LODGMENT OF PRIVATE HEALTH INSURANCE INFORMATION IN ACCORDANCE WITH THE PRIVATE HEALTH INSURANCE ACT 2007
 
I, Stephen John Vesperman, Deputy Commissioner of Taxation, make this determination under Section 282-30 of the Private Health Insurance Act 2007.

1.            Revocation of previous instrument
Lodgment of Private Health Insurance Information in Accordance with the Private Health Insurance Act 2007 – F2013L00627, registered on the 8th April 2013, is revoked on the commencement of this determination.
2.            Name of Determination
This determination is the Lodgment of Private Health Insurance Information in Accordance with the Private Health Insurance Act 2007.
3.            Commencement
This determination is taken to commence on 01 June 2013.
4.            Application
In accordance with section 282-30 of the Private Health Insurance Act 2007, I require the Chief Executive Medicare, within 120 days after the end of each financial year, to give me the following information:
(a)     the name, date of birth and residential address of each person who was insured under a complying private health insurance policy for that financial year for private health insurance rebate and/or Medicare Levy Surcharge purposes;
(b)     the name of the health fund that issued the policy;
(ba)   the fund membership number of the policy;
(bb)   the unique personal identifier of each reported person;
(bc)   the identification code of the fund that issued the policy;
(bd)   the type of membership provided by the fund in respect of the policy;
(be)   age rebate percentage;
(c)     where relevant, the share of premiums paid in the financial year to a health fund in respect of the policy;
(d)     where relevant, the share of Australian Government rebate received in respect of the policy;
(e)     where relevant, the share of the total cost of the policy
(f)     the period of complying hospital cover provided in the financial year;
(g)     the total amounts paid by the Chief Executive Medicare under Division 26 for the financial year.
5.            Determination
This determination applies to the Chief Executive Medicare.
