GOVERNMENT REGULATION



of 31 March 2004. August 2011



establishing a model of compliance with the controlling Authority for the protection of personal

data



The Government directs pursuant to § 38 paragraph 1(a). 5 (b). a) of Act No. 101/2000 Coll., on the

protection of personal data and on amendments to certain laws, as amended by Act No.

439/2004 Coll.:



§ 1



The pattern of the oily-card ("card") is given in the annex to

application of this regulation.



§ 2



(1) the card is double-sided paper card with rounded corners on the dimensions

6.8 x 9.9 cm that is packed into the transparent laminating film

7.4/x 10.5 cm.



(2) the card is on the back and the back equipped with the safety features,

which are the šedožlutý bitmap background with gradient colors, irisovým

watermark, the thread is visible in daylight and visible in the thread

ultra violet light.



(3) the Pre-printed information on the card are shown in capital letters.



§ 3



(1) on the front of the card is in the upper left corner of the great

coat of arms. Next to the large coat of arms is printed the text of the "OFFICE for

PROTECTION OF PERSONAL DATA ", which depending on the certificate holder

places the text "CARD INSPECTOR" or "CARD of DESIGNATED EMPLOYEES".



(2) on the front of the card is in a single column on the right under it

the text should be printed "title, name, or. First and last names "," CARD NUMBER "and

"RELEASE DATE".



(3) in the lower left corner of the RS card is printed the text "PHOTO

3.5 x 4.5 CM ", which is placed in the holder's color photograph

3.5 x 4.5/cm; the requirements for the technical construction

photos, the provisions of other legislation governing

the requirements for the technical construction photos for the issue of the civil

licence.



§ 4



(1) on the reverse side of the card is printed at the top in the middle of the text "HOLDER

THIS LICENCE IS ENTITLED TO PERFORMANCE AUDIT ACTIVITIES ACCORDING TO THE LAW NO.

101/2000 Coll., on the protection of personal data and on AMENDMENTS to CERTAIN LAWS, and

RELATED LEGISLATION. ".



(2) in the bottom of the reverse side of the card is in the middle of the text should be printed

"The PRESIDENT of the OFFICE for the PROTECTION of personal data" above which is placed

handwritten signature of the President of the Office for the protection of personal data and

the round official stamp.



(3) at the bottom is in the middle of the text should be printed card WS

"THE MISUSE OF THE CARD WILL BE PUNISHED".



§ 5



The validity of professional licence issued pursuant to Decree-Law No 8/2005

Coll., on the model of professional licence inspector to the Office for the protection of personal

data and the model of professional card of designated staff of the Office for

protection of personal data, conducting control activities, ends

the end of March 31. December 2011.



§ 6



Decree-Law No 8/2005 Coll., on the model of professional licence of the Inspector

The Office for the protection of personal data and on the model of professional licence

authorised staff of the Office for personal data protection, enforcing

control activities, is hereby repealed.



§ 7



This Regulation shall enter into force on the first day of the calendar month

following the date of its publication.
