REGULATION of the MINISTER of TRANSPORT of the REPUBLIC of INDONESIA, the REPUBLIC of INDONESIA NUMBER am 69 year 2012 ABOUT the GUIDELINES for PREPARING the ANNUAL PERFORMANCE PLANS, PERFORMANCE and ACCOUNTABILITY REPORTS ASSIGNMENT PERFORMANCE in the ENVIRONMENT of the MINISTRY OF TRANSPORTATION with the GRACE of GOD ALMIGHTY MINISTER of TRANSPORT of the REPUBLIC of INDONESIA, Considering: a. that in order to increase and control of achievement of performance on each of the work units, the Ministry of transportation has assigned Minister of transportation Regulation Number pm. 86 in 2010 on guidelines for the preparation of reports on performance accountability in the Environmental Ministry of transportation;
b. that the regulation of the Minister of transportation Number PM. 86 in 2010 on guidelines for the preparation of reports on performance accountability in the Environmental Ministry of transportation referred to in letter a, is no longer appropriate to the conditions and needs in the field so that needs to be enhanced;
c. that based on considerations as referred to in subparagraph b above, the need to establish Regulations the Minister of transportation of the Republic of Indonesia concerning the guidelines for preparing the Annual Performance Plans, performance and Accountability Reports Assignment performance in the environment of the Ministry of transportation;

Decide: define: REGULATION of the MINISTER of TRANSPORT REGARDING the GUIDELINES for PREPARING the ANNUAL PERFORMANCE PLANS, PERFORMANCE and ACCOUNTABILITY REPORTS ASSIGNMENT PERFORMANCE in the ENVIRONMENT of the MINISTRY OF TRANSPORTATION.
CHAPTER I GENERAL PROVISIONS article 1 In this regulation of the Minister, is: 1. Performance accountability is the embodiment of an obligation of government agencies to guarantee his success/failure of the implementation of the Organization's mission in achieving the goals and objectives that have been established through periodic accountability tool.
2. Performance accountability system of government agencies hereinafter abbreviated SAKIP pennyelenggaraan accountability is a process of Organizational Units that are interconnected to each other which substantially consists of the activities of the preparation of strategic plans, Performance Plans preparation activities (CTR), monitoring and observation of the implementation of the basic tasks and functions of the agencies, the measurement of performance and achievement of performance evaluation and reporting of the performance of the thoroughly integrated and to encourage the creation of the accountability of government agencies as one of the conditions the creation of good Governence and trusted.
3. The annual Performance Plan hereinafter abbreviated CTR is the document that contains the elaboration of goals and programs that have been set out in the strategic plan (RENSTRA), which will be implemented by government agencies through a variety of annual activities and contains information about the target performance level or form of outputs and/or outcomes who want manifested by an organization on a given year.
4. plan performance and Budget Ministries/Agencies hereinafter abbreviated RKA-K/L is an annual financial plan document ministries/Institutions arranged according to the part of the budget of the ministries/agencies.
5. Determination of Performance which further abbreviated PK is a performance that will be realized within 1 (one) years by a head of work Unit supervisor Directly.

6. Report of the performance accountability which further shortened LAKIP is reporting performance accountability of government agencies which are yearly that contains a performance liability agencies in achieving goals/strategic objectives of agencies.
7. Main performance indicators further abbreviated as IKU is a measure of the success of the product a strategic organizational goals and objectives to which dtetapkan.
8. Performance indicators Activities hereinafter abbreviated IKK is a measure of the performance of the implementation activities in producing goods and services in accordance with planned in order to achieve goals and objectives of the organization. 9. The Minister is the Minister of transportation.

10. the General Secretary is the Secretary General of the Ministry of transportation.

11. The Inspector General is the Inpektur General of the Ministry of transportation.

12. the Director-General was Director General at the Ministry of transportation.

13. The head of the Agency is the head of the Agency at the Ministry of transportation.
14. Work units is Organizational Unit Level Echelon I, Organizational Unit Level Echelon II, and Self-contained work units (units Technical Implementers/UPT) in the environment of the Ministry of transportation.
Article 2 (1) guidelines for the preparation of Annual Performance Plans, performance and Accountability Reports Assignment performance in the environment of the Ministry of transportation, as a reference to any work units the Ministry of transportation in drawing up documents: a. the annual Performance Plan (CTR);

b. determination of performance (PK); and c. the performance Accountability Report work units the Ministry of transportation.
(2) any work units the Ministry of transportation is required to draw up Annual Performance Plan Documents, the determination of performance and Accountability Reports the performance of government agencies sebgaimana referred to in paragraph (1).
ANNUAL PERFORMANCE PLAN CHAPTER II article 3 Annual Performance Plan Document (CTR) is the elaboration of the strategic plan (RENSTRA) and work plan (RENJA) as the basis for preparing the Documents in order to work plan and budget Ministry/Agency (RKA-K/L) and Performance Assignment Documents (PK).

