About the safety of technical equipment and requirements for technical equipment dedicated
pressure, lifting and gas in mining activities and
activities associated with mining

PART ONE

PRELIMINARY PROVISIONS


§ 1
Subject Matter


(1) Decree

A) establishes requirements to ensure the safety and health at work on technical installations
^ 1) and the safety of their operations (hereinafter
"Occupational Safety and Traffic") in mining activities and activities carried
mining method, ^ 2)

B) provides that technical equipment is considered reserved, and their inclusion in
classes

C) establishes requirements for the production of dedicated technical equipment
pressure,

D) defines the conditions imposed on the competence of organizations ^ 3)
operating dedicated technical equipment, professional competence
their employees and the competence of other individuals.

(2) Decree does not apply to hand tools to drill and hammer drill
with accessories, to electrical technical equipment; 4) and technical
whose operation and control are governed by special laws
regulations. ^ 5)

§ 2


Definitions
For the purposes of this Decree

A) work on technical equipment - installation, removal, operation, repair and maintenance
^ 6) technical equipment

B) inspection - an activity which verifies that the technical equipment
the operating conditions, if not visibly damaged,
shows if an obvious defect and that its operation is not compromised
Occupational Safety and Traffic ,

C) operational documentation - a set of documents of the organization formed by order
inspection, maintenance and testing, local operational safety regulations
^ 7) and operating book; operational documentation also contains
workflows and requirements accompanying documentation ^ 8)
adapted to the local conditions

D) operating book - a book with numbered pages in which
recorded data on the operation, the results of inspections found defects
and remove them and carry out tests and revisions

E) revision - checking the status of technical equipment in terms of safety
work and traffic control and compliance of the actual implementation of the technical
facilities with accompanying or operational documentation and requirements
technical regulations

F) other machinery - the technical equipment used in the
mining activities or activities associated with mining, which is dedicated
technical facilities ^ 9)

G) placing technical equipment into operation - the time after
prescribed revisions of technical equipment or control ^ 10)
to determine its status and influence on the environment and the working environment, and after evaluation

H) test - checking the status of technical equipment according
accompanying documentation, or local traffic safety regulation, whether
equipment is able to fulfill its purpose and that its condition or operation
does not endanger the safety and operation.

Requirements to ensure the safety and operation and the method of verification


§ 3

(1) The organization will issue operational documentation

A) a set of technologically interconnected technical facilities (complex)
technological line with a similar unit,

B) technical equipment operating in hazardous environments or in hazardous conditions
,

C) technical equipment used in operations for which special legislation
^ 11) requires the processing of operational documentation,

D) equipment and capital výpomocného fan and heating mine air,

E) dedicated technical equipment,

F) technical equipment, for which no accompanying documentation.

(2) For technical equipment not specified in paragraph 1 shall have the organization
accompanying documentation and lead a service book. Operating

(3) Order of inspection, maintenance and testing includes:

A) individual acts of inspections, maintenance and tests taking into account
provisions of the applicable technical regulations and operating conditions
technical equipment

B) periodic deadlines to accomplish these tasks and responsibilities for its implementation
,

C) the method of recording the results of inspections and tests, as well as detected and deleted
defects in operational book,

D) periods of regular inspections, if the device is subject to revisions.

(4) Technical equipment after repairs can be operated only after verification that
due to repairs can pose a threat to safety and operation;

(5) Technical equipment must be protected against corrosion. If so stipulated

D) other machinery, that has not been before putting into operation
or reuse inspected or
test

E) other machinery, for which there is no record of inspection or proof of
test not older than 6 months for mobile devices and
12 months for the other devices documented "good status" or "satisfactory
state".

§ 4

(1) Eligibility of technical equipment for safe operation is verified
inspection, examination or revisions that are executed within the deadlines and
extent specified in this decree, if accompanying documentation or
local operating safety regulations writting shorter periods.

(2) Performing inspections pursuant to Annex no. 1 and test
technical equipment according to Annex no. 2 together with the result recorded in his book
operating whoever carried it out.

(3) A separate certificate of testing of technical equipment, in addition to the record in the book
operating in accordance with paragraph 2, draw up the one who made it.

