Who is a qualified provider of assistive technology?









The provider of assistive technology must be an assistive technology vendor contracted with DDA or one of the following professionals contracted with DDA and duly licensed, registered or certified to provide this service:
(1) Occupational therapist;
(2) Physical therapist;
(3) Speech and language pathologist;
(4) Certified music therapist;
(5) Certified recreation therapist;
(6) Audiologist; or
(7) Behavior specialist.
