Order of October 22, 2003 setting out the models of documents provided for in articles 9, 12, 16, 18, 19, 47 and 83 of Decree No. 2003-1010 of October 22, 2003 relating to the budgetary, accounting and financial management, and to the financing and pricing of social and mediocial institutions and services referred to in I of Article L. 312-1 of the Code of Social Action and Families, and of the establishments referred to in 2°111-2

Considering the code of social action and families;
In light of Decree No. 2003-1010 of 22 October 2003 on budgetary, accounting and financial management, and on the financing and pricing of social and mediocial facilities and services referred to in I of Article L. 312-1 of the Code of Social Action and Families, and the establishments referred to in 2° of Article L. 6111-2 of the Code of Public Health,
Stop:

Article 1


Pursuant to Article 12 I of the above-mentioned Decree of 22 October 2003, documents relating to the presentation, vote and control of the budget of social or social institutions or services must be in accordance with the model set out in Annex 1 to this Order.

Article 2


Pursuant to article 16, paragraph 3, of the above-mentioned Decree of 22 October 2003, the multi-year funding plans for social or psychosocial institutions or services are presented in accordance with the model set out in Appendix 2 to this Order.
The table of operating costs in Appendix 10 to this Order is attached to the multi-year funding plans.
The documents provided for in articles 5 and 6 of this Order are attached to the multi-year funding plans.

Article 3


Pursuant to the II of Article 9 of the above-mentioned Decree of 22 October 2003, the table of apportionment of expenses common to the main budgets and annexes of social or social institutions or services shall be in accordance with the model set out in Appendix 3 to this Order.

Article 4


Pursuant to the 4th of Article 16 of the above-mentioned Decree of 22 October 2003, the accounting balance of the establishment or social or medico-social service managed by a private law body must conform to the model set out in Appendix 4 to this Order.

Article 5


Pursuant to Article 19 of the above-mentioned Decree of 22 October 2003, the investment programmes of social or psychosocial institutions or services and their funding plans are presented in accordance with the model set out in Appendix 5 to this Order.
The documents in Annexes 2, 6, 7 and 10 to this Order are attached to the Investment Programs.

Article 6


Pursuant to Article 47 of the above-mentioned Decree of 22 October 2003, the financial balance of an institution or social or medical-social service is presented in accordance with the model set out in Annex 8 to this Order.

Article 7


Pursuant to section 83 of the above-mentioned Decree of 22 October 2003, the standard table on annual movements of the liaison accounts of a privately managed socially or medically-social institution or service is presented in accordance with the model set out in Appendix 9 to this Order.

Article 8


Pursuant to the third paragraph of Article 18 of the above-mentioned Decree of 22 October 2003, the staffing table shall conform to the model set out in Annex 11 to this Order.

Article 9


The Director General of Social Action is responsible for the execution of this Order, which will be published in the Official Journal of the French Republic.
