DECREE



of 3 July 2003. August 2012



on the establishment of the list of diseases, conditions or defects, which exclude or restrict the

medical fitness to practise the profession of doctor, dentist,

as a pharmacist, medical and other professional health care professional

the worker, the content of medical examinations and medical terms

opinion (Decree on the medical fitness and healthcare professional

other professional worker)



The Ministry of health shall lay down pursuant to § 37 para. 1 (b). e) of the Act

No. 95/2004 Coll., on conditions for the acquisition and recognition of professional competence

and specialized qualifications for the medical profession of the doctor,

dentist and pharmacist, as amended by Act No. 189/2008 Coll. and under section

90 para. 2 (a). f) Act No. 96/2004 Coll., on conditions for the acquisition and

recognition of the competence for the exercise of paramedical professions and to

the performance of activities related to the provision of health care and amending

certain related laws (the law on the paramedical

occupations), as amended by Act No. 189/2008 Coll.:



§ 1



Introductory provisions



This Decree lays down the list of diseases, conditions or defects, which exclude

or limit the medical requirements for the exercise of the professions of doctor, dentist

doctor, pharmacist, or other health care professional medical

professional worker (hereinafter referred to as "the person concerned"), the content of medical

§ 2



The list of diseases, conditions or defects



The list of diseases, conditions or defects that exclude or limit health

fitness to practise the profession, is listed in Appendix 1 to this

§ 3



The content of medical examinations

(b) evaluation of the data in the anamnestic) medical documentation kept by



1. the person under consideration its medical provider licensing

services in the field of general practical medicine,



2. the provider of occupational health services, where appropriate, the evaluation concluded

the medical report issued by the provider of occupational health services,

If the person concerned is an employee,



(c) physical examination including) comprehensive guidance auditory,

vision and skin and guidance of neurological examination with a focus on

detection of diseases, conditions or defects are listed in annex 1 to this

the Decree,



(d)) the basic chemical examination of urine and

§ 4



Requirements to medical opinion



(1) From the end of the medical report must be clear, whether it is considered

§ 5

Annex 1



The list of diseases, conditions or defects that exclude or limit health

fitness to practise the profession



And.



Diseases, conditions or defects, which are non-medical requirements

professional relationships, and at the same time cause health

complications or deviations negatively affecting the safe provision of

health services



a) clinically important disturbance in attention, memory, intellect, judgment and

orientation, and it



1. dementia-degenerative atroficko of origin, secondary dementia,

organic amnestický syndrome, chronic or recurrent organically

disorders of consciousness,



2. organic personality disorder or conditional



3. clinically severe form of specific personality disorders,

paranoid, schizoid, disociální or emotionally unstable fault

personality,



b) untreated, recurrent or chronic psychotic disorders,



1. schizophrenia,



2. the schizoaffective disorder,



3. trouble with delusions or



4. schizotypní disorder



c) untreated, recurrent or chronic disturbances of mood, and it



1. manic episode



2. major depressive episode or



3. bipolar affective disorder,



(d)) personality disorders or behavioral disorders caused by the effect of the abuse

alcohol, psychoactive substances, pharmaceuticals, or a combination thereof, and



1. the inability to give up alcohol or abuse

psychoactive substances, pharmaceuticals or their combinations influence

immediately and repeatedly the ability of the medical profession,



4. active dependency on alcohol, psychoactive substances, pharmaceuticals

or their combinations,



5. residual or psychotic disorder with late start

induced effect of alcohol, psychoactive substances, pharmaceuticals or their

reminiscence, amnestický syndrome, dementia.

1. acute psychotic disorder with symptoms of schizophrenia or without symptoms

c) mood disorders with satisfactory remission,



1. recurrent major depressive disorder,



2. bipolar affective disorder



3. the dystymie or



4. cyklotymie,



(d)) serious forms of obsessive-compulsive disorder, anxiety fobické

disorder or persistent personality change after catastrophic experience,



e) clinically severe form of specific personality disorders,

histriónská, anankastická, or anxiózní personality disorder,



(f)) personality disorders or behavioral disorders caused by the effect of the alcohol,

psychoactive substances, drugs or a combination of them, especially the addiction

on alcohol or psychoactive substances or drugs or their

combinations; for eligible with restriction can be recognised as

person, if the length of the withdrawal period lasts continuously for at least 2

years; the length of the withdrawal period shall be certified by affidavit

relationships and the doctor performing the alcohol treatment
