DECREE



of 21 June 1999. August 2007,



amending Decree No. 364/2005 Coll., on the conduct of schools and

school facilities and school to the Registrar and to the transmission of documentation

schools and educational institutions, and from the school Registrar's Office (the Decree on the documentation

schools and educational institutions), as amended by Decree No 389/2006 Sb.



The Ministry of education, youth and sports is determined according to § 28 para. 6

Act No. 561/2004 Coll. on pre-school, primary, secondary, higher

vocational and other education (the Education Act), as amended by Act No.

383/2005 Coll.:



Article. (I)



Decree No. 364/2005 Coll., on the leadership of schools and school documentation

facilities and school to the Registrar and to the transmission of documentation of schools and

educational institutions and from the school Registrar's Office (the Decree on the documentation of schools and

school facilities), as amended by Decree No 389/2006, is amended as follows:



1. In article 3, paragraph 3. 1 (b). a), the words "of the pupils or students" shall be replaced by

"pupils, students and applicants for admission to education".



2. In paragraph 3, at the end of paragraph 2 the following sentence "the transmitted data shall include

also, changes to the content of the school Registrar's Office and the documentation of the school or school

the device, which occurred from the corresponding applicable data in

preceding calendar year. ".



3. in article 3, the following paragraph 3a is inserted:



' section 3a

4. in annex No. 1 is in the article. 1 (1). 1 (b). a) after the word "year"

which has permanent residence, except in the case of the Czech Republic ".



6. in annex No. 1 is in the article. 2 the following paragraph 3 is added:



"(3) by candidates for admission to education in the higher vocational school

passes a set of data, that contains:



and departmental identifier) of the school and the record date,



(b) the applicant's identification data):



1. social security number, year and month of birth, sex,



2. the country of citizenship,



3. the municipality, which has a permanent residence in the territory of the United States, where appropriate,

State in which the permanent residence, except in the case of the Czech Republic,



c) details of any previous education candidates:



1. the previous point of application applicants



2. departmental identifier of the school in which the candidate received secondary education

with the GCSE, the scope of education, year of execution of the graduation exams,



d) data on education, to which the candidate reports, including the scope of the

education, the length and form of education,



e) particulars on the outcome of the recruitment procedure and any information about the training,

to which the candidate has been accepted, including the scope of training, the length and form of the

education and on the registration of candidates to the appropriate higher vocational education

school. ".



7. in annex No. 1 of the following article. 5, including the title reads as follows:



"Article. 5



The transmission of the individual data from the school Registrar's Office and the documentation of the basic

school



(1) a basic set of data is to be transmitted on each pupil primary school and

contains:



and departmental identifier) of the school and the record date,



(b)) the identification of the pupil:



1. social security number, year and month of birth, sex,



2. the country of citizenship,



3. the municipality, which has a permanent residence in the territory of the United States, or State

which has permanent residence, except in the case of the Czech Republic,



c) details of any previous education pupil:



1. the previous point of application of the newly adopted student



2. departmental identifier of the school in which the pupil is educated before the adoption of

to education in elementary school,



d) details of the relevant education begins in primary school including

the date code and begin training,



(e) data on their education) in the primary school:



1. the date code and their education in the elementary school



2. an indication of how their education,



f) information about the education in elementary school:



1. scope of the education code, the length of training,



2. the year in which the pupil educates, the number of completed years of compulsory

school attendance,



3. identification of the class, group,



4. teaching language, foreign language codes, which the student learns



5. teaching subjects in a foreign language, number of items, number of hours,



6. during the education code, grade, how implementation of mandatory

school attendance,



7. the financing of the education of the pupil from the public budgets,



g) control items:

8. in annex No. 2 is in the article. 1 (1). 3 and 4(1). 1 (1). 4 (b). and)

9. in annex No. 2. 1 (1). 3, the first sentence is inserted after the phrase "Data

the numbers of teachers of secondary schools, conservatories and colleges

15. in annex No. 2. 7 (2). 2 at the end of the text of subparagraph (a))

the words "handicapped" shall be replaced by "pupils with special

17. in annex 2, article. 8 the following article. 8A, including title

added:



"Article. 8A



Passing the aggregate data from the school Registrar's Office and the Central documentation

school and Conservatory, relating to the admission to training



(1) the data for the current school year: the number of candidates registered and

admitted to the first year of education at a time form of training in

the first round of the admissions process by industry education, the nature and length of the

education and previous location, the number of registered and taken to

education to a higher level of six years of first-year and eight-year

high school in the first round of admission referred to in the previous point of application.

20. in annex No. 2. 15 paragraph 2 reads as follows:

23. in annex No. 2. 18 at the end of the text of paragraph 1, the words

Article. (II)



This Decree shall take effect on the date of its publication, with the exception of article. (I)

point 6, which shall take effect on 1 January 2000. January 1, 2008.
