BUPATI BALANGAN, weighed:

a. that the construction of a construction service is part of the development activities related to the planning and fulfillment of infrastructure facilities to support economic, social and cultural growth;

CONSTRUCTION SERVICES.

CHAPTER I OF THE GENERAL PROVISION

Article 1

In this Regional Regulation referred to:

1. The area is Balangan County.

2. The Regional Government is the Regent and the area's device as an element of the regional government's organizer.

3. " Bupati is the regent of Balangan.

15. Qualifying is a part of the registration activities to establish the business class in the field of construction services according to the level/depth of the competency and the ability of the skills, or the skill profession of the individual person's work and work skills. 16. Coaching is the set-up activities, empowerment, and supervision of the Local Government for service providers, service users and the public.

17. The Institute is a Construction Services Development Institute which is next called LPJK, in accordance with the mandate of Government Regulation No. 4 of 2010 on the Changes to Government Regulation No. 28 of 2000 on the Effort and the Role of the Community of Services Construction.

CHAPTER II CONSTRUCTION SERVICES

Article 2

The construction of the construction services consists of an individual effort and an enterprise entity.

Article 3

Concurrent Construction Services permit

Section 5

Any business entity that carries out the work of construction planners, constructs, and construction supervision in the area is required to have an IUJK.

Section 6

(1) The Regent publishes IUJK for the services business entity

domiciled construction in the Region.

(2) The regent may bestow its authority as referred to in paragraph (1) by pointing Officials on the SKPD are certain as IUJK publishers.

(3) IUJK as referred to in paragraph (1) applies to carrying out construction services activities across the Republic of Indonesia.

Article 7

(1) IUJK is given in the form of a signed certificate

Section 8

IUJK is given after obtaining a letter of recommendation and assessment from SKPD which breeding construction.

Article 9 (1) The recommendation as referred to in Article 8 only

can be provided to BUJK which at least meets the criteria:

a. have SBU, SKA, and/or SKT issued by the Board of Certification Of The Accreditation Association of Construction Services;

b. the location of the BUJK office in accordance with the domicile certificate;

c. The BUJK in question is not being sanctioned; and

d. The BUJK is not on the blacklist.

(2) In giving a recommendation, it is mandatory for field verification first regarding the truth of the effort.

(4) The form and content of the letter of recommendation as referred to

The Second Part of the IUJK Classification and Qualification

Article 10

(1) the IUJK grant must list the classification and

the qualifying services of the construction services.

(2) The classification and qualification of the construction of the construction services as referred to in the paragraph (1) must conform to those set forth in the Entity Certificate.

Third Section

Requests Permission

Section 11

(1) The request for permission was submitted in writing by the BUJK leadership.

(3) Surrender of power in the form of a power letter to

authoring the permit by signing above the Rp.6.000.00 certificate,-(six thousand) rupiah by the BUJK Chairman.

Section 12

(1) The Construction Services Business Entity submitting a request

IUJK is required to have an Entity Engineering Responsibilities Handler.

(2) The engineering of the planning services business entity, the execution service and the Supervision services must have a certificate of skill and/or expertise in accordance with the classification and qualification of the construction workforce.

(3) The Acting Agency's Engineering Responsibilities as referred to

on the paragraph (1) is required to have a Engineering Governing Card provided by an instance that is responsible for the construction service.

New Permission

Section 13

Any new permit requests are required to qualify as follows:

a. fill the application form correctly and clearly;

b. submitted the recording of Akta Establishment of BUJK;

g. submitted a list of the life history of the business entity;

Second paragraph

Extension Permit

Article 14

(1) IUJK The extension may be submitted within 14 (fourteen) Calendar days before expiration.

(2) Exemption Extension, must supplement the provisions

c. submitted the Recording Management Card Record (PJT-BU);

d. Hand-in-the-person-to-charge (PJT-BU) to be delivered to the Board of Engineering Responsibilities (PJT-BU), which the Institution has registered;

e. Handing over footage of the People's Sign Card, NPWP, SKA's formal education diploma, the ahli/skilled power SKT in the event of an employee's shift;

g. submitted the recording of the applicable Domisili BUJK Attraction and Leges it;

j. submitted an IUJK original certificate.

Third Paragraph Data Change

Section 15

(1) Any IUJK holders who perform the mandatory data changes

notify the authorized provider no later than 14 (fourteen) business days after a change occurred.

(2) The data change can only be done in terms of:

a. The entity is not in the state:

1). has been set to enter into a black list of companies that are in trouble by the authorities.

2). In the process of investigation or investigation by the authorities in relation to a case of violation of law.

3). is set on the basis of a court ruling committing acts against the law.

b. An Entity ID can only be used for the following:

3). transfer of addresses and data supporting the administration of the BUJK office unless moving out of area.

4). changes to the classification and or qualification of the construction services.

(3) The change of data is required to complete the provisions as follows:

a. Fill out an application for a change.

b. submitted the recording:

1). the deed of the name change/administrator name and or change the name of the Entity created by Notary for changes to the management and or the BUJK name;

2). the applicable Domisili BUJK captions and are rumored to be a Kelurahan for the change of the BUJK address;

3). An Entity ID that is still in effect for classification changes and business qualifications; and

c. submitted the original IUJK.

