DECREE



of 26 March. October 2006



the terms of the licence, staff of the institutions of sickness insurance

responsible for compliance with the mode temporarily unable to work

of the insured person



The Ministry of labour and Social Affairs shall be determined according to § 199 para. 1 of the law

No. 187/2006 Coll., on sickness insurance:



§ 1



Pass the employee sickness insurance institution entrusted with the control of the

meeting mode temporarily work insured includes:



and) the designation "Card of the employee responsible for control"



(b) the name of the Insurance Authority) that issued the licence,



(c)) a large coat of arms,



(d) an employee of the corresponding photo) demands on

photos for the issue of ID card ^ 1)



e) title, name, or first and last name of the employee,



f) personal number of the employee



(g)) the control permission expressed by the words "the holder of this licence has

permission to perform the compliance monitoring mode temporarily unable to work

the insured pursuant to Act No. 187/2006 Coll., on sickness insurance ",



h) number of the licence,



I) date of issue of the licence,



j) official stamp of authority of sickness insurance and signature of the person

authorized to issue a license.



§ 2



This Decree shall take effect on the date on which it takes effect, the law No.

187/2006 Coll., on sickness insurance.
