establishing patterns of applications and forms used in the exchange of

operational information with the Member States of the European Union in the framework of the

police cooperation by the authorities of



The customs administration of the Czech Republic Ministry of finance determined in accordance with section 68

paragraph. 3, § 69 para. 3 and § 72 para. 3 of Act No. 17/2012 Coll., the customs

administration of the United States:



§ 1



Model application for the provision of operational information is set out in the annex

No 1.



§ 2



Model of form for the required operational information or communication

the reasons for which cannot be operational information to provide in law

the time limits, or reasons for refusal to provide operational

the information in the time limits laid down by law, is set out in annex 2.



§ 3



This Decree shall enter into force on 1 January 2000. January 1, 2013.