Paragraf Fourth Closing Permit

Article 16

(1) Any BUJK that wishes to close permissions in

area, is required to notify the Regent or The designated officer.

(2) The closure of the mandatory permit complements the following provisions:

a. Fill out a list of the following:

b. submitted the original IUJK; and

c. submitted the Nihil Tax Letter.

(3) BUJK that has met the requirements as referred to in paragraph (2) will obtain the IUJK Closing Attraction signed by the Regent or the Official which is designated.

Article 17

Any application submission as referred to in Article 13, Section 14, Section 15 and Section 16 at the time of the completion of the submitted documents, the applicant must show the original document to be known by authorized officials.

Section 18

(1) Any published IUJK is mandatory using the code number

permission.

Part Sixth

IUJK Appliances

Article 19

(1) IUJK has a period of three (three) years in effect and may be extended for any time expiration.

Section 20

CHAPTER IV RIGHTS AND OBLIGATIONS

Section 22

Article 24

IUJK holders are entitled to:

a. follow the construction services procurement process; and

b. getting coaching from the Government.

Article 25

(1) IUJK holders are required to meet the provisions of:

a. purified, covering general safety requirements, building construction, quality of job results, quality of materials and or building components according to planning, and equipment quality in compliance with applicable standards or norms;

b. security, safety, and health site construction sites in accordance with applicable laws;

c. Applicable;

d. Local environment and environmental management are in accordance with applicable law.

(2) IUJK Licensee is required to carry out the construction work in a timely manner, quality and time.

(3) IUJK Licensee with The fields of the Managing and Supervising Effort are obliged to oversee the results of the construction products according to the specifications and design in the contract and refer to the provisions of the software.

(4) IUJK holders with the Business Planning, compulsory

Article 26

Article 27

d. IUJK closing list;

f. list of BUJK affected by administrative sanctions; and

(4) The form, content and layout of the report-making as referred to in paragraph (1) are further set up with the Regent Regulation.

(5) The form, content and layout of the report by the Regent to the Governor as referred to in the paragraph (2) indicated on this Section Regulation attachment.

CHAPTER V COACHING

Section 28

Article 30

Article 31

Article 32

Section 33

IUJK-Regional IUJK supervision guidelines are further regulated by the Rules of the Regents.

BAB VI

SIGN OF THE INDIVIDUAL PEOPLE ' S EFFORTS

Article 34

Article 35

(1) The registration of individual persons is performed through the Regents designated by the Regent.

Section 36

Article 37

BAB VII SANCTION ADMINISTRATION

Article 38

(1) The Count or Acting Officer based on its authority

may provide administrative sanction to Licensee. IUJK.

(2) administrative sanctions as referred to in paragraph (1) are:

a. a written warning as a first warning, a rebuke that does not stop and negate the company's right to attempt;

b. freezing IUJK, which will cause the company to not be allowed to carry out construction work for a while; or

c. revocation of IUJK that will negate the company's attempt.

(3) The introduction of sanctions against BUJK As referred to in paragraph (1) is mandatory to the general public between the construction service information system and/or the IUJK publishing agency's board of announcements.

Section 39

The written warning is conducted if the IUJK holder commits a breach of the provisions as referred to in Article 28 of the paragraph (1) the letter d and Article 28 of the paragraph (5).

Article 40

BUJK will be subjected to IUJK freezing sanctions if:

a. ignoring a written warning as set out in Article 39 with a term of 30 (thirty) calendar days,

by not fulfilling its obligations and notwithstanding the warning was delivered;

b. In violation of the provisions of Article 25 of the paragraph (1) letter a, letter b, letter c, letter d, Section 25 of the paragraph (2), paragraph (3) and paragraph (4); or

c. entry into the blacklist.

Section 41

The IUJK freeze mechanism as referred to in Section 40 as follows:

a. IUJK certificates from BUJK dropped freezing sanctions were withdrawn by the IUJK provider Instancy; and

b. The IUJK agencies issue an IUJK freeze certificate.

Article 42

BUJK will be subject to IUJK revocation sanctions if:

a. Committing a violation as referred to in Article 40 and already under IUJK freeze sanctions;

b. are getting the IUJK freeze sanctions as referred to in Article 41 but still carry out the work;

c. has proven to lead to construction failures and/or building failures; and

d. follows an auction process that is not an area or its efforts.

Section 43

IUJK is reapplied when it has met the following criteria:

a. The company has heeded the warning warning; and

b. exercised its obligations in accordance with the applicable provisions.

Article 44

The IUJK reenaccation mechanism as referred to in Section 43 is as follows:

a. The company submits an IUJK reinstatement in writing by attaching the proof of fulfilment of the required obligations to the Regent or its designated Acting Officer;

b. A regent or a designated officer checks the application of the application and performs field verification if necessary;

c. In the event of a request file as well as a proof of the fulfilment of the obligation, a Regent or a designated Acting Officer may provide an IUJK reenaccation letter;

d. The selected Bupati or Acting Officer may give the IUJK certificate back to BUJK the applicant; and

Article 45

(1) For the individual person's efforts in violation of the provisions of Article 34 are subject to written commemoration.

Article 46
