DECREE



of 26 March. February 2004,



implementing the law on Antarctica and on amendments to certain laws



Ministry of the environment shall determine pursuant to section 31 of Act No. 279/2003

Coll., on Antarctica and on amendments to certain laws (hereinafter the "Act") to

the implementation of § 8 para. 1 (b). I), § 14 para. 2, article 15, paragraph 2. 8, § 16 para.

12 and section 24 (b). h) of the Act:



§ 1

A. IDENTITY OF THE NOTIFIER



1. for natural persons, the name or names, surnames, date of birth,

nationality, phone, and address of the place of residence or

the address of the residence of aliens abroad; for the entrepreneurial natural persons

Special licence the Observer pattern



The front side of the card contains the following information: the observer



In the upper left part of the frame is a photo card of the observer. In

the bottom left of the card is a big coat of arms of the Czech Republic. In the right

the top half is distinctive inscription "Licence the observer under the Treaty

Antarctica ", under which lists the card number, name and surname

his observer, issue date, the designation of the authority which issued the licence, and

the signature of the person responsible.



On the back of the licence the observer lists the same information as the

the front side of the card in English, French, Spanish and Russian

language.
