1.2 project submission evaluation criteria;

2. The purpose of the activity is to arrange an outpatient health care provider infrastructure, improving the availability of family physicians in municipal areas, thus ensuring the facilities and improving the primary health care provider network.
3. activities within the projects co-financed by the European regional development fund. Activities within the funding available is 3 679 881 lats, including the European regional development fund-3 127 899 dollars and private funds, provided by the applicant in the project – not less than 551 982 lats.
4. Activity implemented an open project selection application form.
5. activities in the implementation of the responsible authority is the Ministry of health (hereinafter referred to as the responsible authority). The responsible authority has the following functions: 5.1 to provide information regarding the preparation and submission of the project to ensure the project application form filling methodology development;
5.2. collect information about the activity according to the legislation of the European Union's structural funds and the cohesion fund management information system design and use;

6. the cooperation body is the economic center of health (hereinafter referred to as cooperation body). The cooperation body is the functional authority. Cooperation institution has the following functions: 6.1. create project submission Evaluation Commission (hereinafter the Commission) by the Commission for participation of the Ministry of health and the health of the economy, the representatives of the Centre the status of members of the Commission, with voting rights, as well as managing authorities and responsible authorities representatives as observers without voting rights, and to develop the Commission's rules;
6.2. develop project submission evaluation procedure;
6.3. the projects proposed for registration submissions, to give them an identification number and immediately after receipt of the application, if the application is lodged by the project personally, or two weeks, if the application be submitted by mail or electronically (signed with a secure electronic signature), a project to issue to the applicant proof of receipt of the application for the project;
6.4. to assess the project submissions and issue administrative acts on the application of the project approval, the approval or rejection of a conditional, if these rules are not complied with the above requirements;
6.5. the development of the draft Treaty and to conclude the contract with the beneficiary of the financing for the implementation of a project;
6.6. to examine health center procurement settlement plan and submit it to the procurement monitoring Bureau;
6.7 collect information about project submissions and projects according to the laws of the European Union's structural funds and the cohesion fund management information system design and use;

9.3. the law on value added tax refund within the eligible cost to be recovered shall not value added tax payments.
10. Financing from the European regional development fund are not eligible costs the following: 10.1 above this rule 9, paragraph cost position;
10.2. What does not satisfy this provision, 18, 19, 20 and 21 of the project referred to in paragraph total eligible cost limitations;
10.3. carried out on the basis of the employment contract;
10.4. not associated with investments in State-paid health care service delivery infrastructure.
11. the project costs not included in the application: 11.1. not specified in this provision in paragraph 9 and 10;
11.2. for which payments are made before the entry into force of these regulations;
11.3. for which payments are made after the expiry of the implementation of the project;
11.4. for which no concluded contract or contracts concluded outside the rules of procurement referred to in paragraph 33;
11.5. resulting from public works, supply or services contract of the incidence.

