and the Ministry of Health, pursuant to § 15 paragraph
. 6 and § 79 para. 6 of the Act no. 361/2003 Coll., On the service of members of security forces
:

§ 1

Types of medical examinations

(1) Types of medical examinations are:

A) entry check

B) periodic inspection,

C) extraordinary tour,

D) exit inspection.

(2) The medical examination under paragraph 1 shall be determined medical condition
citizen who applies for admission to the service, or
member. Part of the medical examination is to determine the clinical history

§ 2


Initial inspection
(1) Initial inspection is performed

A) citizen before taking up duty,

B) in the case of a prior provision of a service position for which

health assessment relating to the current performance
service position.

(2) The entrance examination will proceed in accordance with § 1 para. 2 that

A) in the case of the initial examination pursuant to paragraph 1. a) to perform a comprehensive physical examination
citizen, or other professional examinations
whose scope is given in Annex no. 2, when another legal
regulation or decision of a public health body not otherwise specified
; according to the identified health status of a citizen can do
additional expert examinations, the scope determined by the physician racing facility
preventive care

B) in the case of the initial examination pursuant to paragraph 1. b) or c)
perform a comprehensive physical examination member, or other
expert examinations, the scope determined by the physician racing facility
preventive care, according to the identified health condition of a if

authority to protect public health otherwise stated.

(3) During the examination on changing the career shall apply
results obtained during final inspection under § 5 para. 2 point. b).

§ 3
Periodic inspection


Periodic inspection is conducted once every two years from the date of execution
entry or the last periodic inspection within the scope
comprehensive physical examination, electrocardiogram and other tests

§ 4


Special Tour
(1) Special inspection is carried out to determine and assess
there was any lost or altered medical

(2) Special inspection is carried out when

A) he ordered the Public Health Authority or the State Office for Nuclear Safety
under special laws ^ 1)

B) the performance of the service is interrupted due to illness for more than 8 weeks
or serious injury or illness associated with coma or other
grievous bodily harm,

C) interrupting the performance of the service or the place of service for more than 6 months
.

(3) Special inspection is carried out further

A) on a proposal from the relevant service official security force

If the member has in the performance of services such drawbacks that can
reasonable to assume that there is a change or a loss of health
competence to perform the service position or service

B) on the initiative of a doctor of preventive care facilities, or
doctor if it finds that there has been a change in health status
member, which justifies a change in health fitness,

C) at the request of a member if againt reasonable suspicion that a change
medical or reasonable belief that his health
difficulties arose, or worsen in relation to the health service performance demands
place on which it is classified.

§ 5


Output tour
(1) During final inspection, in order to establish the state of health
member at the time of termination of service shall be performed
medical examinations within the scope of a comprehensive physical examination by a doctor device
preventive care, which provides for appropriate
range of other professional examinations according to the identified health status of a member, unless otherwise
law or by an authority to protect public health
otherwise stated.

(2) Output inspection is done before

A) termination of service,

B) the transfer of a business place, are required to be different

§ 6
Medical assessment


(1) A medical certificate of medical fitness for duty
issued on the basis of medical examinations always contain the information specified in Annexes no.
3 and # 4.

(2) The medical opinion ends later in the day, when it should be done
periodic preventive examinations, or on the day on which he
deadline conducting further medical examination.

§ 7
Efficiency


This Decree shall take effect on the date of entry into force of Law no. 361/2003
Coll., On the service of members of security forces.

Appendix 1


Conditions for defining health classification
Section
I
General


First Medical classification list of diseases, conditions and defects (Section III)
divided according to the health demands of the columns I to III, it is assigned a classification
tag that corresponds to the appropriate medical eligibility
citizen or member for performance service position.
Health classifications reflect the range of capabilities for service in the church
security and are a basis for the expert opinion of medical
. Refer to the appropriate classification mark.
Classification mark "A" expresses his medical fitness to carry
service on-site service, without limitation, the classification symbol "C" indicates
limit his fitness for the performance of services
service location and classification brand " D "expresses his medical
incapacity to perform services on-site service or a long-term
incapacitation for the performance of services.

Second In determining medical grade "C" in Section 3

Third Citizens and members are assessed under the relevant column health
