THE REGULATION OF THE REPUBLIC OF INDONESIA FINANCIAL MINISTER NUMBER 57 /PMK.05/ 2014

ABOUT THE GENERAL SERVICE AGENCY SERVICE RATE

THE LEPROSY HOSPITAL DR. RIVAI ABDULLAH PALEMBANG ON THE MINISTRY OF HEALTH

WITH THE GRACE OF THE GOD ALMIGHTY REPUBLIC OF INDONESIA,

DRAWS: A. that under the terms of Article 9 of the Government Regulation No. 23 of 2005 on the Financial Management of the Public Service Agency as amended by Government Regulation No. 74 of 2012, the service rate of the agency that applies the Management The Financial Services Agency is defined by the Minister of Finance for the proposal of the minister/leader of the agency;

b. that Kusta Hospital Dr. Rivai Abdullah Palembang on the Ministry of Health has been designated as the Instancy of the Government implementing the Financial Management Agency General Services under the Decree of Finance Minister Number 3/KMK.05/2010;

c. that the Minister of Health by Letter of Number: KU/Menkes/326/VII/2013 dated July 9, 2013, has delivered a proposal for the Service Agency service rate

General of the Kusta Hospital Dr. Rivai Abdullah Palembang on the Ministry of Health;

d. that the proposed tariffs on the services of the General Services Agency of the Kusta Hospital Dr. Rivai Abdullah Palembang on the Ministry of Health, have been discussed and studied by the Assessor Team;

e. that under consideration as intended in the letter a, the letter b, the letter c, and the letter d, need to establish the Finance Minister's Regulation on the Tarif of the General Services Board of the Hospital of Kusta Dr. Rivai Abdullah Palembang on the Ministry of Health;

Given: 1. Law Number 17 Year 2003 on State Finance (Indonesian Republic of Indonesia Year 2003 Number 47, Additional Gazette of the Republic of Indonesia Number 4286);

2. Law No. 1 of 2004 on the State Treasury (Indonesian Republic Gazette 2004 No. 5, Additional Gazette of the Republic of Indonesia Number 4355);

3. Government Regulation No. 23 Year 2005 on the Financial Management of the Public Service Agency (State Gazette 2005 Number 48, Additional Gazette of the Republic of Indonesia No. 4502) as amended by Government Regulation (Pmc) Number 74 In 2012 (sheet Of State Of The Republic Of Indonesia In 2012 Number 171, Additional Gazette Of The Republic Of Indonesia Number 5340);

Decide: Setting: A FINANCIAL MINISTER ' S REGULATION ON TARIFFS

Article 1 (1) of the General Services Agency service Board of the Kusta Hospital Dr. Rivai

Abdullah Palembang on the Ministry of Health is the reward received by the General Services Agency of the Hospital of Kusta Dr. Rivai Abdullah Palembang on the Ministry Health of services services provided to service users.

(2) The service user as referred to in paragraph (1) consists of a general public patient and guarantor party.

(3) The guarantor is referred to in paragraph (2) consisting of the central government, the government. region, and other guarantor companies.

Section 2 of the Tarif service as referred to in Section 1 of the paragraph (1) consists of:

a. Service rate based on class; b. Service rates are not based on class; and c. Pharmacy fare.

Article 3 of the Tarif service based on the class as referred to in Article 2 of the letter a consists of: a. Outpatient rate; b. Non-Operative Medical Action Rates; c. Operating Medical Action (s); and Medical Support Measures.

Article 4 of the Tarif service is not based on the class as referred to in Article 2 of the letter b consists of: a. Patient Registration Rate; b. Consulting rates; c. Action Rates at Emergency Installation; d. Take Care Of The Road; e. Medical Rehabilitation Rates; f. Educational and Exercise Rates; g. Dormitory Usage Rate; h. Ambulance Use Rates and His Graduate Car;

i. The Body's Rate Of Satisfaction; J. Service rates Medico Legal (Visum); and k. Incenerator Rate.

Section 5

(1) The services based on the class as referred to in Section 3 are distinguished by Class III, Class II, and Class I.

(2) The Class II service Tarif as referred to in paragraph (1), is subject to the general public patient as set forth in Annex I which is an inseparable part of this Minister's Regulation.

(3) Class III Tarif as referred to in paragraph (1), imposed on the highest general public patient of 90% (ninety percent) of Class II fare as referred to in paragraph (2).

(4) The Tarif Class I as referred to in paragraph (1) is subject to the lowest general public patient of 120% (one hundred and twenty percent) of the Class II tariff as referred to in paragraph (2).

Section 6

(1) Further provisions of Class III fare and Class I fare as referred to in Section 5 of the paragraph (3) and paragraph (4) are specified with the Decree of the Principal Director of the General Services Agency of the Hospital of Kusta Dr. Rivai Abdullah Palembang on the Ministry of Health.

(2) Principal Director of the General Services Board of the Hospital of Kusta Dr. Rivai Abdullah Palembang on the Ministry of Health delivered a copy of the Decision regarding Class III tariffs and Class I tariffs as referred to on verse (1) to the Minister of Health and the Finance Minister cq. The Director General of the Treasury.

Article 7

The service of the service is not based on the class as referred to in Article 4 imposed to the general public patients as set forth in Annex II which is an inseparable part from the Regulation of this Minister.

Article 8

Further provisions concerning the degree of charge of service rates based on class and service rates are not based on classes as set forth in Annex I and Annex II of the Minister's Regulations this is set with the Principal Director of the General Services Agency of the Kusta Hospital Dr. Rivai Abdullah Palembang on the Ministry of Health.

Article 9 (1) of the Pharmaceutical Tarif to general public patients as

referred to in Article 2 of the letter c, are generic drugs, non-generic drugs, free drugs and health tools are set to be large. Netto Pharmacy Price (HNA) plus Value Added Tax (PPN)

plus profit margin up to 25% (twenty-five percent) of HNA + PPN.

(2) HNA + PPN is the sale price of the Drug Factory and/or the Greater Pharmaceutical Traders to the Government, Hospital, Apotek, and Other Health Service Sarana.

(3) More provisions on the Pharmaceutical tariff as intended on verse (1) is specified by the Decree of the Principal Director of the General Services Agency of the Hospital of Kusta Hospital Dr. Rivai Abdullah Palembang on the Ministry of Health.

Article 10

(1) The General Services Agency of the Hospital of Kusta Dr. Rivai Abdullah Palembang on Ministry of Health may provide services services in the field of health to the guarantor based on the requirements of the guarantor through the contract of cooperation.

(2) the health care service with the guarantor is referred to in paragraph (1) a
