non-governmental organisation, a trader; 10.8.3. social services funding feature – national, local, client, another source; 10.9. in the event of refusal, indicate on what grounds was denied social allowances or social service; 10.10. social allowance or social service payment: 10.10.1. award type – in cash or in kind; 10.10.2. month for which is assigned; 10.10.3.; 10.10.4. due date. 11. the register shall include the particulars of the State-financed long-term social care and social rehabilitation services and Assistant services for persons with disabilities in the municipality. 12. the information systems section of the "persons case" shall include the following data on State-funded long-term social care and social rehabilitation services: 12.1. personal application for granting of services: 12.1.1. name of the applicant (name), surname, identity code; 12.1.2. the date of filing; 12.1.3. social services; 12.1.4. preferred service provider; 12.2. data on the legal representative: 12.2.1. name (name), surname, identity code; 12.2.2. the declared or specified registered residence address; 12.2.3. phone and other means of communication; 12.3. the decision makers need the service (municipality): 12.3.1 name; 12.3.2. address; 12.3.3. phone and other means of communication; 12.4. list of documents submitted; 12.5. details of additional documents may be requested; 7.8. decision on granting of services: 12.6.1. the date of the decision; 12.6.2. document number; 12.6.3. the name of the service provider; 12.6.4. date from which the service is granted; 12.7. decision on admission in line service: 12.7.1. the date of the decision; 12.7.2. document number; 12.7.3. the name of the service provider; 12.7.4. date from which the shot line. 12.8. the information on the receipt of services and service delivery institutions: 12.8.1. admission to the institution in the posting date and document number; 12.8.2. the institution of the date of accession; 12.8.3. the particulars of the State social insurance agency administered payments; 12.8.4. contract with the person that takes the institution: 12.8.4.1. the date when the contract is concluded; 12.8.4.2. contract number; 12.8.4.3. If the contract is closed by another person, in addition to the following information: 12.8.4.3.1. name (name), surname, identity code; 12.8.4.3.2. registered or declared, the specified home address; 12.8.4.3.3. phone and other means of communication; 12.8.5. the person located outside the institution's prolonged absence; 12.8.6. personal service provider change application: application submission date 12.8.6.1.; 12.8.6.2. requested the name of the service provider; 12.8.6.3. other information; 12.8.7. the application to change the service type: 12.8.7.1. the date on which the application was lodged; 12.8.7.2. the requested service type; 12.8.7.3. other information; 12.8.8. termination of service of an application, the filing date; 12.8.9. the decision on the termination of the service person: 12.8.9.1. date of adoption of the document; 12.8.9.2. document number; 12.8.9.3. the date when the decision takes effect. 13. the information systems section of the "persons case" shall include the following data on State-funded Assistant services for persons with disabilities in municipalities: 13.1. date when the municipal social services for persons with disabilities registered with the application for the service; 13.2. the date of the municipal social service's decision on granting of services; 13.3. a mark that a person with disabilities receive the services; 13.4. do not require refund of transport service – is not; 13.5. the information about the Assistant: 13.5.1. name (name), surname, identity code; 13.5.2. gender; 13.5.3. relationships to the service received person; 13.5.4. types of employment (contract of employment, the company contract or other agreement, the information that the person is self-employed); 13.6. information on services provided by Assistant: 13.6.1. what actions are used to Assistant service; 13.6.2. the assigned hours per week; 13.6.3. Assistant hours worked per week; 13.6.4. Assistant worked hours per month; 13.6.5. hours; 13.6.6. Assistant gross salary; 13.6.7. transport refund; 13.6.8. the total amount; 13.6.9. contract number and period; 13.6.10. service start date; 13.6.11. Assistant State social security payment rate; 8.5. information on the municipal social service projected funding, financial leverage and performance indicators; 13.8. the municipal social service reports on the Assistant's services for persons with disabilities. 14. the information systems section of the "social service provider for inspection" shall include the following data of the Ministry of social services carried out checks: 14.1. information on social service provider: 14.1.1. the name of the service provider, a social service provider registration number; 14.1.2. service address; 14.1.3. the service supplied; 14.1.4. service test data: 14.1.4.1. date of inspection; 14.1.4.2. test type; 14.1.4.3. type of inspection; 14.1.4.4. the examiner; 14.1.4.5. the test rated requirements; 14.2. the test assessed the requirements list: 14.2.1. code requirements; 14.2.2. the requirements; 14.2.3. conclusions on the compliance or non-compliance with the requirements; 14.3. test results the recommendations in: 14.3.1. code requirements; 14.3.2. Description of the recommendation; 14.3.3. the deadline for implementation of the recommendation; 14.3.4. review of the implementation of the recommendation. 15. the information systems section "reports and reports" include State and local social services and social assistance, a private social services provided information based on national statistical surveys: 15.1 the social services and social assistance district (annual and monthly reports); 15.2. long-term social care and social rehabilitation services (annual report); 15.3. social rehabilitation services of the unlawful activity of the injured children (annual report).
III. The right to receive free of charge from the information system, including through e-services are: 18.1. individuals – for themselves and under the auspices and people; 18.2. scientists and researchers – scientific or statistical studies, subject to the condition that the message does not contain personally identifiable data; 18.3. the State and local authorities-for the functions these bodies the necessary data; 18.4. the persons delegated administration tasks, the tasks delegated to the extent necessary. 19. 19.3. information systems information and technical resources comply with the laws and regulations governing the circulation of electronic documents and storage; 19.4. the information system is ensured compliance with at least the following infrastructure protection requirements: 19.4.1. systems and infrastructure (servers, disk arrays, switches, which connected system server) protect against unauthorized access, theft and wilful or accidental damage (e.g. flood, fire); 19.4.2. System Security Manager is responsible for ensuring that interoperable systems and infrastructure facilities savietotāj enter only those persons who need it for the performance of the duties of the job; 19.4.3. infrastructure facilities shall be equipped with a security alarm (detector, which records the unauthorised opening of doors and Windows), smoke and fire detectors, automated gas fire-fighting system, or those with fire-fighting appliances; 19.5. the compliance of the system is ensured by the following system of logical protection requirements: 19.5.1. the system's internal computer networks separate from the internet by a firewall; 19.5.2. systems and infrastructure (servers, disk arrays) in a separate subnet separated or separated by a firewall; 19.5.3. provides system antivirus protection.
