DECREE



of 31 March 2004. October 2002,



establishing a range of knowledge to obtain certificates attesting to the

knowledge hub, the way the tests, as well as the requirements of the application and certificate

(the Decree on the test, the knowledge hub)



The Ministry of health shall, pursuant to section 19 para. 1 (b). h) of the Act

No. 110/1997 Coll. on foodstuffs and tobacco products and about the change and

certain related laws, as amended by law No 146/2002

Coll. (hereinafter the "Act"):



§ 1



Introductory provisions



This Decree regulates the scope of knowledge for obtaining

proof of knowledge of wild and cultivated mushrooms, how to

the tests, the requirements for an application and the certificate attesting to the knowledge of the hub of

which asks the applicant, which puts into circulation or used to produce a freely

growing or cultivated edible mushroom intended for sale to the consumer, or

for further processing for food purposes, or designated, responsible

the representative of the legal person, who is a manufacturer, dealer or

Importer (hereinafter referred to as "the applicant").



§ 2



Requirements for an application



(1) an application for testing knowledge, the applicant submitted to the authority of the hub

Test of knowledge hub



§ 3



(1) during the test, the knowledge hub, the applicant must demonstrate the knowledge of the

the most common poisoning from mushrooms, health symptoms of poisoning from mushrooms and

related policies providing first aid. The list of distinguished

poisonous mushrooms is included in annex 2 of this order.

§ 4



(1) a test of the knowledge hub is performed before the examining Board (hereinafter referred to as

"the Commission"), which establishes the authority to protect public health. The Commission is

three members, its Chairman is a university educated person primarily active in

the area of the protection of health in the field of food safety, at least for a period of 5

years of age.



(2) the protection of public health for at least 14 days before the date of the test

knowledge hub shall inform applicants of the place, day and hour of examination.



(3) the Commission shall act by a majority of votes. If the candidate has succeeded in both written and

the oral part of the examination, the Commission shall evaluate the test result statement "succeeded".

If the applicant in written or oral tests, evaluate

the test result of the Commission's verdict "did not succeed".



§ 5



(1) during the test, the knowledge hub of the Commission shall draw up a Protocol, in which the

shall indicate the

§ 6



Certificate



(1) an applicant who successfully performed the test knowledge hub, the authority

the public health certificate, the requirements are specified in the

Annex 1 to the present Decree.



(2) an applicant who test the knowledge hub has accomplished successfully, can test

repeat at least 2 months.



(3) in connection with the termination of validity of the certificate for the time specified

time since his release [§ 21 para. 10 (b)), c), (d))] can the owner

submit a certificate request to conduct tests in accordance with this Ordinance.



§ 7



The effectiveness of the
