Key Benefits:

Purpose: A single admission application file is in place for dependent elderly accommodations.
Entry into force: the text comes into force on 1 June 2012; establishments that, locally, already use a single file have a one-year deadline to comply with the national single file.
Notice: the desire to generalize the use of a single application file for admission to residential care for dependent elderly people (EHPAD) meets a goal of simplifying the procedures for admission to EHPAD for the benefit of older persons and their families, medical professionals including treating physicians, but also co-ordinating physicians. There are many applications, with very heterogeneous content, varying according to the establishments. The decree provides that the application file is in accordance with a model set by order. This application file has been developed in consultation with institutional managers as well as with health professionals from cases already implemented in certain regions or departments.

The Prime Minister,
On the report of the Minister of Labour, Employment and Health and the Minister of Solidarity and Social Cohesion,
Considering the code of social action and families, including its article L. 312-1;
Considering the opinion of the National Committee of the Health and Social Organization of 23 February 2012;
Considering the opinion of the National Committee of Retirees and Older Persons of 14 March 2012;
In light of the Advisory Committee's opinion of 1 March 2012,
Decrete:
Paragraph 9 of subsection 2 of section 1 of chapter II of title I of Book III of the Code of Social Action and Families is supplemented by an article D. 312-155-1, as follows:
"Art. D. 312-155-1.-The application for admission to a dependent elderly accommodation facility is in accordance with a file defined by decree of ministers responsible for the elderly and health. »
The provisions of this Decree shall enter into force on 1 June 2012.
Dependent elderly accommodation facilities that use, at the same department level, a single admission file have a one-year period from that same date to comply with the provisions of Article D. 312-155-1 of the Code of Social Action and Families.
The Minister of Labour, Employment and Health and the Minister of Solidarity and Social Cohesion are responsible, each with regard to him, for the execution of this decree, which will be published in the Official Journal of the French Republic.