(4) Eligibility dedicated technical equipment for safe operation
in the cases specified in accompanying documentation or in this decree
verifies revisions. Part of the revision is also carried out inspection and test

(5) Responsible worker organizations in the same business day on which

§ 5

Inspection, testing and inspection of technical equipment

(1) If the accompanying documentation specifies the technical inspection device

(2) If the accompanying documentation specifies testing of technical equipment and
their extent, determines the organization, which also designate persons to their

(3) The content and scope of inspection of technical equipment and contents
report on the revision laid down in Annex no. 3; if accompanying documentation determined

(4) The time limits for the testing of technical equipment pursuant to paragraph 2
revision under paragraph 3 provided in Annex no. 4. If
accompanying documentation for the revision provides for shorter deadlines, the deadlines set out in the

(5) The provisions of paragraph 4 shall not apply to dedicated technical equipment of lifting
used only sporadically. Before using this equipment

PART TWO


A dedicated technical equipment of their class

§ 6

Dedicated technical equipment

(1) A dedicated technical equipment of pressure are considered

A) boilers and pressure vessels ^ 14) stable if the maximum working pressure
^ 15) exceeds 0.5 bar ^ 16) and their volume exceeds 10 liters and
product of their maximum allowable pressure and volume exceeds 100 bar.

B) pressure vessels for the transport of gas, if the maximum working pressure
exceeds 0.5 bar.

(2) A dedicated technical equipment lifting to consider lifting equipment
motorized, and it

A) cranes and hoists including chain hoists with lifting capacity exceeding 3200 kg
,

B) mobile working platforms with a lifting height exceeding 3 m

C) construction hoists with lifting height exceeding 3 m, unless they are also
transporting persons

D) elevators with capacity of over 100 kg and a lift height of over 2 m
which are a permanent part of the building,

E) shelf stacking vertically movable operator,

F) vertical transportation equipment in underground structures. ^ 17)

(3) A dedicated technical equipment gas are considered technical equipment


A) the processing or storage of gases

B) for filling gas tanks, as well as for increasing and decreasing pressure
gases

C) for gas consumption by combustion, including gas turbines,

D) for the extraction and storage of gases or liquids in natural rock
structures and in underground spaces (underground storage of gases and liquids
) ^ 18) with the exception of natural rock structures underground
space, wells and probes and their equipment,

E) for gas distribution, with the exception of gas distribution with comprehensive internal
capacity less than or equal to 0.01 m3.

(4) Part of restricted technical devices is their safety and
pressure accessories.

§ 7

The inclusion of dedicated technical equipment in classes

(1) Dedicated technical devices are classified into classes according to the level of risk
work safety and traffic that their operations cause, while
devices with the highest risk are grouped in Class I.

(2) boilers mentioned in § 6 para. 1 as a dedicated technical equipment
pressure classified

A) Class I, if it is a steam boiler with a rated power
steam produced in excess of 115 t. h-1,

B) to Class II, if it is a steam boiler with a rated power
steam produced in excess of 50 tonnes. h-1 or liquid boiler with a heat
capacity exceeding 35 MW,

C) Class III, if it is a steam boiler with a rated power
steam produced 8 t. h-1 or higher or liquid boiler with a heat output of 5.8 MW
or higher

D) Class IV, if it is a steam boiler with a rated power
steam produced less than 8 tons. h-1 or liquid boiler with a heat
output less than 5.8 MW.


(3) pressure vessels specified in § 6 para. 1
as dedicated technical equipment Pressure classified

A) Class I when the operating pressure exceeds 2 bars and a product
their maximum allowable pressure and volume exceeds the 1000 bar. l

B) Class II other pressure vessels stable.

(4) Pressure vessels for transport of gas specified in § 6 para. 1
as dedicated technical equipment Pressure classified

A) Class I, regarding

1.2.1.1, 1.2.1.2 and 1.2.1.3 Annex no. 4

Second pressure vessel type tank, battery-wagon or container designed to transport gas
specified in paragraph 1.2.2.1 of the Annex no. 4

Third Barrel-type pressure vessel designed to transport gases mentioned in paragraph 1.2.3
Annex no. 4

B) to Class II, if it is a pressure vessel mentioned in paragraphs
1.2.1.4, 1.2.2.2 and 1.2.2.3 Annex # 4.

(5) Dedicated technical equipment hoisting classified

A) Class I, in the case of devices referred to in § 6 par. 2 point. a)

B) to Class II in case of equipment referred to in § 6 par. 2 point. b) to f).

(6) Selected Technical Equipment gas are classified

A) Class I, in the case of devices referred to in § 6 par. 3 point. a) to d)
except equipment for gas consumption by combustion
referred to in point b)

B) to Class II in case of equipment referred to in § 6 par. 3 point. e)
and equipment for gas consumption by combustion, including gas turbine
rated power of 70 kW including, if such devices are connected to the distribution
with an overall capacity less than or equal to 0.01 m3.

