DECREE
of 24 July 2003. in July 2006,
amending Decree No. 364/2005 Coll., on the conduct of schools and
school facilities and school to the Registrar and to the transmission of documentation
schools and educational institutions, and from the school Registrar's Office (the Decree on the documentation
schools and educational institutions)
The Ministry of education, youth and sports is determined according to § 28 para. 6
Act No. 561/2004 Coll. on pre-school, primary, secondary, higher
vocational and other education (the Education Act), as amended by Act No.
383/2005 Coll.:
Article. (I)
Decree No. 364/2005 Coll., on the leadership of schools and school documentation
facilities and school to the Registrar and to the transmission of documentation of schools and
educational institutions and from the school Registrar's Office (the Decree on the documentation of schools and
school facilities), is amended as follows:
1. In article 3, after paragraph 1, insert a new paragraph 2 is added:
"(2) the data transmitted shall be based on the content of the school Registrar's Office and documentation
school or school facility on the date set out in annexes 1 and 2 to the
This Decree (hereinafter referred to as "the material date"). ".
The former paragraph 2 becomes paragraph 3.
2. In section 4, paragraph 4. 3 the words "critical date" shall be replaced by the words "the applicable

The transmission of individual data
PART THE FIRST
METHOD OF TRANSMISSION OF THE INDIVIDUAL DATA
Article. 1
Dates of transmission of data, record date, a form of transmission and the
technical resources
(1) the individual data from school registers and documentation of schools and
educational institutions shall be transmitted electronically in a secure form.
The data are transmitted to the competent administrative authority: ^ 5)
and) within 30. on the reference date of April 31. March of the same school year and
(b)) to 31. on the reference date of October 30. September of the same year.
(2) if the last day falls for the transmission of the individual data according to
paragraph 1 on a Saturday, Sunday or public holiday, the last day for the transmission
the individual data nearest the next business day.
PART TWO
THE TRANSMISSION OF THE INDIVIDUAL DATA LEGAL PERSONS ENGAGED IN
THE ACTIVITIES OF THE SCHOOL
Article. 2
The transmission of the individual data from the school Registrar's Office and documentation higher
the vocational training school
(1) a basic set of data is to be transmitted on each of higher vocational students
of the school and contains:
and departmental identifier) of the school and the record date,
(b)) the identification of the student:
1. social security number, year and month of birth, sex,
2. the data on nationality,
3. details of the place of residence,
(c) details of any previous education) student:
1. the previous point of application of the newly adopted by the student, for example, the Central
school, College, international school,
2. departmental identifier of the school in which the student earned a secondary education
with the GCSE, the scope of education, year of execution of the graduation exams,
3. highest level of education that the student has achieved before the adoption of the
education in higher vocational school
d) details of the initiation of the appropriate training in higher vocational school
including the date code and begin training in the field of education
and the form of education,
(e) data on their education) in the relevant higher professional school:
1. the date code and their education in the field of education and form
education in higher vocational school
2. an indication of how their education,
3. series and number of the form for the issued certificate of graduation and
Diploma of graduation in higher vocational school
f) information about the higher vocational education:
1. the applicable scope of the education code, a form of education, length of training,
2. the year in which the student educates, total interrupt time
education, including school holidays,
3. identification of the study group,
4. teaching language, foreign language codes, that the student is studying,
5. during the education code, grade, interruption of education,
6. sources of funding the student's education,
g) data on the discharge:
1. indication whether the student successfully or to refrain from doing, discharge date
the holding of the discharge,
2. the code of the holding of the resits, repair or replacement of the test, the defence
3. the code of a foreign language, from which the student is being held,
h) control items:
1. control the resolution of the student or graduate school,
2. the date and change the data type code,
3. the date of expiration of the sentence, the sentence.
(2) for students with special needs, student
which is covered by the study group for students with disabilities,
education of students is based on individual educational plan and
the exceptionally talented students with a basic set of data passes also in
anonymised form with the addition of:

