Dated November 13, 1997, amending and supplementing Act
Czech National Council no. 368/1992 Coll., On administrative fees, as amended
amended, and amending Act of the Czech National Council no.
551/1991 Coll., on the General health insurance Company of the Czech Republic,
amended, and Czech national Council Act no. 565/1990 Coll.
on local fees, as amended

Parliament has passed this Act of the Czech Republic:
Article I


Czech National Council Act no. 368/1992 Coll., On administrative fees, as amended
Czech National Council Act no. 10/1993 Coll., Act no. 72/1994 Coll., Act No.
. 85/1994 Coll., Act no. 273/1994 Coll., Act no. 36/1995 Coll.
Act no. 118/1995 Coll., Act no. 160/1995 Coll., Act no. 301 / 1995 Sb.
And Act no. 151/1997 Coll., Is amended as follows:

First § 6. 1 and 3 added:

"(1) The fees prescribed in the tariff fixed amount is paid without
assessment and are payable prior to the operation specified in each item
Tariff, unless otherwise provided in the notes to the individual items
Tariff. If the taxpayer does not pay the fee at this time, invite
him the authority to do so within 15 days from the day following the receipt
challenges. at the same time teach him that, if not met the challenge, launched
the procedure is stopped. Therefore, the challenge to a payment can be submitted
appeal within 15 days from the day following the receipt of the invitation.

(3) the administrative authority will issue the outcome of the operation until after the taxpayer || | proves that it paid the fee, if not in the notes to the individual items

period referred to in paragraphs 1 and 2, the administrative body fails to act or
stops commenced proceedings if they do not exclude special rules under which the administrative authority
performs an act subject to a fee.

payment of the fee and proceed in the proceedings as if it were

2. in § 7 para. 2 sentence of the third and fourth sentences are replaced by"

3rd § 7 para. 3-5 added:

" (3) Conversion of Czech currency to foreign currency and conversely performs

exchange market, the Czech National Bank does not declare, the rate to be used for USD

4. § 8 par. 3, 4 and 6 are added:

5th § 9. 1 point. c) reads:

7th In § 9. 2 is deleted letter d), including footnotes. 3a) and
letter e) refers to the letter d).

8th In § 9. 4, after the words "appropriate steps" following words: "

9th § 9 the following paragraph 6, including footnote no. 4) reads:

"(6) The health insurance while proving entitlement to the exemption
pursuant to paragraph 2. A) not subject to a confidentiality clause by

10th Under § 9, the following § 9a is inserted:

"§ 9a

(1) of the charge collected under item 20 point. C) Tariff
competent administrative authority (the municipality) is obliged to transfer to the state
budget for each gaming machine permit to operate amount

- CZK 5,000 in permits issued in more than three months,

- 8000 CZK when the permit issued for a maximum of six months

- CZK 15 000 to enable issued for a calendar year.

(2) the municipal authority for the amount referred to in paragraph 1 purchase revenue stamps and pasting
upper parts revenue stamps in its filing records relating to

charges, as amended

Czech National Council Act no. 565/1990 Coll., On local fees, as amended
Czech National Council Act no. 184/1991 Coll., The Czech National Council
no. 338/1992 Coll. and Act no. 48/1994 Coll., is amended as follows:

First In § 1, after letter f) is replaced by a comma and full stop, point
g), added:

"G) fee for the operation of slot machines.".

Second Under § 10 the following § 10a), which, including footnotes. 15) reads:

"§ 10a

(1) Fees for the operation of slot machines subject to any
released gaming machine. ^ 15) The municipality is not obliged to grant an exemption from this fee
.
|| | (2) the fee for the gaming machine pays its operator.

(3) the rate of the fee for each gaming machine to three months makes
from CZK 5,000 to CZK 20,000.

Article V


This Act comes into force on 1 January 1998.
