JAYAPURA COUNTY COUNTY REGULATION NUMBER 4 IN 2006

ABOUT THE ESTABLISHMENT, ORGANIZATIONAL AND ORGANIZATIONAL ARRANGEMENT

AREA GENERAL HOSPITAL

WITH THE GRACE OF THE ALMIGHTY GOD

BUPATI JAYAPURA, weighed: a. that health is one of the very important conditions in

order to build/form a healthy, intelligent, and strong successor generation;

b. that to implement the Government of Jayapura Government policy in the field of public health services in order to build/form a generation of healthy, intelligent, and powerful successors as in letter a has been built of General Hospital Area;

c. that for the Regional General Hospital as mased on the letter b can be operational then it needs to be formed arrangement of the organization and the general hospital layout of the area specified with the Regional Regulations;

ESTABLISHMENT, ORGANIZATIONAL ARRANGEMENT AND GENERAL HOSPITAL LAYOUT OF THE AREA.

Article 1 In the Regulation of this Region, referred to: 1. Area is Jayapura County. 2. Local Government is the Government of Jayapura Regency. 3. "Bupati is the regent of Jayapura. 4". functional medics. 11. The Nursing Committee is a group of nurses/midwife professions whose members consist of

nurses/midwife. 12. Functional Medik Staff is a group of doctors and dentists who work in the inner installation

functional office. 13. Functional office is the position that displays the task, responsibility, authority, and

the right of a civil servant in order to perform the subject matter and the function of expertise and/or skill to achieve the organization's objectives.

14. Eselon is the structural office level of the Jayapura County General Hospital.

BAB II

FORMATION

Article 2 With the Regulation of the Region is formed the Structure of the Organization and the Working Hospital of the District General Hospital Jayapura.

CHAPTER III, PRINCIPAL AND FUNCTION

Article 3

RSUD is the Technical Institution of the District of Jayapura as the acting element of the Local Government.

Article 4 (1) RSUD has the task of carrying out health service efforts to the community

for useful and successful use By priorizing the recovery efforts, the recovery is performed in a sericable, integrated treatment effort and prevention and implementing referral efforts, in accordance with applicable laws.

(2) RSUD in Performing the task as referred to in paragraph (1) has a function: a. Staging a medical service; b. Hosting Services for Medical and Non-Medical Services; d. Hosting Services; e. Hosting Education and Training; f. Establishment of Research and Development; g. host of the General Administration and Finance.

BAB IV

ORGANIZATION

First Part Structure Organization

Article 5

(1) The RSUD Organization Structure is made up of: a. Director; b. Sub-section of the Business Service Nursing Section; d. Service Section; e. Medical Record and Planning Section; f. Functional Position Group. (2) Bagan Structure of RSUD Organizations as listed on the annex which is

the inseparable part of the Regulation of this Region.

Paragraph 1 Director

Article 6

(1) The RSUD is led by a Chief with a designation The Director is below and

is responsible to the Regent through the Regional Secretary. (2) The Director as referred to in paragraph (1) has the task of leading, compiling

the wisdom, fostering execution, coordinating and supervising the execution of tasks in accordance with applicable laws.

(3) For carrying out such task in paragraph (2), the Director has a function: a. Plan and build a work program; b. organized the execution of a work program; c. coordinate the implementation of the work program; d. oversee, evaluate and analyze the execution of the work program.

Paragraph 2 Sub Section Tata Effort

Section 7

(1) Sub Section of the Effort is led by a Sub-Part Chief who has a task

(2) To carry out the subject task in paragraph (1), the Chief Sub-Section of Tata Business has a function:

a. planning and compiling a work program in the general administration, Workforce,

finance, homes, supplies, and libraries; c. carry out general administrative affairs, employment, finance, home-to-date,

equipment, and libraries; d. coordinate the implementation of general administrative affairs, staffing, finance

fixtures, crowds, and libraries with other working units; e. oversee, control and evaluate the execution of activities in the administrative fields

Paragraph 2 of the Nursing Section

Article 8

(1) The Nursing Section is led by a Section Chief who has a principal task

Paragraph 3 Service Section

Article 9

(1) The Service Section is led by a Section Chief who has a principal task

coordinates all medical services needs and medical support, monitoring, supervision of the use of medical services facilities, medical support and acceptance control as well as patient repatriation.

(2) To carry out such staple duties on paragraph (1), the Chief of Service Section has a function:

a. Build a plan and a work program as a task execution guide; b. carrying out medical services programs, medical care logistics, ethical improvement

profession and medical services quality; c. set up medical care standards, medical care logistics, professional ethics improvement

and the quality of medical services; d. Drafting a placement plan and a medical mutation over the director's approval; e. carrying out education and training; f. Carry out another task given by the leadership.

