Judgment of September 22, 2004 setting the list and regulation of complementary specialized medical diplomas

In view of the amended Decree No. 88-321 of 7 April 1988 establishing the organization of the third cycle of medical studies;
Considering Decree No. 2004-67 of 16 January 2004 establishing the organization of the third cycle of medical studies;

    The list of complementary specialized medical degrees that can be acquired under the third cycle of medical studies is based on their membership group as follows:


    Group I


    Addictology.
    Allergology and clinical immunology.
    Andrology.
    Cancerology.
    Dermatopathology.
    Foetopathology.
    Hemobiology-transfusion.
    Reproductive medicine.
    Legal medicine and medical expertise.
    Sports medicine.
    Emergency medicine.
    Vascular medicine.
    Neonatalogy.
    Neuropathology.
    Nutrition.
    Orthopedics dento-maxillo-facial.
    Infectious and tropical, clinical and biological pathology.
    Clinical Pharmacology and Therapeutic Assessment.
    Child and adolescent psychiatry.


    Group II


    Child surgery.
    Maxillofacial surgery and stomatology.
    Surgery of face and neck.
    Orthopaedic surgery and traumatology.
    Reconstructive and aesthetic plastic surgery.
    Thoracic and cardiovascular surgery.
    Urological surgery.
    Vascular surgery.
    Visceral and digestive surgery.
    Geriatric.
    Medical animation.
    Group II diplomas are entitled to the qualification of a specialist corresponding to the title of the diploma.

    The content of each complementary specialization degree is specified in a model annexed to this Order. It defines the duration of the training, the program of theoretical teachings, practical training courses and the list of specialized degree courses to access it.

    Registered for the complementary specialized degrees referred to in Article 1 above, the medical staff and assistants of the hospitals of the armed forces.
    Internals take an annual administrative registration with the university of the subdivision, which they report, according to the rules established by the board of the medical training and research unit and approved by the president of the university concerned.
    In order to be able to enrol in a complementary specialized degree in Group II, the internals must have completed, no later than the end of the fifth internship, a course specific to this degree.
    Enrolment to several complementary specialized degrees is not permitted.

    The studies for the complementary specialized diplomas referred to in Article 1 shall have a duration of two years if they belong to Group I and three years if they belong to Group II, carried out consecutively or not in the approved services, in accordance with the procedure provided for in Articles 68 and 68-1 of the aforementioned decree of 7 April 1988 and by Article 30 of the decree of 16 January 2004 referred to above.
    For the so-called supplemental specialization diplomas of Group I, two internships must be carried out during the course of the boarding school, unless duly justified, granted by the co-ordinating teacher mentioned in article 23 of the aforementioned decree of 16 January 2004. The other two courses have hospital-university or hospital functions in approved services.
    For the so-called Group II complementary specialization degrees, four internships must be completed during the internship. The other two courses have hospital-university or hospital functions in approved services.

    In each interregion, universities with at least one medical training and research unit may be empowered to issue the complementary specialized diplomas referred to in Article 1 of this Order.

    The teachings are organized by the medical training and research units of the universities authorized for this purpose, in terms determined by the councils of the medical training and research units, on the proposal of the teacher diploma coordinator in the interregion, and approved by the university president(s).

    Lessons are taught in training and research units, regional hospital centres that are part of the hospital and academic centres and other candidate-level institutions.

    The preparation of each complementary specialized degree is under the responsibility of a teacher responsible for coordinating the organization of the theoretical and practical teachings of each complementary specialized degree in the interregion; It is designated, for a period of three years, renewable once in a row, upon presentation of a training project, by the directors of the inter-region medical training and research units after advice from the relevant training and research units as well as speciality teachers.

    The co-ordinating teacher may, by derogation from the provisions of Article 4 above, after the advice of the competent commission for the relevant specialised degree, authorize the candidates to complete or three internships of their training during the boarding school, where the practical training obligations of the postulated specialised degree permit, or after the graduation of the specialized degree.

    Interregional co-ordinating teachers of the same degree of complementary specialized studies are responsible, after consultation, for formulating proposals:
    (a) Training and medical research units on the content, modalities and methods of teaching evaluation;
    (b) To the various boards for the accreditation of the internships, provided for in article 30 of the decree of 16 January 2004 referred to above, with regard to the criteria for the accreditation of the services, including:
    1. The supervision and educational means;
    2. The degree of responsibility of the internals;
    3. The nature and importance of care activities and possibly clinical research.
    As necessary, the coordinators of two complementary specialized degree degrees are working together on the content and conditions of access to the theoretical teachings of their training and make proposals for joint accreditation of internship.

    In each interregion or group of interregions, an interregional commission of coordination and evaluation specific to each complementary specialized degree assists the co-ordinating teacher; she proposes the grant of the complementary specialized degree at the end of the last internship.
    The Complementary Specialty Diploma may only be issued to the holders of one of the specialized degree diplomas mentioned in the schedule specific to the Complementary Specialty Diploma.
    The interregional commission referred to in Article 11 is based on:
    - the validation of the entire theoretical training;
    - the validation of all courses required for the complementary specialized degree, certified by an internship notebook or, if not, by the forms annexed to the order relating to the organization, conduct and validation of the interns;
    - the appreciation of the coordinator teacher;
    - the opinion of the director of the training and research unit within the unit.

    The interregional commission, established for each complementary specialized degree, includes:
    The professor of hospital-practice to coordinate for each interregion the organization of theoretical and practical teachings;
    At least three teachers and hospital staff holding hospital and university centres, including two of the discipline, appointed by the directors of the training and medical research units jointly organizing the teachings. These teachers are responsible for the teaching of the relevant complementary specialized diplomas; they must belong to the various training and medical research units in the interregion.
    The members of the commission are appointed for a period of three years by the directors of the medical training and research units, the commission meets at least once a year.

    The Interregional Coordination and Evaluation Commission meets at least once a year, convened by the co-ordinating teacher, to examine the content and modalities of teaching and validation of teaching and internships. She hears, in an advisory capacity, an intern enrolled in the complementary specialized degree; it is designated by the co-ordinating teacher on the proposal of the Association of Internals of the speciality under consideration and, where applicable, the most representative interns in medicine union.
    It is consulted, for advice, by the teacher coordinator of the specialized degree concerned as part of the filing of applications for accreditation of the practical intern training courses provided by each hospital or extra-hospital service chief.

    Additional practical courses, validated in approved services under a specialized degree or a different complementary degree of study, may be taken into account for the validation of the training by the commission referred to in Article 12 above, provided that they are carried out in addition to the theoretical and practical training obligations required by the model of the approved university degree of complementary studies postulated and after agreement of the teacher coordinator, according to the rules

    Different teachings from those of the complementary specialized degree to which the candidate is enrolled may be taken into account for the validation of the training in accordance with the terms defined for practical courses in Article 13 above.

    The provisions of this Order shall apply to internal staff who are enrolled in a complementary specialized degree from the 2004-2005 academic year.
    The provisions of the Orders of May 4, 1988 setting out the list and regulation of complementary specialized medical diplomas are repealed.
    Internals in the course of complementary specialized studies remain subject, for their training, to the models annexed to the decision of May 4, 1988 on the regulation of complementary specialized diplomas.

    The Director of Higher Education at the Ministry of National Education, Higher Education and Research, the Director of Hospitalization and Care Organization and the Director General of Health at the Ministry of Health and Social Welfare are responsible, each with respect to him, for the execution of this Order, which will be published in the Official Journal of the French Republic.
