on industrial safety

The national security agency determined according to § 15a paragraph 1. 7, § 64, § 75a
paragraph. 4 and section 135 (a). (b)), d), (g)), i) to) Act No. 412/2005 Coll. on
the protection of classified information and security, as amended by
amended, (hereinafter referred to as the "Act"):
§ 1
Patterns in the field of industrial safety
(1) the model application under section 96 paragraph entrepreneurs. 1 of the Act is set out in
Annex 1 to the present Decree.
(2) the model certificate pursuant to § 54 para. 3 of the Act is set out in
Annex No 2 to this Decree.
(3) the model for the extradition of a foreign certificate power pursuant to § 57
paragraph. 2 of the Act is set out in annex 3 to this notice.
(4) the model application for recognition of the security permissions of the entrepreneur according to §
62 para. 2 of the Act is set out in annex 4 to this notice.
(5) the model questionnaire pursuant to section 97 of the Act an entrepreneur is given in annex No.
5 to this Decree.
(6) the model Declaration of waiver of confidentiality under section 96
paragraph. 2 (a). (d)) of the Act is set out in annex 6 to this Ordinance.
(7) the model of the certificate of the entrepreneur according to § 15a of the Act is given in annex
# 7 of this Decree.
§ 2
Justification for the request of the entrepreneur according to § 96 para. 1 of the law
Justification for the request contains data on entrepreneurs
and) line of business in which the entrepreneur wants to access
classified information,
(b)), in the framework of the public contract which required access to classified
information, the contracting authority and the expected duration of the contract
management,
(c)) the concession contract, under which required access to classified
information, the contracting authority and the expected duration of
the award procedure,
(d) a public contract or concession) contract before the start of the award or
the award procedure by which the entrepreneur intends to apply and under which
assumes access to classified information, the contracting authority or the public
the contracting authorities shall award the contract and estimated date of the award or
management,
e) Treaty, or other law, on the basis that it is
access to classified information, the subject matter of this agreement and
providers of classified information,
(f)) of the grounds on which entrepreneurs will emerge a classified
information,
(g)) the circumstances that justify the desired form of access in accordance with section 20

§ 3
Documents to verify compliance with the conditions for the issue of a certificate of the entrepreneur

§ 4
Request of the entrepreneur according to § 96 para. 4 and 5 of the Act
The application of the entrepreneur, filed pursuant to § 96 para. 4 and 5 of the Act an entrepreneur
shall be accompanied by the questionnaire of the entrepreneur, which fills in the identifying information in the

§ 5
The method and form of submission of the request of the entrepreneur

§ 6
Notification of changes in the application range of the entrepreneur
(1) changes to the information in the request entrepreneurs under § 103 para. 2 of the Act is

§ 7
The method and form of submission of notification of changes

§ 8
Safety instructions pursuant to § 75a para. 4 of the law

§ 9
Regulation (EEC)
Shall be repealed:
1. Decree No. 526/2005 Coll., on the establishment of patterns used in the field of
industrial safety and of the lists of documents and their constituent parts
necessary to verify compliance with the conditions for the issue of a certificate of the entrepreneur and of the
method of submission of the application (Decree on industrial safety).
2. Decree No. 11/2008 Coll., amending Decree No. 526/2005 Coll. on
the determination of patterns used in the field of industrial safety and the
lists of documents and their constituent parts necessary to verify compliance with
the conditions for the issue of a certificate of the entrepreneur and the way of application
entrepreneurs (Ordinance on industrial safety).
