HEALTH POLYTECHNIC PUBLIC SERVICE AGENCY ' S SERVICE RATE
SURABAYA AT THE HEALTH MINISTRY
WITH THE GRACE OF THE ALMIGHTY GOD
FINANCE MINISTER OF THE REPUBLIC OF INDONESIA,
Weigh: a. that under the provisions of Article 9 of the Government Regulation No. 23 of 2005 on the Financial Management of the General Services Agency, the rate of service of the agency that applies the Financial Management of the Public Service Board is set by the Minister of Finance for the proposal menteri/lead agency;
c. that the Minister of Health through the Letter of the Number KU.01.06/III/1134/2011 dated 18 July 2011, has submitted a proposal for the services of the Surabaya Health Polytechnic General Services rate on the Ministry of Health;
D. that the proposed tariffs on the services of the Public Services Board of the Surabaya Health Ministry in the Ministry of Health, have been discussed and studied by the Assessor Team;
e. that under consideration as intended in the letter a, the letter b, the letter c, and the letter d, need to establish the Finance Minister's Regulation on the Tarif of the Public Services Board of the Surabaya Health Ministry in the Ministry of Health;
Remembering: 1. Law Number 17 Year 2003 on State Finance (Indonesian Republic of Indonesia Year 2003 Number 47, Additional Gazette Republic of Indonesia Number 4286);
2. Act No. 1 of 2004 on the State Treasury (State Sheet of Indonesia Year 2004 Number 5, Additional Gazette Republic of Indonesia Number 4355);
3. Government Regulation No. 23 Year 2005 on Financial Management of the Public Service Agency (State Gazette 2005 Number 48, Additional Gazette Republic of Indonesia Number 4502);
4. Presidential Decree No. 56/P of 2010;
DECIDED:
Establish: REGULATION OF THE MINISTER OF FINANCE ABOUT THE TARIFF ON THE SERVICES OF THE GENERAL SERVICES OF SURABAYA ' S HEALTH MINISTRY ON THE MINISTRY OF HEALTH.
Section 1
The Surabaya Health Politecnik Public Services Agency's service rate at the Ministry of Health is in return for service services provided by the Surabaya Health Polytechnic General Services Board of the Ministry of Health.
Section 2
Service rates as referred to in Section 1, consist of:
a. New Student Acceptance Selection Tariff (Sipenmaru);
B. Health Test Rates;
C. Psychological Test Rate;
D. The Introduction tariff of the Student Study Program (PPSM);
e. Student Sign Card Rate (KTM);
f. Promise Ucap Rate;
G. Matriculation Fare;
h. Educational Hosting Donation Rates (SPP);
i. Education Development Fund (DPP) fare;
J. Short Semester Fare;
No, Diploma Company Rate:
I. Graduation rates;
M. English Language fare;
N. Internet fare;
O. Library Fare;
p. Legalize Diplomas Fare;
Q. The Rate Of Legalizing Transcripts;
R. Student Health Service Guarantee Rate (JPKM);
It's Student Dorm Room Rental Rates;
No. Boarding Student Dining Service Rates;
u. Laboratory Rate;
v. Tarif Clinic;
W. Electromedik Engineering tariff; and
x. Competency rates and competency test facilities.
Section 3
The Surabaya Health Polytechnic Public Service rate at the Ministry of Health as referred to in Article 2 is listed in the Attachment that is an inseparable part of the Regulation of this Minister.
Section 4
(1) The Public Services Agency of Surabaya Health at the Ministry of Health may provide service services in the fields of education, research, and community service based on the needs of the service users through cooperation contracts.
(2) The services of the Surabaya Health Polytechnic Public Services Board on the Ministry of Health as referred to in paragraph (1) are set forth in the contract of cooperation between the Director of the Surabaya Health Polytechnic in the Ministry of Health with A service user.
(3) The Director of the Ministry of Health of Surabaya at the Ministry of Health is required to submit a copy of the contract document in cooperation with the service user to the Minister of Health and the Minister of Finance.
Section 5
(1) The Public Services Agency of the Surabaya Health Polytechnic in the Ministry of Health can conduct Operational Cooperation (KSO) with other parties in the framework of service services in the fields of education, research, and devotion to the public.
(2) The Tarif service derived from the KSO with the other party as referred to in paragraph (1) is set under the contract of cooperation between the Surabaya Health Polytechnic Director in the Ministry of Health with the other party.
(3) The Director of the Ministry of Health of Surabaya at the Ministry of Health is required to submit a copy of the contract document with other parties to the Minister of Health and the Minister of Finance.
(4) The KSO as referred to in paragraph (1) does not include KSO of asset utilization.
Section 6
(1) Against students of the Miskin Family (Gakin) may be granted a maximum educational cost assistance of 50% (fifty percent) of the tariff on the Donation of the Education Agency Public Service Education Agency of Health Surabaya at Ministry of Health.
(2) The provision of service rates of 50% (fifty percent) as referred to in paragraph (1) is executed by considering the financial condition of the Surabaya Health Polytechnic General Services Board in the Ministry of Health.
(3) Students of the Miskin Family (Gakin) who may be given a 50% (fifty percent) Charitable Contribution of the Program as referred to in paragraph (1) are determined by the Decree of the Director of the Polytechnic of Health. Surabaya.
Section 7