Article. 3
The transmission of the individual data from the school Registrar's Office and documentation

(1) a basic set of data is to be transmitted on each pupil of the Conservatory and

the adoption of the education in the Conservatory,
3. highest level of education, which the pupil has attained prior to the adoption of the
education in the Conservatory,
d) details of the education begins in the Conservatory including
the date code and begin training in the field of education and form
education,
(e) data on their education) in the Conservatory:
1. the date code and their education in the field of education and form
education in the Conservatory,
2. an indication of how their education,
3. series and number of the form for the issued certificate of graduation and
Diploma of graduation in the Conservatory and a certificate of matriculation examination,
f) information about the education in the Conservatory:
1. the applicable scope of the education code, a form of education, length of training,
kind of education,
2. the year in which the pupil educates, total interrupt time
education, including school holidays, the number of completed years of compulsory
attendance,
3. the indication of the education in the second main scope
4. teaching language, foreign language codes, that student is studying,
5. during the education code, grade, interruption of education,
6. sources of financing the education of the pupil,
g) data on the discharge or the matriculation examination:
1. indication whether the pupil succeeded or discharge, or to refrain from doing
graduation exam, date of discharge, or graduation
the tests,
2. the code of the holding of the resits, repair or replacement of the test, the defence
3. the code of a foreign language, from which the pupil takes place the discharge, or

The transmission of the individual data from the school Registrar's Office and the Central documentation
school
(1) a basic set of data is to be transmitted on each pupil secondary school and
contains:
and departmental identifier) of the school and the record date,
(b)) the identification of the pupil:
1. social security number, year and month of birth, sex,
2. the data on nationality,
3. details of the place of residence,
c) details of any previous education pupil:
1. the previous point of application of the newly adopted by the pupil, for example, a primary school,
high school, foreign school,
2. type and departmental identifier of the school in which the pupil is educated before
the adoption of the education in high school
3. highest level of education, which the pupil has attained prior to the adoption of the
education in high school
d) details of the relevant education begins in high school, including
the date code and begin training in the field of education and form
education,
(e) data on their education) in the high school:
1. the date code and their education in the field of education and form
education in high school
2. an indication of how their education,
3. series and number of the form for the issued certificate of final exam and
training sheet or certificate of matriculation examination,
f) information about the education in high school:
1. the applicable scope of the education code, a form of education, length of training,
kind of education,
2. the year in which the pupil educates, total interrupt time
education, including school holidays, the number of completed years of compulsory
attendance,
3. identification of the class,
4. teaching language, foreign language codes, that student is studying,
5. during the education code, grade, interruption of education,
6. sources of financing the education of the pupil,
g) information about the holding of graduation or final exams:
1. the code of the type carried out the tests,
2. indication whether the pupil succeeded or to refrain from doing the exam date
the tests,
3. code examination, and test it in due course, the test in

Transmission of aggregated data
PART THE FIRST
THE METHOD OF TRANSMISSION OF AGGREGATED DATA
Article. 1
Dates of transmission of data, record date, a form of transmission and the
technical resources
(1) the aggregated data pertaining to the school and the school and educational documentation
the device shall be transmitted to the competent administrative authority ^ 5) on the statements, either on the
listinném form, or electronically.
(2) for the purposes of this annex, the present school year means school
year, which falls on the last day for the transmission of aggregated data.
Past school year means the school year immediately
previous to the current school year.
(3) the aggregated data pertaining to the school and school documentation, in accordance
with the other parts of this annex shall transmit to the competent administrative authority on the
listinném form not later than 4. October or electronically no later than
14. on the reference date of October 30. September of the current school year.
Information referred to in article 14(2). 13 shall be transmitted electronically not later than 10. March to
reference date 28. February of the current school year.
(4) the aggregated data pertaining to the school and school documentation
the device shall be transmitted in the case of the educational establishments referred to in:
and) article. 16, 21 to 24 on listinném form not later than 4. October or
electronically, by 14. on the reference date of October 30. September

(b)) article. 15, 17 to 20 on listinném form not later than 4. November
or electronically at the latest by 11. November on the reference date 31 December 2006.

