21 NCAC 34B .0501          APPLICATION FOR LICENSE

Applications by

out‑of‑state licensees for licenses pursuant to G.S. 90‑210(b)(1)

shall be made upon forms to be furnished by the Board.  The application must be

verified by the applicant and filed with the Board.
