On the nomination of the Secretary of State for Welfare, Health and Culture and our Minister of Justice, done in accordance with our Minister of Internal Affairs of 14 July 1993, DGVgz/GVC/CB/931801,
Having regard to the Articles 14 , 20, third member , and 21, 4th paragraph, of the Special Withdrawals Act in Psychiatous Hospitals ;
The Council of State heard (opinion delivered on 28 October 1993, No W13.93.0457);
Having regard to the further report of the Secretary of State for the aforementioned and done, on behalf of our Minister of Justice and of our Minister of Internal Affairs of 1 November 1993, No CSC/CB 9369;
Have found good and understand:
For the purposes of this Decision, the law shall mean: Law on special drawings in psychiatric hospitals .
1 A petition, as referred to in Article 4 of the Act , in any event, indicate:
(a) the name of the person for whom a provisional authorisation, conditional authorisation or observation licence is requested and the place where it is located;
(b) the name and address of the person submitting the request;
(c) in which the applicant is requested to be the person for whom a provisional authorisation, conditional authorisation or observation period is requested.
2 The first paragraph shall apply mutatis mutandis to a petition as referred to in Article 34g of the Act .
A declaration as referred to in the Articles 5 , 14a, fourth member , 14c, fifth Member , 14h, fourth member , 16, first paragraph , 21, 1st Member , 33, third member , 34 c of the law and a decision as referred to in Article 20, first paragraph, of the Act shall be drawn up in accordance with the model to be established by Our Minister for Health, Welfare and Sport.
This Decision shall enter into force at the time specified in Article 80, second paragraph, of the Act .
This decision may be cited as: Decision administrative provisions Bopz.
Burden and order that this Decision, together with the accompanying note of explanatory note to the State Sheet will be placed.
