Replace paragraph 3, the words "primary health care doctor" with the words "family doctor".
2. To make the first sentence of paragraph 5, the following wording: "the employer is responsible for the storage of personal medical notebook control institution."
3. Replace annex 2:3.1 the words "home address" with the words "the declared place of residence";