Article 4 (1) the annual Performance Plan Documents as referred to in article 3 and at the lack of consists of: a. an annual Performance Plan form is the level of the Ministry of transportation;
b. Annual Performance Plan Form-level organizational Unit Echelons I at the Environment Ministry of transportation;
c. Annual Performance Plan Form-level organizational Unit Echelon II and work units Independently in the environment of the Ministry of transportation.
(2) the annual Performance Plan Document Form referred to in subsection (1), using the Format as contained in Appendix I which was part an integral part of the regulation of the Minister.

(2) in the preparation of the performance Assignment Documents as referred to in paragraph (1), based on Annual Performance Plan Document (CTR) and document work plan and budget Ministry/Agency (RKA-K/L).
(3) the determination of the Performance Document referred to in subsection (1) at the lack of load: a. a statement of determination of performance;

b. Preface; and c. Attachments Form the setting of performance.
(4) determination of Performance Documents referred to in subsection (1), using the Format as contained in Appendix II that are part an integral part of the regulation of the Minister.
Article 8 determination of the performance of the Statement referred to in article 7 paragraph (2) letter a, at least contain: a. a statement of the head of the Unit of work to realize a performance in any given year;

b. the date of the signing of the statement of the determination of performance;

c. Signature head of work Unit;

d. Direct Supervisor's approval.

Article 9 Annex a form of determination of Performance referred to in article 7 paragraph (2) Letter c, contains the following things: a. strategic goals, declared the results that will be achieved in the real by work units in the formulation of more specific, measurable, within 1 (one) year; b. IKU and/or IKK, as quantitative or qualitative measure of the achievement of the strategic goals set;
c. Target performance, value or achievement of IKU and/or IKK targeted will be achieved by Units Working within 1 (one) year; d. the main Program or main activity, describe the main tasks are the responsibility of the work Unit;

e. the amount of the budget allocated for the realization of strategic goals.

The second part of the performance Assignment Procedures article 10 Drafting Performance Assignment Documents, coordinated by: a. the Secretary-General for the preparation of Performance work Unit Assignment Document Level Echelon I in order to Document drafting Assignment performance of the Ministry of transportation;
b. the Secretary General Inspectorate, Secretary of the Directorate General, the Secretary of the Agency, the head of the Bureau for the preparation of the planning Documents Performance Assignment work Unit Level Echelon II and Self-contained work units in the framework of the preparation of the document is the determination of the performance of the work Unit Echelons I question.

b. Performance Assignment Documents from around the work Unit Level Echelon I submitted to the Minister, no later than on 30 February next year;
c. Ministry of transportation Performance Assignment Document from the Minister conveyed to the President through the Minister of empowerment of State apparatus and the reform of the bureaucracy, no later than on 30 March next year.
Article 12 the signing Statement of determination of performance, are set as follows:

a. Statement of determination of performance for work Unit Level Echelon II and Self-employment Units, signed by the leadership of the work Unit concerned and approved/signed by the leadership Echelon Level Organizational Unit I as a Direct Supervisor;
b. a statement of determination of performance for work Unit Level Echelon I, autographed by the leadership of the work Unit concerned and approved/signed by the Minister.

Article 16 Reporting performance accountability as stipulated in article 15, compiled with the following conditions: a. made systematically, obviously, and solid;
b. describe the overall Work Unit activities related to the implementation of the tasks and functions of formulaic objectively and systematically;
c. present the relevant data and information, in order to be evaluated about success and failure of a work Unit in achieving strategic goals and objectives in a more extensive and profound.
The second part of performance Accountability Report Drafting article 17 performance accountability Report as stipulated in article 15, compiled with the following conditions: a. performance accountability Reports compiled by the Ministry of Transportation Minister, and delivered to the President through the State Minister of empowerment of State apparatus and Bureaucratic Reforms;
b. report of the performance accountability work Unit Level Echelon I at the Environment Ministry of transportation arranged by the Secretary General, Inspector General, Director General, and head of the Agency, and delivered to the Minister;
c. report of the performance accountability work Unit Level Echelon II and work units Independently in the Environment Ministry of transportation arranged by the leadership of the work Unit concerned, and conveyed to the leadership of the work Unit Level Echelon I respectively.
Article 18 the performance Accountability Report preparation, coordinated by: a. the Secretary-General for preparing the report on performance accountability work Unit Level Echelon I in order the preparation of performance accountability report of the Ministry of transportation;
b. the Planning Bureau Chief, Inspectorate General Secretary, Secretary of the Directorate General, and the Secretary of the Agency for the preparation of reports on performance accountability work Unit Level Echelon II and Self-contained work units in order of performance Accountability Report preparation work Unit Echelons I question.
The third part of the report Form and Systematic accountability for performance of article 19 (1) Public performance accountability Report Systematics, consisting of: a. the preface;

b. Executive Summary;

c. Introduction;

d. planning and Agreement performance;

e. performance accountability;

f. Concluding; and g. the attachment-attachment.
(2) performance accountability Report as referred to in paragraph (1), using the Format as listed in Annex III which is an integral part of part of the regulation of the Minister.
Article 20 (1) performance accountability Report is made in the form of a spiral bound documents, with the cover and the back cover is made of cardboard.
(2) on the front cover of the upper part, noted the Ministry of transportation's coat of arms with the title “ ” PERFORMANCE ACCOUNTABILITY REPORT with uppercase, using the example of the Format as contained in Appendix IV which is part an integral part of the regulation of the Minister.
(3) the report typed using font ARIAL 11, 1.5 line spacing using paper HVS 80 grams of white. (4) paper size is A4.

The fourth part time and performance accountability report preparation Procedures Article 21 the deadline for the submission of performance accountability Report is as follows: a. performance accountability Report from around the work Unit Level Echelon II and Self-contained work units submitted to the Secretary General and the head of work Unit Echelons I as a Direct Employer, no later than 15 January of the following year;
b. performance accountability Report from around the work Unit Level Echelon I submitted to the Minister, no later than 15 February of the following year;
c. performance accountability report of the Ministry of transportation of the Minister conveyed to the President through the State Minister of empowerment of State apparatus and the reform of the bureaucracy, no later than 15 March of the following year.
Section 22 (1) every year end Secretary General delivered a circular letter to all the leadership Echelon I work units to draw up and submit a report to the Minister's performance Accountability with copy to the Secretary-General.
(2) to follow up the implementation of the Circular referred to in subsection (1), then to: a. the Inspectorate General Secretary delivered a circular letter to the leadership of the work Unit Echelons II in the environment Inspectorate-General, to draw up and submit a report to the Inspector General's performance Accountability;
b. the Directorate General Secretary delivered a circular letter to the leadership of the work Unit Echelon II and Self-contained work units concerned, to draw up and submit a report to the Director General of Performance Accountability is concerned;
c. the Secretary of the Agency delivered a circular letter to the work Unit Echelon II and Self-contained work units concerned, to draw up and submit a report to the head of performance accountability Bodies concerned;
d. Planning Bureau Chief delivered a circular letter to the leadership of the work Unit Echelons II in the environment of the Secretariat-General to draw up and submit a report to the Secretary General of the Performance Accountability.
Part Five Report Of The Distribution Performance Accountability


Article 23 performance accountability Report work units the Ministry of transportation as stipulated in article 21, delivered in stanzas 4 (four), and accompanied by a softcopy in the form of Compact Disk (CD).

Article 24 (1) performance accountability report of the Ministry of transportation, is distributed with the following conditions: a. the original Performance Accountability Reports submitted to the Minister, and delivered the President through the State Minister of empowerment of State apparatus and Bureaucratic Reforms;
b. a copy of the performance accountability, 1 (one) each document submitted to the Inspector General, the Secretary-General, the Director-General, and the head of the Environmental Agency in the Ministry of transportation;
c. a copy of the performance accountability, one (1) documents submitted to the Ministry of Transportation Planning Bureau Chief for the archives of the Ministry of transportation.
(2) performance accountability report of the work Unit I, distributed with the following conditions: a. the original Performance Accountability Reports submitted to the Minister, the Secretary General and the Inspector General;
b. a copy of the performance accountability Report, one (1) each document submitted to the Inspector General, the Secretary-General, the Director-General, and the head of the Environmental Agency in the Ministry of transportation; c. a copy of the performance accountability Report, one (1) Archival documents to the concerned work units.
Chapter V CLOSING PROVISIONS Article 25 at the time of this Ministerial Regulation took effect, then Transport Minister Regulation Number pm. 86 in 2010 on guidelines for the preparation of reports on performance accountability in the Environmental Ministry of transportation, revoked and declared inapplicable.

Article 26 this Ministerial Regulation comes into force on the date of promulgation.

In order to make everyone aware of it, ordered the enactment of this Ministerial Regulation with its placement in the news of the Republic of Indonesia.
