DECIDED:

SET: THE REGULATION OF THE CITY AREA TARAKAN ABOUT THE SERVICE PERMIT LEVY. CONSTRUCTION.

CHAPTER I OF THE GENERAL PROVISIONS

Article 1

In This Region Regulation referred to: 1. An area is a unit of legal society that has boundaries that

2. The Regional Government is the Mayor and the Regional Device as an organizer of the Regional Government.

Article 9 Under the name of the Retribution Of The Construction Services of the Construction Service is levied as payment for the granting of IUJK publishing services.

Article 10 of the Subject of retribution is a private person and or the Agency is getting the IUJK publishing ministry.

CHAPTER VI GROUP LEVY

Article 12 of the construction of the construction services permit is classed as a specific licensing levy in the field of construction services.

FILING REQUIREMENTS IUJK

Section 14 (1) Requirements of Ijin Construction Services Construction:

1. Photocopy of the Construction Services/Consultancy Service Certificate that is still in effect;

2. Photocopy Akte Establishment of the Company; 3. Photocopy of the License of the Place (SITU); 4. Photocopies of the installation and proof of the payment of the tax payment that

still apply; 5. Photocopy Card Members of the Construction Services Association/Consultation are still

applicable; 6. Photocopy of the Subject to Tax (NPWP); 7. Photocopy Card Population (KTP) Director still in effect; 8. Photocopy Card Population (KTP) Technical Power still in effect; 9. Photocopy of Technical Power; 10. Pas Photo Director Size 4 X 6 = 3 Lembar (Blue Color Background); 11. Fit Photo Technical Power 2 X 3 = 3 Lembar (Color Background

CHAPTER IX ORGANIZATION MANAGEMENT AND STAGING

IUJK ISSUANCE

SECTION 15 (1) MANAGEMENT and IUJK publishing organizers are executed by Head of Regions which

is devolve to The Office of Integrated Services (KPPT) of the City of Tarakan; (2) The Office of Integrated Services referred to in paragraph (1) of this section

BAB X

COACHING

Carrying out surveillance in accordance with its authority to

BAB XI

SANCTION OF THE ADMINISTRATION

Article 18 (1) The Regional Chief may provide administrative sanction over the breach in Rule

This area and its implementation rules; (2) Administrative sanctions as referred to paragraph (1) of this section, are;

a. Warning in writing; b. Detention and or retrieval of a permit as an examination material if

is deemed necessary; c. Temporary freeze permit; d. Revocation of the license, if a reprimand is written in writing by 3

(three) times in a row with each 7 (seven) time periods. day;

(3) In terms of mandatory Retribution not pay in time or less pay, the administrative sanction is a fine of 2% (two) percent each month of a debt of debt that is not or underpaid, and billed by using the Regional Retribution Bill (STRD).

BAB XII

P E N G A W AS A N

Article 19 (1) Head of the Regions through the Construction Services Pembina Construction Team did watch

BAB XIII

CRIMINAL provisions

Article 21

g. Making a stop, forbidding a person from leaving the room or a place at the time of examination is underway and checking the identity of the person and or the document as referred to in the letter e;

h. Photographing a person relating to criminal conduct in the field of construction services;

i. Calling people to be heard of his attachment and being examined as a suspect or a witness;

BAB XV

TRANSITIONAL provisions

Article 22 (1) With the enactment of the Regulation of this Region, then the Mayor's Decision Number 20

2003 on the Procedure of the Publishing Procedure of the Construction Services Agreement (SIUJK) of the Procedure For The Construction Of Construction Services (SIUJK). The area of Tarakan Town is declared revoked and does not apply anymore;