PART TWO
PASSING THE AGGREGATE DATA LEGAL PERSONS CARRYING OUT ACTIVITIES
SCHOOL
Article. 2
Passing the aggregate data of the legal entity performing activities
school
(1) the data for the previous school year: for more education in the middle and upper
vocational school.
(2) the data for the current school year: teachers by type of school, according to the
groups of subjects, teachers in classes or study groups for children,
pupils or students with special educational needs, the head of the
staff by types of schools, other educational staff, classrooms,
use of information technology in education.
(3) the information referred to in paragraphs 1 and 2 shall not be transmitted for elementary art
school and language school with the law of the State language exam.
Article. 3
Transmission of aggregated data from school registers and documentation of schools
(1) for the purposes of forwarding aggregated data in the next text class and
study groups for children, pupils and students with disabilities
disabilities and the class of a school established in a school for the performance
institutional care, protection and education for preventive educational care
referred to as the "special class", or as "special groups", and
school exclusively composed of special classes or special groups such as
"special schools". As a "regular class" or "current group" is marked with the
class or study group, which is referred to in the preceding sentence does not indicate
as a special class or special group. As a "normal school"
marked with at least one normal school class or a common group.
(2) in the case of special school for children, pupils or students with
disabled persons shall also mention of the type of health

(3) in the case that the school has established one or more isolated

individual branch offices, with the exception of basic art schools
and language schools with the law of the State language exam, which
transmitted only aggregated data for the school.
Article. 4
Passing the aggregate data from the school's Registrar's Office and the documentation of the parent
school
(1) the data for the previous school year: departures and arrivals, their attendance
the kindergarten in the breakdown of the normal and special classes, recovery
stays.
(2) the data for the current school year:
and) class and the children by gender, broken down by type of operation and the length of the
attendance and teaching of language and in terms of common and special classes
(b)) a special class and children in them according to the type of disability, or
social disadvantage and the type of operation,
c) individually integrated children with disabilities by type
disability and the type of operation,
d) children by citizenship and aliens under the regime of the stay,
foreigners who do not have access to education under the same conditions as
citizens of the Czech Republic, according to the type of operation in the normal and
special classes
(e)) the age distribution of children according to sex in the breakdown of the normal and special
class
f) application for admission to pre-primary education, which was rejected.
Article. 5
Passing the aggregate data from the school Registrar's Office and the documentation of the basic
school and primary school special
(1) the data for the previous school year: completion of compulsory schooling and the
departures to the secondary schools in the breakdown of normal and special class a
gender, recovery stays, additional indicators.
(2) the data for the current school year:
and pupils according to branches of education),
(b)) class and pupils according to grades and gender in regular class
and special classes
c) pupils repeating grade and students transferring into the upper grades,
(d) teaching in foreign languages) the breakdown of the common class and special class
advanced teaching of some subjects or groups of subjects in the breakdown by
classes and groups and by educational subjects referred to in the relevant
the framework curriculum,
e) optional subjects (compulsory) as optional subjects according to the years
years, teaching courses in a foreign language,
(f)) special classes according to the type of disability or social
handicaps, for whom they are intended,
g) individually integrated pupils with disabilities by type
disability, individual educational plans for students exceptionally gifted and
students with disabilities,
h) pupils fulfilling compulsory education in abroad or in foreign
school on the territory of the United States or in any other way the performance of mandatory
school age prescribed by law according to vintages,
I) age structure of pupils, including the newly admitted to the first year, in
the breakdown of the normal and special classes
j) students according to nationality and foreigners under the regime of the stay,
courses to get to) primary education and courses for the acquisition of the foundations
education, preparatory classes for children with handicaps.
Article. 6
Passing the aggregate data from the school's Registrar's Office and the documentation of the parent
or elementary school in a healthcare facility
(1) the data for the previous school year: the average number of children and pupils.
(2) the data for the current school year: the current number of children and pupils, children and
pupils by citizenship and aliens under the regime of the stay.
Article. 7
Passing the aggregate data from the school Registrar's Office and the Central documentation
school
(1) the data for the previous school year:
and graduates by industry education), the type, length, and forms of education and
by gender,
(b)) prospěchová scholarships, rewards for productive activity,
c) recovery stays and ski training courses,
(d)) in the system of retraining graduates studying disciplines education according to
fields of education and of the length and form of education and by sex.
(2) the data for the current school year:
and vintages,) class, level of education and the type and form of education,
(b)) the pupils by industry education, the type, length, and forms of education and in accordance with
vintages and gender with the earmarking of students with disabilities and students in
daily form education recurring year or transferred to a higher
vintage,
c) pupils newly recruited freshmen by industry education, kind of,
the length and form of education and by sex, and newly recruited students in first
year of daily form of education also by age and prior,
d) pupils learning foreign languages in a regular class and a special
class
e) special classes according to the type of disability or social
handicaps, for whom they are intended,
f) individually integrated pupils with disabilities by type
disability, individual educational plans for students exceptionally gifted,
disabled and other broken down by level of education and type of
education,
(g)) the age distribution of pupils by sex and level of education and the type and
forms of education,
h) students according to nationality, aliens under the regime of the stay,

