THE HEALTH CARE LEVY ON THE PUBLIC HEALTH CENTER

WITH THE GRACE OF GOD ALMIGHTY

MAYOR OF YOGYAKARTA,

Draw: a. that to improve the quality of public health in particular in the areas of health care at the Public Health Center the Government requires a role as well as society, thus becoming a mutual responsibility between the Government and the Community;

b. that under the Law No. 28 Act 2009 on Regional Taxes and Regional Retribution and to improve health care as the letters a above, then Yogyakarta City Regional Regulation No. 5 of 2006 on Retribution Healthcare at the Public Health Center is no longer appropriate, so it needs to be repealed and replaced;

c. that based on consideration as in the letter a and the letter b above, it needs to form the Regional Regulation. about the Health Care Retribution On the Public Health Center;

Set Out: County Regulations On Health Care Levy

AT THE PUBLIC HEALTH CENTER

CHAPTER I OF THE GENERAL PROVISIONS

Article 1

In this Area Regulation is referred to as: 1. The area is the City Region of Yogyakarta. 2. The Local Government is the Government of Yogyakarta City. 3. The mayor is mayor of Yogyakarta. 4. An officer is an employee who is given a specific assignment in the area of the area levy appropriate

7. The next health care levy called retribution is the payment of the health ministry on Puskesmas.

8. A General Services levy is a levy on services provided or provided by the Regions for the purpose of general benefit and benefit and can be enjoyed by a person or person.

CHAPTER II NAMES, OBJECTS AND SUBJECTS OF RETRIBUTION

Section 2

With the Regulation of the County stipulating levies on each health care center in the public health center with the name of a health care levy.

Article 3

(1) The Retribution Object is the health ministry in The library owned and/or managed

by the Local Government, except for the ministry Registration. (2) The type of health care service in Puskesmas as referred to in paragraph (1) includes:

a. a common treatment; b. public health measures; c. health of the mother and child; d. Dental and mouth health measures; e. Other health.

(3) Attribution Subject is a person or body who is granted health care in Puskesmas.

CHAPTER III GROUP LEVY

Section 4

The Health Service Retribution in Puskesmas is classed as General Services Attribution.

CHAPTER IV MEASURES THE RATE OF SERVICE USAGE

Article 5

(1) The basis of the levy of retribution is the level of service of services. (2) The service level of services as referred to in paragraph (1) is based on the factors-

the services of the Sarana Services and Service Services.

CHAPTER V PRINCIPLES AND OBJECTIVES IN PRICING

Section 6

The Principles and objectives in the designation of the magnitude of the levy tariff is based on the Organizational Discretion with regard to the cost of providing services, community capabilities, aspects of justice and control of the ministry.

CHAPTER VI OF THE STRUCTURE AND MAGNITUDE OF THE TARIFF LEVY

Article 7

(1) The structure and magnitude of the levy rate are set with the summation of the Services of the means plus Service Services.

(2) The services of the means as in paragraph (1) are the summation of the Habis Use Medical Materials (BMHP) plus medical and non medical means.

(3) Service services as referred to in paragraph (1) are 35% (thirty-five per hundred) divided by 65% (sixty-five per hundred) multiplied by the services of Sarana.

BAB VII LEVY LEVY

Section 8

(1) Penetration of the levy rate for health care in Puskesmas as such in this Regulation Attachment.

(2) For the Regional Population who may indicate the original Registered Citizen (KTP) Card or the original Family Card, it gets a reduction in retribution by 60% (sixty-one hundred) from the general treatment levy tariff and 25% (twenty-five per hundred) of the action levy tariff.

(3) For residents who follow the Public Health Guarantee, Social Health Guarantee, Regional Health Guarantee and health insurance Otherwise, fully, fully covered by the guarantor, as defined in the paragraph (1) of this section.

(4) For advanced patients the reduction of a levy of 60% (sixty per hundred) of the general treatment levy tariff and 50% (fifty per hundred) of the tariff levy rate.

BAB VIII DISTRICT POLLING

Article 8

Retribution Retribution is levied in the Region region.

BAB IX PAYMENT DETERMINATION, PLACE OF PAYMENT, INSTALLMENT AND DELAY

PAYMENT

Section of Payment Determination

Article 9

(1) The levy cannot be degraded. (2) Retribution is levied by using SKRD or any other document that

is interred.

Article 10 (1) the debted Retribution must be paid paid off. (2) Each payment of the levy is given a valid sign of payment of payment. (3) The pain of the payment is guaranteed by a third party, the retributary levy

Article 11

(1) Payment of retribution for poor families, street children, victims of violence and the equated, and certain groups are charged to the Local Government.

Second Section

The Payment Place

Article 12

(1) The payment of retribution is in place of service services/facilities or a locket that has been determined by the Mayor or the designated official.

(2) Place Payment is defined as the following: a. General public who don't have a health guarantee, then the payment

is done in Puskesmas; b. General public that has a health guarantee, then the payment is appropriate

with applicable laws.

CHAPTER X LEVY AND MOMENT OF DEBT

Article 14

CHAPTER XI ASSIGNMENT RETRIBUTION

Article 16

BAB XII

REDUCTION, LENIATION AND LIBERATION OF RETRIBUTION

Article 17

(2) Reduction, leniation and exemption of retribution are provided with regard to the Compulsory Retribution.

(3) The terms and terms of the reduction, leniation and release of retribution are further regulated by Mayor's rule.

BAB XIII LEVY MANAGEMENT

Article 18 (1) Health Service Retribution to Puskesmas is returned by 100% (one hundred

per hundred) to the Regional Device Workforce authorized in the field of health.

BAB XIV

SANCTION ADMINISTRATION

BAB XVII

INQUIRY section 23

f. Order to stop and/or prohibit a person from leaving the room or place at the time of examination in progress and checking the identity of the person, objects, and/or documents that are brought;

g. Photographing someone with regards to the criminal Retribution. call people to be heard of his description and be examined as a suspect

or a witness; stop the investigation; and/or i. Stop the investigation; and/or j. conduct other actions that need to be subject to the investigation of a criminal action in

the field of Attribution in accordance with the provisions of the laws. (4) Investigators as referred to in paragraph (1) notify the start of

the inquiry and convey the results of its inquiry to the Public Prosecutor through the Investigator of the State Police of the Republic of Indonesia, in accordance with the provisions set forth in the The Criminal Event Law Act.

Article 24

In addition to the General Investigator, the investigation of the criminal acts as referred to in Article 20 is carried out by the Civil Servlet Investigator (PPNS) in the Regional Government environment.

BAB XVIII CRIMINAL provisions

ABOUT

THE HEALTH CARE LEVY ON THE PUBLIC HEALTH CENTER I. GENERAL EXPLANATION Indicators the success of health development is improving the health degree

society with (1) Improving effective and efficient health care quality, (2) Affordable health care costs, (3) structuring health efforts with role as well as the public for healthy living. Efforts to improve service quality by increasing the cost of the Puskesmas operational cost, then the analysis of the cost of service in Puskesmas is based on the Real (Cost Unit).

Relationship with that and the addition of new types of services as well as new facilities

at the Public Health Center, then to improve health care above Yogyakarta City Regional Regulation Number 5 of 2006 on The Health Care levy On Puskesmas needs to be adjusted to the current situation and conditions.

For those purposes the Government of Yogyakarta is taking policy with

review of the Regional Regulation above to be customized to the progress demands of the state.

II. THE EXPLANATION OF THE ARTICLE BY ARTICLE