§ 8

Produce technical equipment dedicated pressure

(1) The production of dedicated technical equipment
pressure is required to comply with the requirements of special legal regulations 19) and production
documentation.

(2) Materials used in the design and construction of dedicated technical

operating conditions. ^ 20)

(3) The production of dedicated technical equipment ensures compressed

§ 9

Requirements for the location of selected technical equipment of pressure

(1) boiler designed for installation in a building object is placed in

§ 10

Requirements for the location of dedicated technical equipment of lifting and operational documentation


(1) Requirements for the location of dedicated technical equipment of lifting

B) conditions for establishing and anchoring devices with regard to load capacity and expected load

§ 11

Requirements for placement of restricted technical equipment gas

(1) The area in which the reserved technical gas equipment
installed permanently, spaces in which are stored or regulating toxic
explosive, flammable or supporting combustion gases and the areas in which || | treated with these agents they must be equipped with fixed lighting.

(2) If the space referred to in paragraph 1, closed rooms must be

§ 12

Testing and inspection of technical equipment

(1) The status of restricted technical equipment is checked and tests
revisions.

(2) The initial review is carried out prior to the reserved
technical equipment into operation, unless a special regulation stipulates otherwise. Default

(9) professionally qualified persons to conduct inspections and tests of dedicated technical equipment
pressure, lifting and gas, including tests

(12) For machinery other before their commissioning
or reuse performed only inspection and test
professionally qualified person according to Annexes 1 and 2.
PART THREE


Proficiency

§ 13
Prerequisites proficiency


(1) competent for production, installation, repair, testing and inspection
dedicated technical equipment and filling gas tanks are organizations

Fulfill the requirements of proficiency in a special legal
regulation ^ 33) which has authorization for these activities. ^ 34) If
mentioned activities carried out in connection with mining activities, the activities carried out by mining method

(2) The application for an authorization under paragraph 1, the applicant shall

B) place of business of an organization or an organizational unit that
organization in the Czech Republic or permanent residence in
enterprising individuals

C) identification number, if assigned, for residential customers
well as date of birth,

D) the name and surname of the person qualified to manage work, including
documents and certificates proving its competence for
activities referred to in paragraph 1

E) the names of other authorized persons, including documents which
competence demonstrate

F) a brief description of the technical equipment, space and organizational
ensure the activities for which they should be issued.

(3) The authorization referred to in paragraph 1 shall be issued unless the applicant demonstrates
that

A) these activities has the necessary technical equipment and facilities,

B) the production and management work on restricted technical equipment has
professionally qualified person or persons

C) is capable of its own activities or contracted
way to ensure the execution of the works, which are necessary or are part of the work in which
requests authorization is issued.

(4) The authorization shall

A) information about the applicant,

B) the subject and scope of activities for which authorization is issued,

C) the registration number of distinguishing features authorization in accordance with Appendix No.
. 5

D) the conditions under which non-compliance with the permissions can change or cancel or suspend his
force

E) the date of authorization.
Authorization shall bear the stamp of the District Mining Authority.

§ 14

Requirements for the competence of individuals to work and other activities
on devices except the inspections and tests

(1) A person is qualified for independent work on the technical equipment
excluding the works and activities referred to in paragraph 2, the natural person
satisfying professional experience and education in accordance with Annex no. 6, which completed a training
where she worked under the supervision of a designated
qualified employee of the manufacturer or operator of the facility, and that
for this activity certificates pursuant to Annex no. 7. If the manufacturer of the device
determine additional competence requirements is to obtain
professional competence necessary to fulfill requirements manufacturer.

(2) Certificate of proficiency is required

A) for the inspection of technical equipment,

B) for servicing of boilers

First automatic with a rated output of steam produced less than 0.5 tons.
H-1,

Second automatic with a thermal output of less than 0.35 MW,

Third with design pressures up to 3 bar and a volume of less than 100 liters,

Fourth flow volume of less than 100 liters,

C) for the operation of dedicated technical equipment of pressure,
dedicated technical equipment of lifting and dedicated technical devices
gas,

D) for servicing, repair and maintenance of machinery other.

(3) Verification of professional competence of individuals to work on a dedicated
technical equipment, with the exception of works and activities referred to in paragraph 2
implementing a written and oral examination before a three-member examining committee
District Mining Authority. The exam board's inspector.

