The Minister of Defence,
In view of the Council of Europe Convention for the Protection of Persons to The automatic processing of personal data, signed in Strasbourg on 28 January 1981;
In view of the law n ° 78-17 of 6 January 1978 relating to computers, files and freedoms, as amended in particular by law n ° 2004-801 of 6 August 2004;
In view of Decree No. 2005-850 of 27 July 2005 on the signature delegations of the members of the Government;
Having regard to Decree No. 2005-1309 of 20 October 2005 for the implementation of Act No. 78-17 of 6 January 1978 relating to Computers, files and freedoms, as amended by Act No. 2004-801 of 6 August 2004;
In view of the receipt by the National Commission on Informatics and Freedoms of 11 April 2006 with number 1150663,
Stop:

Article 1


The Ministry of Defence, at the Percy Army Training Hospital, is hereby established an automated processing of personal data, referred to as " Cardiac stimulation and/or defibrillation ", implemented by the cardiology department and whose purpose is the follow-up of medical devices implanted with cardiac stimulation and/or defibrillation.

Article 2


The following categories of personal data are registered:
-identity (name, first name, date of birth, sex, address, telephone number);
-health (history, targeted diagnosis and coded, etiology, symptoms).
Registered personal data are retained 20 Years.

Article 3


The recipients of registered personal data are, according to their respective responsibilities And the need to know:
-referring physicians, specialists, and contractors;
-the secretaries of the service.

Article 4


The planned opposition right Article 38 of the aforementioned Law of 6 January 1978 cannot be invoked as part of this processing.

Article 5


The right Of access and rectification provided for in Articles 39 and following of the Law Exercised with the Central Directorate of the Military Health Service (Office of Information and Communication Systems), BP 125, 00459 Armées.

Article 6


The Chief Medical Officer of the Percy Army Training Hospital is responsible for the execution of this Order, which will be published in the Official Gazette of the French Republic.
