3.2. information about resources — forms, name, type, unit of measure, quantity. 4. the resources required for the restoration of railway traffic after the accident, the carrier passes the railway infrastructure manager immediately after a written or oral request, a receipt (request). 5. the request shall include information on the nature of the accident and the resources for rail infrastructure manager's discretion are best suited to restore rail traffic, as well as information about the type of assistance requested, amounts and time limits. 6. the carrier of the request shall immediately take a decision on the dispatch of resources and orally or in writing inform the rail infrastructure managers for resources. 7. If the carrier resources involved in rail traffic, then reimburses the value of the resource (expenses), but will not be reimbursed for income that is not achieved because of the carrier's resources were involved in rail traffic. 8. in order to receive reimbursement for expenses, the carrier two weeks after rail traffic restoration work completed shall be submitted in writing to the railway infrastructure manager application for reimbursement for the use of resources to restore railway traffic after the accident (hereinafter application). 9. the application shall contain the following information: 9.1. remuneration of a claimant (the carrier's) full name, registration number, address, telephone number;
9.2. details of resources involved (type, name, type, quantity, unit of measure) and engagement (from a date and time to a date and time);
9.3. the calculation for each resource (type, name, type, quantity, unit of measure) separately and together;
9.4. checking account credit institution to which the remuneration payable;
9.5. list of attached documents. 10. the application shall be accompanied by a copy of the document certifying the applicant's resources or possession of the property. Railway infrastructure manager has the right to ask the carrier to produce a copy of the original documents submitted. 11. rail infrastructure manager, within two months from the date of receipt of the application, examine and take decision on the reimbursement. 12. If the railway infrastructure manager after receiving the application, further information is necessary, it may request the relevant information from the requesting remuneration during the month from the date of receipt of the application. 13. Railway infrastructure manager of your decision in writing to the carrier.
