Dated 25 January 2016
Amending Decree of the Ministry of Health no. 490/2000 Coll., On
range of knowledge and other conditions to become proficient in some
fields of public health, as amended by Decree no. 472
/ 2006 Sb.
Ministry of Health, pursuant to § 108 paragraph. 1 of Law no.
258/2000 Coll., On protection of public health and amending some
related laws, as amended by Act no. 274/2001 Coll., Act No. .
320/2002 Coll., Act no. 274/2003 Coll., Act no. 392/2005 Coll., Act no. 222/2006 Coll
., Act no. 151/2011 Coll., Act no. 223/2013 Coll. and Act No.
. 267/2015 Coll., To implement § 19 para. 3, § 59 para. 2, § 60 par. 1 and 3 and §
83E par. 2 Act:
Art. I
Decree no. 490/2000 Coll., On the extent of knowledge and other conditions
to become proficient in some fields of public health
, as amended by Decree no. 472/2006 Coll., Is amended as follows:
First In the introduction to the decree, the words "§ 19 para. 4" is replaced by "
§ 19 paragraph. 3 "and the words" § 60 par. 1 and 3, "the words" and § 83E paragraph.
2 ".
Second In § 1, letter c) reads:
"C) the examination board, the test conditions in fields
under a), patterns certificates of professional competence and pattern
certificate of authorization to health risk assessment.".
Third In § 3 para. 1 letter a) reads:
") The name or names, surname, academic title, date of birth and address
place of permanent residence or domicile, unless the person
permanent residence in the Czech Republic."
Fourth Under § 7 the following new § 8, which reads:
"§ 8
Specimen certificate of professional competence pursuant to § 58 par. 1-3 of the Act is
given in Annex no. 5 to this decree. ".
Fifth Under § 11a is inserted § 11b, added:
"§ 11b
Specimen certificate of authorization under § 83E paragraph. 1 Act is shown in Annex
no. 6 hereto. ".
6th After Annex no. 4 is added to the Annex no. 5, which reads:
