WITH THE GRACE OF GOD ALMIGHTY

REPUBLIC OF INDONESIA HEALTH MINISTER,

DRAWS: A. that in order to improve the access and quality of poor public health services, it has organized a public health guarantee program (Jamkesmas);

b. that the Jamkesmas program has undergone extensive expansion, including the expansion of the membership in which the granting of health care to the sufferers of Thalassaemia Major;

c. that the Major Thalassaemia is a hereditary disease that has not yet been found. The cure, and because of the disease, may impoverde the family sufferers;

DECIDED:

SET: A HEALTH MINISTER ' S REGULATION ON TECHNICAL INSTRUCTIONS OF THE WARRANTY OF TREATMENT SERVICES. THALASSAEMIA.

Section 1

(1) Thalassaemia Medical Guarantee Technical Guidance is a reference to the Central Government, Provincial Government, District/City Government and related parties including the Thalassaemia Foundation (YTI), the Association of Parents Sufferers of Thalassaemia Indonesia (POPTI) and Hospital in hosting the Thalassaemia Treatment Program.

(2) Thalassaemia Medical Guarantee Technical Guidance is referred to in paragraph (1), as set forth in the Regulation Attachment This.

Article 2

The Medical Services Guarantee Thalassaemia aims at:

1. Improve access and treatment services for people with Thalassaemia Major including the poor and unable to outside the participants of the Jamkesmas Program;

2. The implementation of the referral service process for the sufferer Thalassaemia Major to the referral health care facility;

3. The financing mechanism is the financing mechanism for the treatment of the Thalassaemia Major treatment service.

Article 3

(1) Warranty Service Warranty of the Thalassaemia Major Program is part of the Jamkesmas Program.

(2) The Severing Warranty Service of Thalassaemia Major constitutes an extension of the membership of the Jamkesmas, which is integrated and managed to follow Jamkesmas governance and management.

Article 4

This Regulation is beginning to apply at the promultable date.

WARRANTY OF TREATMENT SERVICE THALASSAEMIA

BAB I

PRELUDE

A. BACKGROUND

Basic Law 1945 article 28 H paragraph (1) mandates that every person is entitled to a prosperous life born and inner, residence, and get a good and healthy living environment and entitled to a service health, and in section 34 verse (3) it is stated that the state is responsible for the provision of proper health care facilities and public service facilities.

In the Law Number 36 of 2009 on Health, exactly in the section 5 paragraphs (1) states that each person has the same rights in obtaining access to the IBM Cloud Service. The resource in the field of health and the paragraph (2) is listed that everyone has a right to secure safe, quality, and affordable health care.

In the course of the conduct of the Base Act and the Act, The Ministry of Health organizes the Public Health Security Program (Jamkesmas) which provides Health Guarantee for the entire poor and inable society. The Jamkesmas program is a breakthrough program that has the goal of eliminating the barriers of health financing for the poor and the unable, so that they are able to get health care according to their medical needs.

It is known that many diseases require huge costs so that it can make the person poor (impoverishment), this problem will become more severe when it occurs in the type of decreased disease (genetic) that continues. It took a lot of money from being born, like Thalassaemia. The disease is not a disease of the wrong society ' s life patterns, but rather a disease

descendants.

There are two types of Thalassaemia that are Thalassaemia Minor and a Thalassaemia Major. Type Thalassaemia minor does not interfere with the health of the suffering but if married to fellow Thalassaemia minor then 25% may decrease the Thalassaemia Mayor disease to his son. Thalassaemia Major is a serious blood disease and there is no cure yet, so to maintain his stamina and health should be done on a regular transfusion of blood since children, and to survive they require huge costs.

The financing of services and treatment for Thalassaemia major sufferers during this time being the burden of families in the Indonesian Thalassaemia Parents Association and for the next time called POPTI, as well as assisted living in the society of the Indonesian National Association. by the Indonesian Thalassaemia Foundation and for the next time it is called YTI. Based on these matters above then the Government through the Ministry of Health provides special treatment assistance for the treatment services Thalassaemia major served through the Public Health Guarantee Program.

To Provide a good service, and, under the Technical Support Handbook of the Thalassaemia Medicine Service, as the technical description of the Public Health Assurance Manual in the treatment services of Thalassaemia. This technical guidance is an integral part of the Public Health Guarantee Guidelines (Manlak).

B. PURPOSE

The Thalassaemia Treatment Service Guarantee program has the following objectives:

1. General Purpose

It ranks access and quality of the treatment services of Thalassaemia to achieve optimal and efficient health degrees through Thalassaemia Treatment Service as a single unit of Health Warranty Program Society.

2. Special Purpose:

a. Provide ease and access of services and treatment for sufferers Thalassaemia at the hospital;

b. Provide quality control of the health service through increased service and treatment of Thalassaemia which is optimal.

c. Organizes health care costs through transparent financial management and accountable.

C. OBJECTIVES

The Thalassaemia Treatment Service Guarantee target is all Thalassaemia Major in Indonesia that has already been diagnosed with the diagnosis.

BAB II

HOSTING TREATMENT

MINISTRY THALASSAEMIA

The treatment of the Thalassaemia Treatment Service is made up of the Warranty Service Recipient and Tatalaksana Medical Services Thalassaemia.

A.

TATALAKSANA THE WARRANTY SERVICE RECIPIENT OF THALASSAEMIA IS MADE UP OF THE WARRANTY OF THE WARRANTY RECIPIENT, THE PROCEDURE TO OBTAIN THE THALASSAEMIA TREATMENT MINISTRY, AS WELL AS THE RIGHTS AND OBLIGATIONS OF ANY WORKING PARTNER

.

1. Warranty Exemption

Service and Thalassaemia Warranty Recipients include:

a. any person who has upheld a positive diagnosis suffers from a Thalassaemia major and has had a Member Card issued Central YTI or whose name has been listed in the Data Base Thalassaemia Foundation Indonesia

b. One who has upheld a positive diagnosis suffers from a Thalassaemia major, but the sufferer has not yet been registered with YTI.

The Indonesian Thalassaemia Foundation reports a database of all Thalassaemia Medical Service Recipients (by name, by address) to the Health Care and Financing Center, General Secretariat of Health through the Manager Team Central Jamkesmas.

2. Procedure To Get The Major Thalassaemia Treatment Service Major

a. All Thalassaemia Service Medical Guarantee recipients are entitled to the Major Thalassaemia Treatment Service, with

first was already listed on the Central YTI and it has had a Member Card published by the Central YTI.

b. In the case of a new Thalassaemia sufferer who is diagnosed by a doctor in the hospital as a Thalassaemia Major, then the person may apply for the Treatment Ministry of Thalassaemia with the same person as the Thalassaemia Hospital. The show is as follows:

1) Contact the nearby YTI/POPTI branch and apply for a Member Card.

2) YTI/POPTI branch will forward the case of the Thalassaemia sufferer to the Central YTI.

3) If the plea approved, then the Central YTI will publish the Member Card

c. For those who do not have a Member Card for Thalassaemia Medical Services, can obtain the treatment of the Thalasaemia Service by using a letter of recommendation of the Director of the local Hospital and then immediately take care of the member's card. on Central/POPTI YTI.

d. For those who do not yet have a Member Card for Thalassaemia Medical Services, but have been listed in the YTI Base Data, then the Sufferer can obtain the Major Thalassaemia Treatment Service by bringing a letter of recommendation that temporarily enforced from YTI/POPTI local branch, before getting the Member Card for the Thalassaemia Treatment Service.

e. In the event of missing Member Card for Thalassaemia Treatment Service, the following steps are performed:

1) the sufferer must report to the nearest YTI/POPTI or the nearest YTI/POPTI to further be done Base Data Security as a registered person,

2) The nearest YTI/POPTI or the nearest YTI/POPTI are obligated to publish a note letter that is correct as sufferers,

3) YTI/POPTI Branch submitted the request to YTI Central to immediately be published a new Member Card.

f. For those who have died, they will automatically be lost and cannot be transferred to another person.

g. For those who abuse the Member Card for this Thalassaemia Treatment Service, then it will be legally sanctioned in accordance with the provisions of the applicable Perundang Rules Act.

3. Rights and Liability of any Working Partner

Given the implementation of the Thalassaemia Treatment Service is a Partnership involving YTI/POPTI, then for sufferers not Jamkesmas participants, procedures and mechanisms are certainly more complex Compared to the Jamkesmas program. In order to guarantee the solubility of the Thalassaemia pain treatment, it needs to be poured on the Rights and Obligability of each associated partner, in order for the implementation of the program to be optimal.

a. Sufferer thalassaemia Major

1) The sufferer Thalassaemia Major who has served his diagnosis has the right to obtain Thalassaemia Treatment Service in accordance with the standards of medical services.

2) For the treatment and treatment of the disease outside Thalassaemia Major ' s diagnosis, then:

a. For Sufferers Thalassaemia Major and Participant Jamkesmas, it remains FINANCED Jamkesmas with the payment pattern following (INA-DRG ./INA-CBG ' s).

b. For the sufferer thalassaemia Major and not the Jamkesmas participant, NOT FINANCED by the Government.

4) The drug-mediated Thalassaemia Major suffering from Jamkesmas is not charged at crowdfunding, the entire cost of his health care is financed from the APBN fund.

b. Liabilities of the sufferer thalassaemia major:

1) The sufferer who has been upheld the diagnosis as a sufferer of Thalassaemia Major is obliged to report and register to YTI/POPTI Branch. (the YTI and POPTI secretariat addresses are attached).

2) Major Thalassaemia Suffers who have obtained Thalassaemia Medical Services are required to exercise the advice of doctors, by means of a regular transfusion of blood, taking medication and the use of an orderly and orderly manner of iron-iron.

c. YTI/POPTI's obligation:

d. Hospital Rights and Liability:

2) reserves the right to receive the Base Data sufferer Thalassaemia Major from YTI/POPTI.

3) is obligated to provide services and treatment of Thalassaemia Major's sufferers as per medical standard, by using drugs recommended effectively and efficiently.

4) is obligated to provide medicines and health tools in full appropriate that are prescribed by the doctor.

5) Prior to the establishment of YTI/POPTI affairs in the Director/Head of the Ministry of services in the Hospital are charged as a decision-taker to be able to Provide a recommendation in a diagnosed person/patient. Thalassaemia on behalf of YTI/POPTI, and further the Hospital must immediately send a copy of the recommendation to the Central Indonesian Thalassaemia Foundation to be created Member Card.

6) is obliged to perform verification and publish SJP. (Treatment of Maintenance) for Thalassaemia Major, who is not a Jamkesmas participant.

e. The rights and obligations of the Verificator of the Order (PT Askes) include:

PT Askes is obliged to verify and publish the SKP for the sufferer Thalassaemia Major who is the participant of Jamkesmas.

B. THERE IS A TREATMENT SERVICE

1. The sufferer Thalassaemia Major visited the Hospital that worked together in the Jamkesmas program at the time of the need for health care.

2. All sufferers of thalassaemia major should be able to show proof of identity of a Jamkesmas card and/or YTI Member Card.

3. For new people and not yet to have a YTI card, it can use the recommendation letter from the YTI branch office or the Director of the Hospital.

6. Reserved Service Type

a. The type of service that is guaranteed a Thassaemia service treatment guarantee program is a treatment ministry that requires Rawat Road and Rawat Inap

For hospitalization is a service in class III of the Government and Private Hospital which has a Cooperation Agreement (PKS) with Jamkesmas. 7. The treatment services provided for all sufferers/patients thalassaemia major both patients/patients Jamkesmas and non Jamkesmas must receive treatment services including;

a. Consulting

b. Laboratory examination consisting of complete Blood, Edge Blood, Ureum, Kreatin, SGOT, and SGPT.

c. Health and Mental Items Wear provided are Spuit 3 ml, 10ml, Blood Filter, Blood Set, Abocath no. 22, NaCL 100 ml, Alcohol Swab, Plester, wingneedle no. 27, aquabides 25 ml;

d. The drugs include 5 mg folic acid, 80 mg, Vit E Tab 200 IU, Vit C 100 mg, Xylocain Jelly

e. Blood transfusion, complete with transfusion device and blood

f. Iron dilation drugs, which can be Suntic Iron Kelation (DFO), Oral Iron Kelation (DFP) and Oral Iron Kelation (DFX) in accordance with a good fit of acceptance in the body of the sufferer, must be provided at once for the need/use in one month.

g. The feritin check must be carried out every three months.

BAB III

FINANCING MECHANISM

The Thalassaemia Service treatment financing mechanism includes the magnitude of the Thalassaemia treatment service rate, allocation and source of funds, as well as nomenclactic Funding liability.

A. Thalassaemia Service Treatment Rate

For the ministry of Thalassaemia is assigned laboratory examination services, drugs, and blood transfusation.

The Tarif used in the responsibility of the sufferer claims Thalassaemia outside the INA-CBG ' s package.

B. Source and Alocation Dana

The financing source for the entire Thalassaemia service sourced from APBN and its budget poured in One DIPA joined the Jamkesmas Program.

C. The responsibility of the Fund's responsibility

A. SUCCESS INDICATOR

As a basis in assessing the success and achievement in the implementation of the "Thalassaemia" warranty, it is used with indicators:

His Terlayman is all Thalassaemia sufferers who are doing the treatment in Health facilitation that works closely with the Jamkesmas program.

B. MONITORING AND EVALUATION

1. Goal

Monitoring needs to be done to get an overview of the suitability between program plans and execution in the field, while the evaluation aims at the achievement of the success indicators.

2. Scope

a. Participant data, logging, and complaint handling

b. The implementation of the Thalassaemia service which includes the number of visits to the Secondary and Tertiary PPK

c. Quality of service execution to the sufferer Thalassaemia

d. The implementation of the funding and verification of the fund's liability

e. Implementation of verification of the use of program funds

f. Program management at the provincial/kabupaten/city level

3. The mechanism

Monitoring and evaluation is performed regularly in both monthly, quarterly, semesters and annual by the Centre and provincial health services/kabupaten/city via:

a. Meeting and coordination

b. Data processing and analysis

c. Supervision

C. HANDLING COMPLAINT

Delivery of complaint is useful as a control for program improvement. The delivery of the complaint can be delivered by participants, unifying, and PPK officers to the program managers in the Health Service both provincial and district level/City with regard to principles:

1. The complaint must be responded quickly and accurately

2. Complaints of complaint can guide the available units on PPK as well as the local Health Service

3. The handling of the complaint is done in a single level

D. COACHING AND SUPERVISION

1. Coaching for the purpose of performing the program is more useful and successful. Coaching is done in accordance with the tasks and functions of the following:

a. Coaching in the preparation of the POA program

b. The execution of a program is performed on the field

c. Coaching in the verification process

d. Coaching in the management information system process

2. Supervision is performed:

a. Surveillance is attached

b. Functional oversight

c. Public oversight

E. LOGGING, REPORTING, AND FEEDBACK

In support of monitoring and evaluation required logging and reporting execution of the program on a regular monthly.

The results of program service activities are performed by PPK on the existing record registers.

2. Reporting

a. PPK is required to report the recapitulation of the program to the County/City Health Service on the 5th of each month.

b. The county/city health service is required to resurrender reports from all reports of the results of the program's execution in the local district/city area and report it to the provincial health service every day of 10 each month.

c. The Provincial Health Service is required to rekapitulate the report of the activities of each District/City health service and report it to the Centre every 15 of each month.

d. The Central Manager is required to resurrender reports from each province to become a national report every month/trimester/semester/year.

3. Feedback

CLOSURE