17.2. the State of paid health care service delivery;
17.3. the calculation of charges;
17.4. the claims payment control and review procedures;
17.5. payment of fees and reimbursement of the overpayment.
18. the applicant Project eligible cost share of total eligible and non-eligible costs (eligible and non eligible cost share) calculated on the percentage by which the project developed infrastructure is designed to use State-funded health care services and other activities. Eligible and non-eligible cost share is determined using the following formula: a = LvalstsN × SN Iata 1990s × 100% where preferential (LvalstsN + LcitsN) × SN Iata – eligible and non-eligible costs, the proportion (in percent);
LvalstsN-the time during which the concerned family doctor practice intended to provide State-paid health care services (hours per week);
LcitsN – the time in which the family doctor practices intended for use in other activities (hours per week);
SN-relevant family medical practice area (square meters);
N-health services to address the existing practices of family physicians (units).
19. the maximum European regional development fund, the aid intensity shall not exceed 85 percent of the total eligible costs of the project.
20. The minimum eligible costs of the activity within a single project is $703, the maximum eligible costs per project-316 262 lats.
21. the project applicant be eligible for eligible costs not exceeding the total of the following indicators: 21.1. table of core funding project – up to 14 000 lats for one project developed a family medical practice;
21.2. the project additional funding on the table of the family doctor practices focusing on one health services address: 21.2.1. up to 2 000 lats for one family doctor practices, if the project is intended to develop the two family physicians practice in one health care service delivery address;
21.2.2. up to 1 500 dollars per family doctor practices, if the project is intended to develop the three family physicians practice in one health care service delivery address;
21.2.3. up to 1 200 dollars per family doctor practices, if the project is intended to develop the four family physicians practice in one health care service delivery address;
21.2.4. up to 1 000 lats for one family doctor practices, if the project is intended to develop the five family doctor practices or more than five family physicians practice in a health care service delivery address.
22. the project application consists of the completed project submission form (annex 1) and the following: 22.1. the project the applicant payer of value added tax, a copy of the certificate of registration, if the applicant is registered with the State revenue service value added tax register of taxable persons;
22.2. the project authorisation of the applicant's identity document certified copies, which the applicant authorizes the project the responsible person to sign the project application and related documentation;
22.3. the documentation certifying the applicant's project: 22.3.1. ownership of the infrastructure in which to invest the European regional development fund, the project applicant is the owner of the infrastructure;
22.3.2. the infrastructure owner (possessor) consent that the applicant will carry out the project, investment in infrastructure and close cooperation with the authority of a contract for the project implementation, determining the responsibilities of the infrastructure owner (possessor) to provide the infrastructure for State-funded health care services for at least five years after the implementation of the project (annex 2), if the infrastructure owner (possessor) is a State or local government.
23. the project applicant project application submitted to the authority of the cooperation in one of the following ways: 23.1. electronic document by sending electronically, signed with a secure electronic signature according to the regulations on the procedure for the circulation of electronic documents;
23.2. the paper form, by submitting personally or by sending by mail. Project application shall be accompanied by an electronic copy of the electronic medium. Project application form and attachments cauršuj (cauraukl) separately.
V. application assessment and Project decision-making 24. the responsible authority shall publish on its website the internet draft application evaluation and assessment methodology, filling out a form and the project application form filling methodology. The liaison body shall publish on its website the internet description of the procedure for the submission of the project evaluation.
25. the project submissions valued under the administrative, compliance, quality and funding criteria (annex 3).
26. the project compliance with administrative applications, eligibility and funding criteria assessed with "Yes" (match), "no" (non-compliant) or ' Yes with conditions "(provided that the applicant carries out cooperation projects in specific activities of the authority in accordance with the provisions of paragraph 31 to the project could properly exercise) if the criterion can be applied to the assessment.
27. the project submission's compliance with the quality criteria assessed by giving it the appropriate number of points. Assessment of the application of the project according to quality criteria is appropriate if the minimum received ratings of at least one point.
28. cooperation Authority assesses project submissions in the following order: 28.1. first project submissions valued in conformity with this provision of the annex referred to in paragraph 1 administrative criteria. If the assessment is in one of the following criteria is "no", the application is rejected and the project evaluation do not continue;
28.2. where the application of this rule 28.1. in the criteria referred to in point rated with "Yes" or "Yes", provided the project submissions valued under the provisions of annex 3, paragraph 2 of the eligibility criteria. If the assessment is in one of the following criteria is "no", the application is rejected and the project evaluation do not continue;
17.6. where the application of this provision in the criteria referred to in paragraph 17.5. evaluates to "Yes" or "Yes", provided the project submissions valued under the provisions of paragraph 3 of annex 3 of the above quality criteria. If the application does not meet the quality evaluation criteria in accordance with the provisions of paragraph 27 of the said conditions, the application is rejected and the project evaluation do not continue;
28.4. If the application complies with this rule 28.3. criteria referred to in accordance with the provisions referred to in paragraph 27 of the conditions of application of the project assessed under the provisions of annex 3, paragraph 4 of the funding criteria. If the rating on this criteria is "no", the application is rejected;
28.5. If the application of these provisions, the criteria referred to in subparagraph 28.4. assessing with "Yes", the application is approved or confirmed by the condition.
29. If two or more project applications have received the same number of points, they ranked the project application priority list in the following order: 29.1. project submissions that received the highest ranking according to annex 3 of these rules referred to in point 3.1., criterion;
29.2. the submission of the project that received the highest rating in accordance with the provisions of annex 3 3.2 referred criterion;
29.3. the application of the project that received the highest ranking according to annex 3 of these regulations 3.3 criterion referred to;
29.4. the application of the project that received the highest ranking according to annex 3 of these regulations 3.4. criterion referred to;

