DECREE
of 5 April 2004. March 2012
on the establishment of diplomatic and service passports of the designs and patterns of the forms
requests for their release
Ministry of Foreign Affairs shall determine according to § 5 para. 5 (b). (b)) and §
20 (2). 8 of Act No 326/1999 Coll. on travel documents and amending
Act No. 283/1991 Coll., on the police of the Czech Republic, as amended
legislation (the law on travel documents), as amended by Act No. 136/2006
Coll., Act No. 140/2008 Coll., Act No. 197/2010 Coll. and Act No.
424/2010 Coll.:
§ 1
Patterns of diplomatic and service passports
(1) the model of a diplomatic passport with a machine-readable data carrier
data with the biometric data is listed in annex 1 to this notice.
(2) the model service passport is machine-readable data carrier and data with
biometric data is given in annex 2 to this Decree.
(3) machine-readable data shall be entered in the specified zone in the bottom of the
the other side of diplomatic and service passports referred to in the annexes.
1 and 2 to this Decree.
(4) the data carrier with the biometric data is located on the other side
diplomatic and service passports mentioned in annexes 1 and 2 to this
the decree and is readable when you open these passports.
(5) the Unabridged names, or names and surnames are recorded in the
official records to side 3 referred to diplomatic and service passports
in annexes 1 and 2 to this Decree.
(6) on pages 6 and 7 of diplomatic and service passports as referred to in
Annexes 1 and 2 to this Ordinance is given index ^ 1) with translations
the names of the relevant data being written into these passports in German,
Italian, Spanish, Portuguese, Greek, Italian, Swedish, Dutch, English,
Finnish, Greek, Irish, Slovak, Slovenian, Polish, Lithuanian,
Maltese, Lithuanian, Estonian, Latvian, Russian, Chinese and Arabic.
§ 2
Forms applications
(1) the model of the application form for the issue of a diplomatic passport with a machine-readable
legible characters and with the data carrier with the biometric data is listed in the
Annex No. 3 to this notice.
(2) the specimen application for the release of a service passport is machine-readable
legible characters and with the data carrier with the biometric data is listed in the
Annex No 4 to this notice.
§ 3
Transitional provisions
(1) the forms for applications for the issue of the diplomatic and service passport are
machine-readable data and with data carrier with the biometric data referred to in
models set out in annexes 7 and 8 of Decree No. 495/2006 Coll., on
the version in force until the date of entry into force of this Decree, may be
used until 31 December 2004. December 2012.
(2) diplomatic and service passports are machine-readable data and with
data carrier with biometric data in accordance with the models set out in annexes
No. 2 and 4 of Decree No. 495/2006 Coll., in the version in force before the date of application
the effectiveness of this order can be issued until 31 December 2004. December
2012.
§ 4
Regulation (EEC)
Shall be repealed:
1. Decree No. 495/2006 Coll., on the establishment of the models of diplomatic and
service passports and patterns of the forms asking for their release.
2. Decree No. 172/2008 Coll., amending Decree No 495/2006 Coll., on
determination of diplomatic and service passports of the designs and patterns of the forms
asking for their release.
§ 5
The effectiveness of the
This Decree shall take effect on the date of its publication.
