The Ministry of Finance shall determine according to § 18 para. 4 of Act No. 424/1991 Coll.,
on Association in political parties and political movements as amended by
Act No. 561/2004 Coll.:
§ 1
Model form for submission of annual financial reports of the political
the parties and political movements, the Chamber of Deputies is given in annex
of this order.
§ 2
Regulation (EEC)
Is hereby repealed Decree No 456/2001 Coll., on the model form for submission of
annual financial reports of political parties and political movements
The Chamber of deputies of the Parliament of the Czech Republic.
§ 3
This Decree shall enter into force on 1 January 2000. September, 2005.

Annex
In the annex, the model form the words "Declaration of bankruptcy on its
assets on the date ... "shall be replaced by the words" insolvency proceedings on
her/his assets on the date ... ".
In the annex, in a pattern the form in part IV. the table first and second donations from
individuals in the second column name: "date of birth giver".

natural persons of the third column name: "the address of the place of residence of the donor."