Paragraph 4 Section of the Medical Reams and Planning

Section 10

(1) The Medical and Planning Recycling Section is headed by a Section Chief who has

the principal task carry out medical records, planning, reporting, law, social marketing and public relations.

(2) To carry out that staple task in paragraph (1), the Chief of the Medical Recam and Planning Section Section has a function:

a. Drafting a RSUD plan and work program as a task execution guide; b. carry out medical record and reporting affairs; c. carry out legal affairs, social marketing and public relations; d. carrying out research and development of health care; e. perform other tasks provided by the leadership.

Second Section Installation

Section 11

(1) Installation is led by a Chief in a functional office appointed and terminated

by the Director as per the provisions In effect. (2) The installation of the Program (1) has the task of assisting the Director in

the implementation of functional services in accordance with the functions of each installation. (3) Installation consists of: a. An outpatient installation; b. An outpatient installation; c. An emergency installation; d. Installation Of Central Surgical; e. Medical Rehabilitation Installation; f. The installation of Radiology; g. Pharmacy installation; h. Installation of nutrition; i. Laboratory installation of the Clintons; j. Hospital Maintenance Installation; k. Sanitary installation and Waste disposal; l. The Laundry and Sterilization installations. (4) To carry out the task in paragraph (2), the Chief Installation has a function: a. carrying out road maintenance and maintenance services as per the

determined RSUD procedure;

. plan, organize and oversee patient care services according to the RSUD predetermined procedure;

c. plan, execute and supervise the emergency services (ER and ICU) in accordance with the provisions and standards that RSUD has specified;

d. plan, execute and supervise the operating and recovery services ' patients according to the RSUD specified standard and procedure;

e. carrying out the physiotherapy ministry according to the standard set RSUD; f. carrying out the rongsent photo service and ultrasound according to the procedure and the standard that

specified RSUD; g. plan for the need, distribution, deviation and drug service according to the procedure

specified RSUD; h. plan, execute and supervision of the feeding and drink of patients; i. plan and carry out the laboratory examination needs and services

according to the procedure and standard set of hospitals; j. Carrying out the maintenance and maintenance of the physical means and swarming of the RSUD; k. plan and oversee waste disposal, hospital environment hygiene

as per the RSUD designated procedure; l. carry out the washing and sterilization of medical and non-medical devices in appropriate standards

CHAPTER V THE MEDIK COMMITTEE, FUNCTIONAL MEDIK STAFF,

AND THE NURSING COMMITTEE

First Section of the Medik Committee

Article 12

(1) The Medik Committee is led by a Chair selected from and by its members who

set out with the Director ' s Decision. (2) The Medik Committee is located below and is responsible to the Director. (3) The Medic Committee has the task of assisting the Director in compiling the standards of the medical ministry,

monitoring the implementation, carrying out the ethics of the ethics of the profession, governing the authority of the protesi staff members of the functional medics and developing the program service.

(4) In carrying out its task the Medik Committee may be assisted by the Medik Committee whose members consist of functional medik staff and other professions ex officio.

(5) Medik Panitia as referred to in verse (4) is a special working group set up to address special issues that set with the Director's Decision.

Second Section

Medic Functional Staff

Article 13

(1) Functional Medic Staff has the task of carrying out diagnosis, treatment, prevention

due to illness, improvement and recovery of health, health care counseling, education and exercise and research and development.

(2) In carrying out the duties of functional medik staff are grouped based on expertise and be responsible to the Chairman of the Medic Committee.

(3) The Functional Medic Staff Group as referred to in paragraph (3) is headed by a Speaker selected from and by a member of his group for the service period 3 (three) years.

(4) Chairman The Functional Medik Staff group was appointed and dismissed by the Director.

Third Part Maintenance Committee

Article 14

(1) The Nursing Committee is under and accountable to the Director. (2) The Nursing Committee is led by a Chair elected from and by its members. (3) The Nursing Committee has a duty to help the Director draw up a standard of nursing,

the coaching of nursing care, and carry out the ethics coaching of the nursing profession. (4) The Chairman of the Nursing Committee is appointed and dismissed by the Director.

(1) The Director is appointed and dismissed by the Regent. (2) The Chief Sub Section and the Section Head are appointed and dismissed by the Regent on the Director's proposal. (3) The Regent may delegate the appointment authority and the dismissal of the Chief Sub Section

and the Section Chief to the Regional Secretary. (4) The Functional Position Group is appointed and dismissed in accordance with the Regulation

applicable laws.

BAB VII TATA WORKFORCE

Article 18

(1) The Director of RSUD in carrying out the mandatory task of organizing coordination with Instancy