(4) Verification of professional competence of persons to work on the technical
equipment referred to in paragraph 2 shall be carried out by an examination before a three-member Board of Examiners

A) name, surname and date of birth of the applicant,

B) the definition of the scope of certification, and this kind of dedicated technical
equipment, type of activity, type of energy, capacity, performance, pressure, load capacity, etc.

C) the registration certificate number of distinctive features in accordance with Appendix No.

D) the date of issue of the certificate.
The certificate shall bear the stamp of the District Mining Authority.

(7) The certificate may be extended upon written request filed
before its expiry, and based on subsequent successful testing
^ 36) of the certificate holder.

§ 15

Special requirements for professional competence to operate
dedicated technical equipment of pressure - boilers

(1) The holder of a certificate staff ^ 37), boilers (stoker) is authorized to operate the boiler
relevant class or classes with a higher numerical designation, but only
regarding boilers same type of fuel; Operation boilers (stoker)
authorized to operate a fluid boilers is not authorized to operate
steam boilers.

(2) An applicant for a certificate of professional competence to operate boilers
undergo verification prior knowledge according to § 14 paragraph. 3
training in the operation of boilers and relevant class for that type of fuel under the supervision of a stoker in length

A) 80 hours for the boiler class I

B) 48 hours for boilers class II

C) 32 hours for boilers Class III

D) 24 hours for boilers Class IV

Special requirements for professional competence to operate technical equipment dedicated
gas

(1) The condition for obtaining a certificate of professional competence to operate
dedicated technical gas equipment class I
addition to meeting the requirements of § 14 para. 1 is also trained by the organization operating
dedicated technical gas equipment this class or
carrying out the installation or repair of the equipment or the manufacturer of dedicated technical equipment
gas. Training must last at least 10 hours;

§ 18

Special requirements for the competence of persons for the installation and repair of selected technical equipment

assembly and repairs of restricted technical equipment, in addition to the requirements of §
14 paragraph. 1 also trained by organizations carrying out activities in the respective
kind of dedicated technical equipment or
manufacturer of such devices in the range of at least 40 hours with regard to the nature
work.


(2) Verification of professional competence pursuant to § 14 para. 3
for obtaining the certificate referred to in paragraph 1 is also attended by a representative of the respective manufacturer
kind of technical equipment or its authorized representative
organization or organizations conducting the review of the species technical equipment.

§ 19

Requirements for the competence of inspection technology and verifying its proficiency


(1) Requirements for job training and education of the applicant for certification
inspection authority specified in Annex no. 6.

(2) drives the issuance of the certificate of an inspection technique is
written request of the applicant to verify the technical competence to perform inspections and tests
dedicated technical equipment. In the application for verification of professional competence

The certificate shall bear the stamp of the District Mining Authority.

(5) The certificate of inspection technique can be extended based on his
written request submitted before its expiry and on the basis
its subsequent successful testing. ^ 36)
PART FOUR


TRANSITIONAL AND FINAL PROVISIONS

§ 20
Transitional provisions


(1) The condition of technical equipment during inspection, test and revise
time in their reconstruction judged according to legal regulations and technical
standards applicable at the time of commissioning.

(2) For the dedicated technical equipment for which it was before the entry into force of this Decree
set a longer interval for performing revision
than provided for in Annex no. 4, or that existing legislation did not require implementation
revision, the revision carried out within 6 months from the entry into force of this Decree
.

(3) Organizations indicate operating documentation for operating
technical equipment referred to in § 3 para. 1 in accordance with this Decree
later than 6 months after its entry into force. At the same time determine how leadership and
instead of storing this documentation.

(4) A certificate issued before the effective date of this decree shall be valid
scope of their definition.

(5) Persons performing still work on the technical equipment for which
this decree does not require certification, and persons performing yet
inspection of technical equipment after the effective date of this decree
deemed to be complying with the requirements of this Decree .
Organization maintains records of such persons.

(6) Persons performing still working on technical equipment, for which
according to this decree must be a certificate, they may operate without a certificate
carry a maximum period of 6 months from the effective date of this decree
.

§ 21
Repealing provisions


Repealed:

First Proceeds of the Czech Mining Office no. 9/1988 OJ Czechoslovakia dated December 16, 1986,
no. j. 6365/1986, on inspections, revisions and testing of gas equipment
(reg. in the amount of 7/1987 Coll.).

§ 22
Efficiency


This decree comes into force on 1 January 2004, except for provisions
last sentence of § 13 para. 1, which takes effect on the date of entry
Czech Republic into the European Union.