j) participants of the retraining of study in the system of education in the disciplines

Article. 8
Passing the aggregate data from the school's Registrar's Office and documentation

Article. 9
Passing the aggregate data from the school Registrar's Office and documentation higher

(1) the data for the previous school year: graduates by educational
programs, the length and form of education and gender, remuneration for
education and prospěchová scholarships.
(2) the data for the current school year:
and) study group, and the students according to years and forms of education in the

Article. 10
Passing the aggregate data from the school's Registrar's Office and the documentation of language
the school with the law of the State language exam
(1) the data for the previous school year: students who have passed State language
exam.
(2) the data for the current school year: students under taught languages and
courses, registration number of teaching staff.
Article. 11
Passing the aggregate data from the school Registrar's Office and the documentation of the basic
art schools
(1) the data for the previous school year: depending on the type of study and graduates
artistic disciplines.
(2) the data for the current school year:
and pupils according to the type of study), and artistic disciplines with the hive-off
foreigners who do not have access to education under the same conditions as
citizens of the Czech Republic,
(b) the number of pupils) in each type of study in the music field, broken down

Article. 12
Passing the aggregate data from the school Registrar's Office and the documentation of the basic
school or high school of sports preparation, aimed at teaching class
physical education or advanced teaching of physical education
The data for the current school year:
and) number of sports classes, classes aimed at teaching physical education at
high school classes or groups and arising from the Division of classes on the base
school with extended teaching of physical education, the number of pupils in these classes,
(b) the pupils according to sports),
(c) the registration number of the coaches by) sports.
Article. 13
Passing the aggregate data from the school's Registrar's Office and the documentation of the school
concerning the initiation of compulsory schooling
(1) the data for the current school year: the number of children under
the result of the registration and, where appropriate, by age, by type of health
disability, and the number of disabled children enrolled in basic
schools as forms integration (only for normal primary schools).
(2) the aggregated data referred to in paragraph 1 shall not be transmitted at school when
health care facilities and schools in the facilities for the exercise of the constitutional or
protective care.
PART THREE
PASSING THE AGGREGATE DATA LEGAL PERSONS CARRYING OUT ACTIVITIES
SCHOOL FACILITY
Article. 14
Common provisions
In the case that the educational establishment relying aggregated data referred to in article.
15, 19 and 20 has established one or more detached units, shall transmit to the

