DECREE
The Czech Mining Authority
of 16 December 2002. October 2002
about the reporting of serious events and hazardous conditions, severe operating
accidents (accidents), serious work-related accidents and malfunctions of the technical
device
The Czech mining Office lays down pursuant to § 6 paragraph 1. 6 (a). (d) Act No. 61/1988)
Coll. on mining activities, explosives and the State Mining Administration, in
amended by laws No. 425/1990 Coll., no. 542/1991 Coll., no. 169/1993 Coll. No.
128/1999 SB., no. 71/2000 Coll., no. 124/2000 Coll., no 315/2001 and no.
206/2002 Coll. (hereinafter the "Act"):
§ 1
In the report under section 6 (1). 3 (b). (b)) of the Act shall indicate
and for serious accidents at work)
1. the name and address of the Organization, "^ 1")
2. the place, date, time and registration number of the accident, ^ 2)
3. the name and surname of the victim (affected),
4. the nature of injury and body part injured
5. nature and source of injury,
(b)) for other major events, dangerous conditions, serious
traffic accidents (accidents) or failure of technical equipment (hereinafter referred to as
"extraordinary event")
1. the name and address of the Organization, "^ 1")
2. place, date and hour when the incident occurred,
3. the type of emergency,
4. probable cause emergencies,
5. estimated the extent of damage.
§ 2
Reporting under section 1 is without prejudice to the obligation to notify the Organization of the ^ 1)
the Office of the district mining accidents at work and to forward the record of the accident according to the
special legislation. ^ 2)

reports and sending a record of injury, accident and record the pattern range of
institutions, which marks the work accident and sends a record of accidents.