29.5. the application of the project that received the highest ranking according to annex 3 of these rules referred to in point 3.5 criterion;
18.4. the project application, which, according to annex 3 of these rules 1 and 2 of the criteria referred to in paragraph received more rating to "Yes".
30. a decision on the approval of the project application, approval or rejection of a conditional, if these rules are not satisfied those requirements, liaison body within five working days after the decision has been forwarded to the project the applicant by post or in electronic form of a document (signed with a secure electronic signature).
31. If the assessment is in one of these rules and 28.2 28.1.. criteria referred to is "Yes with conditions", requesting the cooperation of the project carried out in the Authority's decisions on project approval with conditions (hereinafter decision to condition), so that it can enter into a contract. 31.2. the financing plan for the project adequate clarification of this provision 3.1.8, 2.2, and 2.4. assessment of the criteria referred to in the instructions, which can affect the quality of the assessment results;
31.3. the project technical clarification of the application in accordance with this provision of the annex 3 1.3, 1.4, 1.5, 1.6, 1.7, 1.8 and 2.4. assessment of the criteria referred to in the instructions, which can affect the quality of the evaluation results.
32. after the project received the cooperation of the authority requesting information on this provision, paragraph 31, the operation, the Commission shall assess the conditions of the decision and the liaison body shall prepare and forward to the opinion no later than 60 calendar days of the rule referred to in paragraph 31 of the decision provided the date of dispatch: 32.1. If a project shall ensure that activities referred to in paragraph 31 of the execution deadline , the liaison body shall prepare a positive opinion on the conditions;
32.2. If the applicant does not provide this provision in paragraph 31, the time limit, the opinion of the authority on the execution of the project is negative and the application shall be deemed to have been rejected.
Vi. implementation of the project activity, 33. based on the information collected by the liaison bodies of approved project applications and volumes of the planned activity (hereinafter information), all purchases and make health organised settlement Center, employing a representative liaison bodies as observers without voting rights. 34. after the conclusion of the contract for the project implementation, cooperation, if it is available in the State budget, on the basis of the beneficiary's written request, provide advance funding for the requested advance payment of up to 20 percent of the European regional development fund, but not more than the amount that the beneficiary can request the payment requests cooperation institution within six months of an advance pursuant to laws and regulations on the procedures for in which the State budget expected to fund the European Union co-financed the project implementation, as well as make payments and preparation of declarations of expenditure.
35. The beneficiary of an individual determines the time the State of paid health care services and other activities that take place within the framework of the project the developed infrastructure and lists this information.
36. The beneficiary country shall be clearly distinguished the paid health services from other activities (and related financial flows). Revenues generated by providing the State paid health care services used to cover only the costs associated with the State of paid health services.
37. the funding recipient project investments made using other activities, provided that other activities are carried out at market price or, if there is no market price, price includes all concerned with the activities of other related costs, without discounts or price reductions.
38. The beneficiary in accordance with the provisions of paragraph 18 of the project is eligible and not included in the eligible cost calculates the proportion of the medium-term prospects, submit a final request for payment and project progress reports for five years following the implementation of the project.
39. If, after approval of the project application, the beneficiary declares that pursuant to paragraph 18 of these rules apply, calculated and not the proportion of the eligible cost of the project does not conform to the information in the application, the cooperation body: 39.1. If possible, retain the European regional development fund co-financing from the ordinary or the final beneficiary for payment;
39.2. If it is not possible for the European regional development fund co-financing in accordance with the provisions of this section, require the 24.3. the beneficiary to repay the European regional development fund co-financing.
40. If, after approval of the project application will change according to the provisions of paragraph 18 for eligible and non-eligible costs, the liaison body of the total eligible costs of the project does not increase.
41. All project eligible costs and not all incidence arising during project implementation, met from private funding provided by the beneficiary.