Article. 15

(c)) the age distribution of children by gender,
d) children by citizenship, aliens according to the mode of stay, aliens
under the terms of access to educational services,
(e) the registration number of teaching staff) in accordance with the categories.
Article. 17
Passing the aggregate data from the school's Registrar's Office and documentation
for institutional education and protective education
(1) the data for the previous school year: allocation of children, comings and goings
from home or the Institute, obtained the qualification of children.
(2) the data for the current school year:
basic characteristics of children),
b) preschool children,
c) children filling compulsory school attendance,
(d)) kids after their compulsory schooling,
e) disabled children by age group and type of disability,
f) total capacity of home or institution,
(g)), the number of professional staff in accordance with the accounting categories according to education.
Article. 18
Passing the aggregate data from the school Registrar's Office and the documentation centre
free time
(1) the data for the previous school year: occasional activities of interest
education, events and activities of the camp.
(2) the data for the current school year: extracurricular services and activities of the participants
interest services according to age, with the hive-off aliens who do not have access
for educational services under the same conditions as the nationals of the Czech
Republic, registration number of teaching staff by category.
Article. 19
Passing the aggregate data from the school's Registrar's Office and documentation
school feeding
(1) the data for the previous school year: the number of issued and cooked meals in
breakdown on lunch, dinner and supplementary food.
(2) the data for the current school year:
and the boarders by species) enrolled schools and educational institutions with regard to the
age category, with a total earmarking of Diners with foreign
citizenship and the breakdown by type of diet, other subscriptions on a regular basis

b) boarders, for which they are transported into the prepared food dispensaries in
breakdown by types of schools and boarders of the species diet, other subscriptions

c) capacity and the capacity of the preparatory part of the dispenser parts

Article. 21
Passing the aggregate data from the school's Registrar's Office and documentation
pedagogic-psychological counselling
(1) the data for the previous school year: clients, which have received care in
last school year, according to the reason of arrival, school inclusion, and
provided by care, the activities of the educational-psychological counselling in the past
school year, clients who could not be examined.
(2) the data for the current school year: registration number of professional staff
by category and by education.
Article. 22
Passing the aggregate data from the school Registrar's Office and the documentation centre
practical teaching
The data for the current school year:
and) jobs
(b) pupils in total) vintages, disciplines of education, level of education and type of,
the length and form of education, with the hive-off of students with disabilities,
(c)) the pupils according to branches of education and, according to the school, for which the Centre
provides practical training,
(d) the registration number of workers under) categories and by education.
Article. 23
Passing the aggregate data from the school's Registrar's Office and documentation specifically
Educational Centre
(1) the data for the previous school year: clients, which have received care in
last school year, according to disability, school inclusion, and
According to the provided care, specifically educational center in
last school year, the clients, who could not be examined.
(2) the data for the current school year: registration number of professional staff
by category and by education.
Article. 24
Passing the aggregate data from the school Registrar's Office and the documentation centre
educational care
(1) the data for the previous school year: clients in outpatient centres,
day and internátním the Department according to the school's inclusion, with the hive-off
clients with a foreign nationality, the reasons for the arrival by type of care
the initiators of the arrival of the client according to the type of care, activities of the Centre.
(2) the data for the current school year: registration number of workers according to the
selected professional categories and by education, spatial conditions
the Centre, the clients of the Centre according to the forms of care provided.
Article. 25
Passing the aggregate data from the school's Registrar's Office and documentation school
Library
Data for the past school year: library collections, users, borrowing and
other services, location, and facilities of the library, the library staff.
PART FOUR

CARRYING OUT THE ACTIVITIES OF THE SCHOOL OR SCHOOL FACILITY
Article. 26
Transmission of aggregated data from schools and educational institutions,
relating to accidents at work of children, pupils and students
(1) Aggregate data according to the legal person performing activities of a school
with the exception of language schools with the law of the State language examinations and legal
person exercising the activity of school education and accommodation,
school facilities for institutional and protective care and precaution
educational care of school facilities for extracurricular education centre
the on-the-job training and special preparatory school.
(2) the data for the previous school year the number of children, pupils and students and school
the fatality rate of children, pupils and students in schools and educational establishments,
injuries registered in the book-related injuries, indemnified in case. "
