NOTICE ABOUT HOW INCIDENT REPORTS ARE TO BE MADE
 
 
 
 
This notice is published by Andrew Keith Tongue, First Assistant Secretary, Office of Transport Security, Department of Transport and Regional Services, under
section 107 of the Aviation Transport Security Act 2004.
 
On and from 10 March 2005, reports referred to in paragraphs 104(4)(a) and 105(4)(a) and subsection 106(2) of the Aviation Transport Security Act 2004 must:
 
1.         include the following information:
 
(a)                the aviation industry participant to which the report relates;
(b)               the date and time of the aviation security incident;
(c)                the location of the aviation security incident;
(d)               if the aviation security incident was a threat of unlawful interference with aviation:
(i)                  whether the person reporting the aviation security incident assessed the threat as genuine or as a hoax;
(ii)                whether, at the time of reporting, tracing of the threat was successful, unsuccessful or not attempted;
(iii)               who received the threat;
(iv)              the nature of the threatened unlawful interference with aviation;
(e)                the nature of the aviation security incident;
(f)                 if the aviation security incident involved an aircraft:
(i)                  the aircraft type;
(ii)                the flight number; and
(iii)               whether or not the aircraft was in flight at the time of the aviation security incident;
(g)                if the aviation security incident involved a building, information sufficient to identify the building, such as the building number or other identifier;
(h)                if the person reporting the aviation security incident is aware that the aviation security incident has previously been reported to the Department of Transport and Regional Services:
(i)                  the approximate time at which the aviation security incident was reported; and
(ii)                the name or position of the person or the name of the area in the Department of Transport and Regional Services to whom the incident was reported;
(i)                  if the aviation security incident involved any other aviation industry participants, the other aviation industry participant or participants involved;
(j)                 a description of the aviation security incident;
(k)               whether or not the report is a result of a routine security inspection of the airport;
(l)                  if the report is a result of a security incident notified to the person reporting the aviation security incident:
(i)                  that the report is a result of a security incident notified to the person reporting the aviation security incident; and
(ii)                the name of the person or organisation who notified the person reporting the aviation security incident;
(m)              the name of the person reporting the aviation security incident;
(n)                the title or position of the person reporting the aviation security incident;
(o)               the name of the employer of the person reporting the aviation security incident, where applicable; and
(p)               the date of the report.
 
2.         be made either:
 
(a)                in writing; or
(b)               orally and followed up in writing within 24 hours,
 
to the DOTARS Operations Centre, using one of the following addresses or numbers:
