The application shall state the grounds for the issue of the certificate, as well as repeatedly under these regulations 4.1 and 4.2 (a) to add photos and documents (if required). 7. In submitting this rule 4 or 6 that application, the person or its authorised representative shall indicate the future for the most desirable way of communication (phone number, e-mail address, or address of the declared place of residence). 8. the certificate is printed on laminated base made certain forms of card with holographic protection.
