DECREE



The Ministry of agriculture



of 28 June. in March 1996,



laying down the requirements for an application for grant of licence in the forest

economy and details of licensing in forestry

The Ministry of agriculture shall be determined according to § 44 para. 5 of law no 289/1995

Coll., on forests, and amending and supplementing certain laws (forest law):



§ 1



Requirements for an application for grant of licence



(1) if the applicant for the grant of licences a natural person, the application for the grant of

the license includes:



and) name and surname, residence, social security number, and the number of the civil

the card,



(b) the business name of if) under that name in business,



(c)) the identification number, if assigned,



(d) the designation of the subject of the license,) "^ 1") whose grant is sought. (I)

the period for which the licence is requested,



e) officially ověřenou2) a copy of the document to achieve vocational forestry

education,



(f) a description of the existing professional forestry) practice of the applicant, indicating the number of years

the duration of the practice,



g) extract from the criminal register not older than six months.



(2) when the applicant is a legal person, the grant of the licence application for the award

the license includes:



and the business name, registered office), and the legal status of the applicant,



(b)) name, surname and residence of the person or persons who are

the statutory body of the applicant,



(c)) identification number,



d) extract from the commercial register, which must not be older than three months,



e) description of the article, the grant of the licence is applied for. and time,

to which the licence is requested,



(f)) name and surname, residence, social security number, and the number of the civil

responsible representative of the card,



g) ^ 2) certified copy of the document to achieve vocational forestry

education responsible representatives,



(h) a description of the existing professional) forestry practice responsible representatives with

indicating the number of years of practice,



I) extract from the criminal register representative not older than

six months.



§ 2



Professional forestry education



In assessing the conditions for the granting of licences to professional forestry

education is illustrated by the



and diploma of) graduating from the Forestry Faculty of the high

school specialization forest engineering, culminating in a State final

in an examination,



(b) proof of matriculation examination) on the central forestry school, study

the scope of the forestry or forestry.

Legal and natural persons to whom the licence was granted shall keep a register

about the professional tasks and activities carried out on the basis of the granted

the license. The documents relating to these activities are required to

This Decree shall take effect on the date of publication.
