Instructions for filling out the form of the final report of the occurrence and consequences of serious
crash
Section A: DESCRIPTION Of MAJOR ACCIDENT
Note 1: a type of severe accident
If necessary, specify additional information additional information expressed
Code A1 (a). In cases where it has become more than one fatal accident,
highlight the link between the code numbers of the text in the note.
Note 2: the current list of dangerous substances
Provide a list of hazardous substances as per the safety documentation and mark
the method of their use in an object.
Source materials are those that are in the object used as the default
the raw materials.
Semi-finished products are products which are for use on
being processed.
Normally manufactured products are products that have been produced for
normal conditions or substances brought to the device only for the purpose of
storage [i.e., the desired end products, unwanted, but normally
emerging preparations (by-products) with low commercial value, and
waste without commercial value].
Products derived from a major accident are the dangerous products or
substances, which in theory may arise or already established and accumulated
in the object or equipment in the course of a major accident as a side
products (e.g. dioxin during a fire in 1976 in Sevesu).
Note 3: the list of dangerous substances involved in directly on a serious
crash
List of dangerous substances involved in a major accident, on the
including placing their actually observed or anticipated
quantity.
The actual amount is the quantity of substances proven to be leaked, burnt
or otherwise lost.
The forecast quantity is the maximum possible leaks (loss)
of hazardous substances. It is an absolute quantity, whose loss can be
normal operating conditions to predict and justify, even if this
the amount has not been captured (removed) when the bailout intervention.
Note 4: the list of substances involved in a major accident indirectly on
The list of substances involved in a major accident, indirectly, by including the
they actually observed or forecast quantity. The substance is on the
involved in a major accident as its initiator or involved in its
other stages, but does not directly toxic its dangerous
characteristics (e.g. a flammable or explosive substance may contribute to the
the emergence of fire or explosion, and initiate so toxic gases,
which are the direct cause of poisoning).
Designation of the actual or anticipated amount of has the same meaning as
in note 3.
Note 5: Comment for dangerous substances
Please indicate any special storage conditions or process (e.g., those
in which the cooling, high pressures or temperatures) and indicate
how, in these conditions of use dangerous substances and
explain the important circumstances related to their actual or
the forecast quantity.
Hazardous substances, for which you can reasonably assume that arose or
has accumulated in the course of a major accident or otherwise, that are not elsewhere
classified, should be evaluated (classified) in accordance with the
the relevant regulations and should be read in all of their properties
due to the risk of a major accident.
Note 6: comment to the source of the crash
Specify the encoded information from paragraph A3 (b) focusing on details
about the type of equipment or parts of the wrecked equipment, manufacturing and old age,
time since the last modification or repair, the standards used in the project,
storage or production conditions, whether at the time of a major accident
production or repair, etc. or any other specific
the circumstances.
Note 7: the weather conditions
Comment status, weather conditions, and how were normal because
to the emergence and impact of the major accident, including the meteorological factors in
days prior to a major accident, which could have an effect on its
the emergence of the (persistent rain, droughts, etc.). In the case that
the weather conditions changed dramatically during the course of a major accident,
Please indicate major changes, including the identification of the weather station, the
that observation applies.
Note 8: comment on the causes of serious accidents
Please provide a brief overview of the major events in chronological order, and specify
the basic information given in section and, where possible, of the occurrence and
during a major accident.
Section B: The CONSEQUENCES Of A MAJOR ACCIDENT
Note 9: commentary on the affected area
Specify the information to be able to clarify the scope of hitting the territory.
The extent of damage to buildings or the concentration of the gas cloud (, etc.)
different distances from the center of a major accident is useful to indicate the
mainly in the map, but detailed reflections on the extent of the damage caused by a major
accidents belong in the part of B5.
Note 10: the effect of severe accidents to persons
The overall threat includes persons demonstrably affected the consequences of serious
crash.
Immediate or subsequent deaths include deaths that are directly related to
a major accident.
Treatment of injuries in the hospital applies to injuries, poisoning, chemical
or other Burns, trauma from injury or other mental disabilities
people requiring hospitalisation for more than 24 hours (in cases other than
only the observation/preventive examination).
Other injuries include injuries clearly requiring medical
intervention without hospitalization for a period longer than 24 hours.
Observation includes persons who complain of health problems in the
the context of the action of the manifestations of a major accident and whose health
the State is then observed.
The required information shall be given in so far as they are the operators of the
known or communicated to affected components of integrated rescue
the system.
Note 11: commentary on the effects of a major accident to persons
Specify information B2 (a) and provide the kinds of injuries or other information
which can be used in the assessment of the effects of a major accident (e.g..
the susceptibility of the individual age categories is some kind of injury,
the health status of the people or the place where the injury occurred). If you have not
No person is affected by the health effect, please explain why.
Note 12: comment on the damage to the environment
Section B3 focuses on the environmental aspects of the affected environment.
Specify here the abridged information on the type and amount of environmental damage
(the numbers of killed animals and destroyed crops, the soiled areas,
the volumes of contaminated soil/water, the concentration of pollutants in the
soils/waters, distance extension, pollution affected range
the percentage of the population, affecting the population/territory, the affected species, etc.).
If there were referred to environmental damage, State and
Please explain the reasons why. Any cost of the remedial measures shall be given in
section B5. Please indicate the likely time frame corrective action
(immediate, medium-term to 3 years long).
Note 1: comment on damage to cultural heritage
Please indicate what has been damaged or destroyed, and specify the nature and the value of the
damage. All available information on the costs shall be entered in section B5. In
the case, that there was no damage to the cultural heritage due to severe
crash, please explain why.
Note 2: damage to property
An estimate of damage to property shall be stated in thousand CZK.
Property damage include actually quantified damage to buildings,
vegetation, material or other property (including farm animals,
crop, etc.), which were due to have a causal relationship with severe
accidents. There are included the cost of repairs of buildings, or the cost of
other repair or loss of production. The cost of repair of the buildings and
demolition here indicate only in the event that these costs have a direct impact
on the amount of damage caused.
The cost of intervention and rehabilitation-fixes include the current or estimated
cost of rescue operations and rendering work. In the event that the estimated cost
to repair the buildings was quantified as losses on assets (especially for
the operator) is not considered again here.
Note 3: Comment property damage
Please specify in detail what is included in the damage quantified in section B5. In
the case that cannot be realistically separated from each other for example. property damage from
the cost of the intervention, the highlight it and give the total number. If you have not
incurred costs or details of the costs are not available, please indicate
the reasons why.
Note 4: a comment on intervention into the lives of people living in the vicinity of the object
or device
Please specify in detail the information provided under a reduced point B5 (a) to
(c), for example. the number of people whose life was somehow limited consequences
a major accident, reasons to evacuate people, reasons to supply interruptions
energy and water, and other effects. If no one referred to in points (a) to (c)
compromised, please explain why.
Note 17: comment to discuss the effects of a major accident
Please provide additional information on the impacts that are not included in section (B), and
that may be useful during the analysis of the impact of a significant event.
