" 4. Supplement to annex 3, note 3 by the following: "3. If the professional guidance education subordinated to the Ministry of culture acquired in vocational secondary education institution, created by reorganizing the Ministry of culture, subordinated to vocational secondary education institutions, on page 3 of the supplementary certificate shall indicate the name of the Department (locative), which implement the education program, if the Department has reformed education institutions historical name." 5. Make note of annex 5, as follows: "notes". 1. A certificate shall be issued by the national armed forces, signed by national armed forces Commander. 2.
