MINISTER OF LABOUR AND SOCIAL POLICY 1)

of 21 December 2009

on the detailed scope of data included in the central registers kept by the Social Insurance Institution

On the basis of art. 33 (1) 5 of the Act of 13 October 1998. o Social Security System (Dz. U. 2009 r. Nr 205, pos. 1585, as of late. (d) the following shall be managed:

§ 1. The Central Register Of Insured Persons referred to in Article 33 (1) 1 point 3 of the Act of 13 October 1998. the social security system, hereinafter referred to as 'the Act', contains the following data:

1) the identification details of the insured: the PESEL number, the type, the series and the number of the document referred to in art. 33 (1) 2 laws, surname, first and middle names, date of birth, family name, nationality, sex;

2) address details of the insured person: check-in, residence, for correspondence: postal code, city, municipality or district, street, house number, office number, post box, as well as other forms of contact: telephone number, e-mail address an electronic, a symbol of the country with a foreign postal code, and for the purposes of payment of benefits-the account number in the bank or in the cooperative savings and credit cages;

3) the data on the subject of insurance: the title of insurance, the type of insurance, the date of entry of the insurance, the date of withdrawal from the insurance, the information for the period until 31 December 2008. employment in specific conditions or in a specific nature and occupation where work is performed in specific conditions or in a specific nature, the working time dimension;

4) data on the degree of disability;

5) data on incapacity for work: total or partial.

§ 2. Central Register of Premium Payers as referred to in Article 33 (1) 1 point 4 of the Act, contains the following data:

1) the identification details of the contribution payer: the NIP number, the REGON number, the PESEL number, the abbreviated name, the type, the series and the number of the document referred to in art. 33 (1) 1 point 2 of the Act, surname, first and second names, date and place of birth, nationality, name or company in accordance with the act of the constitutive entity, the name of the founding authority, the type of entitlement on the basis of which the non-agricultural act is being carried out the activity, the number of the business entitlement, the name of the registration or registration authority issuing the non-agricultural activity, the date of entry in the register or the issue of the non-agricultural holding authority activities, registration number or authority to conduct non-agricultural activities, the date of the obligation to pay social security contributions, the reason for the registration, the date of the registration, the bank account numbers of the payer's contributions;

2) address details of the payer's contributions: type of address (address of business address, residence, for correspondence) and postal code, locality, commune or district, street, house number, office number, post office box, as well as other forms of contact: telephone number, address electronic mail, the symbol of the country together with a foreign postal code, telephone number for teletransmission;

3) data concerning the accounting office of the contributor of contributions: NIP number, REGON number, abbreviated name.

§ 3. Central Register of Members of Open Pension Funds referred to in art. 33 (1) 1 point 5 of the Act, contains the following data:

1) identification data of a member of the open pension fund: PESEL number, type and number of the identity document, surname, first and middle name, date of birth;

2) data on the membership of an open pension fund, hereinafter referred to as "OFE": the date and time of the conclusion of the OFE contract, the manner of conclusion of the OFE contract, the date of deregation with OFE, the OFE ID.

§ 4. Central Register Receiving Pensions from Retirement Pension Funds as referred to in Article 33 (1) 1 point 6 of the Act, contains the following data:

1) the identification details of the person entitled to benefit from the pension funds pension fund: PESEL number, type and number of the identity document, surname, first and middle names, date of birth;

(2) data relating to the provision of a pension fund for life capital pensions: the type of benefit and the symbol of the benefit.

§ 5. Run by the Social Insurance Institution on the basis of art. 33 (1) 1 point 7 of the Act, the Central Register of Family Members of Insured Persons Entitled to Health Insurance, which, including the register mentioned in § 1, serves to cooperate with a specific separate regulation with the branches of the National Health Fund, contains the following data:

1) the identity of the insured family member and the course of insurance: the PESEL number, type, series and number of the identity card or passport, surname, first name, date of birth, degree of kinship or affinity with the insured, degree disability, staying in a common household with the insured person, the date of obtaining and the date of the loss of the health insurance rights by a family member;

2) address details of a family member of the insured person: postal code, city, municipality or district, street, number of the house, number of the premises, symbol of the country together with foreign postal code, telephone number;

3) data concerning the provincial branch of the National Health Fund competent for the family member of the insured person.

§ 6. The Regulation of the Minister of Labour and Social Policy of 12 March 1999 shall be repealed. on the detailed scope of data included in the central registers kept by the Social Insurance Institution (Dz. U. Nr 30, pos. 293).

§ 7. This Regulation shall enter into force on 1 January 2010.

U. Nr. 248, pos. 1485).
