The application shall specify: 2.1. details of the data subject: 2.1.1. name (name);
2.1.2. the surname;
2.1.3. date of birth;
2.1.4. the code (if any);
2.1.5. place of birth;
2.1.6. nationality;
2.1.7. identity document (if any), number, type, the name of the issuing authority, date of issue and date of expiry;
2.2. request the amount of information (information about the data subject, information about the data subject, the recipients of personal data);
2.3. information (personally on arrival at national police authority or agency or send the information to the specified address);
2.4. the individual's first name, last name, ID number (if applicable) and date of birth, if the information is requested by an authorised person. 3. The data subject, submitting the application, confirm your identity by presenting the identity card. The authorised person shall produce notarized authorization, the right to receive information about the data subject, or present a document certifying that the parents, guardians or adoptive parent, guardian. If the application is submitted electronically, the identity of the data subject confirm secure electronic signature. 4. the data subject or his authorized person specified in the application information provided confirms with a signature. If the application is submitted electronically, the veracity of the information certified by a secure electronic signature. 5. the national police or the mission official, when receiving an application, make sure that it is submitted by the data subject or a person authorised by the data subject, and send the application to the Department of State police-SIRENE Bureau of Latvia. 6. The SIRENE Bureau of Latvia shall examine the application and, within one month of receipt of the information requested or refuse to provide information, by sending a letter to the specified address or issued against the signature. If the data subject has specified that the answer want to receive personally on arrival at national police authority or agency, the SIRENE Bureau of Latvia sends the requested information to the respective State police authority or agency. 7. The information requested shall refuse to give (indicating the reason for the refusal) if: 7.1. application of this provision have supplied all the information referred to in subparagraph 2.1, or it is not exact, so disturb the identification of data subjects;
7.2. the provision of the information requested is not permissible under the operation of the Schengen information system. 8. If the provision of the information requested is not permissible under the operation of the Schengen information system or the law it is not stored in the Schengen information system or information system, SIRENE applicant is provided the following response: the contents of the "Schengen information system and Sirene information system there is no such information about you, which should be based on the operation of the Schengen information system in the statutory obligation to provide information." 9. If the data subject or his authorized person refuses to provide the information requested or to respond in accordance with the provisions of paragraph 8, the data subject or his authorized person shall have the right data State Inspectorate for the application of the need to carry out the statutory rights of the data subject.