Part A: Filling inspections of technical equipment

When examining the technical equipment is verified:

C) equipment, condition and functionality of safety equipment, such
brakes, emergency stop devices and blocking and guard

meets the requirements of fire protection

E) accessibility for operation and maintenance

F) functionality alarms and status indicators, such as equipment for feedback
message when the remote control switches and light detectors.

Part B: Record of the inspection of technical equipment

Record of inspection of technical equipment includes:

A) the date of the search

B) the result of the inspection (good condition, satisfactory condition, requiring repair)

C) defects and their severity, or a proposal for their
removal

D) the name and signature of the person who carried out the survey

E) where the contractor performed inspection, the stamp indicating the
name and registered organization which carried out the survey.

Part A: Filling testing of technical equipment

First Checking the conformity of the actual implementation of technical equipment with accompanying documentation

pressure equipment at the test overpressure.
Pressure test using a liquid given in the accompanying documentation, otherwise use
water. If the manufacturer does not specify the test pressure higher, do not be
test pressure less than the greater of the following values:

A) pressure corresponding to the maximum load, which may be
pressure equipment subject in service taking into account its maximum allowable pressure
a maximum working temperature of the pressure device, multiplied by the coefficient
1.25, or

B) the maximum allowable pressure multiplied by the coefficient 1.43.

In technically justified cases where the test

manufacturer and serial or inventory number, year of manufacture)

B) date of test,

C) test result (in good condition, satisfactory condition, requiring repair)

D) identified deficiencies

E) the deadline by which it must be carried further test

F) the name and signature of the person who performed the test,

G) if the test is performed contractor, as well as the stamp
with the name and seat of the organization that performed the testing.

Inspection of technical equipment

Part A: The contents and scope of the revision of dedicated technical equipment

I. During the revision is carried out:

First Conformity assessment of the actual implementation of dedicated technical equipment
accompanying documentation and completeness check
accompanying documentation.

Second Assessment of dedicated technical equipment in terms of compliance
conditions for its installation according to the accompanying documentation and its comparison
with the actual condition and whether it is properly labeled.

Third Operations to verify the condition of the revised dedicated technical equipment
including inspection and testing, including functional testing and control elements designed
accompanying documentation to ensure safety; while
determines whether the device during operation complies with the requirements of security
work and operation.

Fourth Functional test equipment dedicated technical listed in the accompanying documentation

Fifth Verification of equipment dedicated technical equipment

II. On initial inspection may also be made:

First Pressure test to verify the strength and tightness of dedicated technical equipment
gas under Annex no. 2 hereof.

Second Function test and the accuracy of security, control,
measuring and control devices, including their function in
simulated faults.

Note:

Test performed by the manufacturer is not required to perform the initial inspection,
if the expiration date of the test laid down by this Decree to perform
further revisions to such an examination.

III. During periodic and extraordinary revision, taking into account the type of revision will further
:

A. with dedicated technical equipment of pressure:

First During periodic revision is verified especially

A) boiler boiler status, including furnaces, equipment pressure unit,
auxiliary equipment and accessories, functional ability to control parts
authenticating devices such as pressure, temperature, level or performance
inspection, testing and maintenance of facilities and equipment, management
documentation, mode of operation in terms of not exceeding the allowable states
and secure access to the boiler,

B) for pressure vessels state pressure vessels, safety kits,
control and locking devices, measuring instruments, implementation
inspections, testing and maintenance of facilities and equipment, documentation,
method of operation in terms of exceedance permissible condition and safe
access to the container.

Second When internal revision

A) boiler identifies and assesses the status of the available walls of the inner and outer surface
boiler

B) the pressure vessel is determined when the container on the inside and outside, the neck including
kits.

Third When the pressure test gas vessels

A) control of the external and internal surfaces of the vessel,

B) pressure test in accordance with Annex no. 2 hereof.

B. with dedicated technical equipment of lifting:

First Checking the accessories and used means of tying, hanging
or grasping loads.

Second In the revision of the test, the test load is also made verification
state of the art equipment including lifting and measuring static and dynamic test
a test load testing including functional elements
accompanying documentation intended to ensure the safety of the revised

A) business name or company name and place of business or business

Fourth Pressure test the hydraulic pressure in accordance with Annex no. 2 of the Decree

welding or riveting, or if the pressure in parts during repair
made holes. The pressure test is also performed when moving
boilers and pressure vessels, if they are not transportable or portable
boiler or if the manufacturer specifies otherwise, and after a stoppage
boilers longer than 1 year, and the pressure vessel
stoppage longer than three years.
