transfers of data to the national health information system



The Ministry of health shall, pursuant to section 120 of Act No. 372/2007 Coll.

on health services and conditions of their provision (law on

health services), to implement section 70 para. 6, section 75 para. 3, § 77

paragraph. 3 and section 78 of the Act on health services:



§ 1



(1) personal and other data are to be transmitted to the national health

information system (hereinafter referred to as the "information system") in electronic

the form of the direct registration or on a technical medium.



(2) direct registration is implemented with secure data transmission protocol

through remote access to the specified e-mail address

listed on the website of the Ministry of health or the

website by designated information System Manager

§ 2

The circle of providers and other people forwarding personal data and other

data in the national health information system for the needs of

the registers and the periodicity and time limits for the transmission of



1. The national cancer registry



1.1 range of providers and other persons that is forwarding the data:



Each provider, which provided for the diagnosis of, and each provider,

who is responsible for the treatment and monitoring of disease from a group of diagnoses

malignant neoplasms [C00-There codes according to the valid version of the International

statistical classification of diseases and related health problems in the

the text of the 10 revision ("ICD-10")] ^ 1), rated as

in situ neoplasms (ICD-10 codes: D00-D09), neoplasms of uncertain or

and) for each cycle of assisted reproduction within 3 days from the date on which the

4. National Register of cardiovascular operations and interventions

Each provider that has provided health care services to a patient in

the context of cardiovascular surgery or interventions.



4.2 Frequency and time limits for the transmission of data:



Data shall be transmitted for each operation and for kardiochirurgickou

each cardiovascular intervention after the release of the patient from the

medical care always for that calendar month, until the end of the following

the calendar month.



5. the national joint replacement registry



5.1 Circuit providers and other persons that is forwarding the data:



Each provider that has provided health care services to a patient in

5.2. Periodicity and time limits for the transmission of data:



Data shall be transmitted for each implantation of artificial joint

refund within 30 days from the date of their inpatient medical care to the patient.



6. National Register of occupational diseases



6.1 Circuit providers and other persons that is forwarding the data:



Each provider, which has recognised occupational disease pursuant to other legal

regulations ^ 2).



6.2 the Periodicity and deadlines, the transmission of data:



Data shall be transmitted for each occupational disease or risk of disease from the

professions that no longer meets the conditions for recognition of occupational diseases

or the risk of occupational disease, not later than the 10th day of

calendar month following the month in which you become a testimonial

about their occupational diseases or threats of occupational diseases with a valid

or a decision on it has power.



7. National Register of treatment of drug users



7.1 range of providers and other persons that is forwarding the data:



7.1.1. each provider of health services providing expert care

persons using addictive substances and persons dependent on those substances

under the law governing the measures to protect against the harm caused by

tobacco products, alcohol and other addictive substances ^ 3) in the form of:

(d)) the detoxification carried out within ambulatory or inpatient care, or



e) substitution treatment by all providers

health services, regardless of their scope or field of expertise.



7.1.2. Each provider of social services ^ 4) support

caused by tobacco products, alcohol and other addictive substances;

advice.



7.2 the Periodicity and deadlines for transmission of data:



7.2.1. Periodicity and deadlines for transmission of substitution treatment: data

related to substitution treatment are to be transmitted for each commenced,

modified or completed treatment. Before the start of substitution treatment is

the provider shall be obliged to verify that the patient is not provided

replacement therapy with another provider. Data related to

substitution treatment are to be transmitted within 24 hours after the first dose

substitution substances and within 24 hours after the termination of or change in the substitution

treatment.



7.2.2. Periodicity and deadlines for transmission of data to other forms of professional

care:



Data related to other forms of specialized care shall be transmitted

not later than the 25th day of the next calendar month after the date of

9. National Register of persons permanently excluded from blood donation

Data shall be transmitted for each person permanently excluded from blood donation,

immediately after the verification of confirmation of a positive result

examination of the person concerned on finding a permanent exclusion from blood donation.



10. National Register of autopsies and Toxicology tests carried out on the

Department of forensic medicine



6.3 Circuit providers and other persons that is forwarding the data:



Each of the departments of forensic medicine where an autopsy was performed and

toxicological examination.



10.2 the Periodicity and deadlines, the transmission of data:



Data shall be transmitted for each autopsy, including the results

the toxicological examination obtained at autopsy, and it immediately after

finding all the necessary data.



11. National Register of providers



11.1 Circuit providers and other persons that is forwarding the data:



Each body referred to in section 75 para. 1 of the law on health services.



11.2 the Periodicity and time limits for the transmission of data:



Data shall be transmitted for each provider of health services to 10

working days from the date of issue or obtain permission to provide

health services or as in the case of changes in those data

or suspension or cessation of activities.



12. National Register of health care workers



12.1 the circle of providers and other persons that is forwarding the data:



Each body referred to in § 77 para. 1 of the law on health services.



12.2 the Periodicity and time limits for the transmission of data:



Data shall be transmitted for each person professionally qualified to carry

the medical professions of doctor, dentist, pharmacist ^ 5)

health-care workers eligible for the medical profession without

professional supervision after obtaining a professional qualification or vocational and

specialized qualifications ^ 6) within 10 working days from the date of acquisition

vocational or specialized qualifications or after commencement, interruption or

termination of the medical profession. In the case of new facts and changes in

the abovementioned data, the data is supplied without undue delay.



13. National Register of donors



8.1 circle of providers and other persons that is forwarding the data:



Each provider of health services, which has identified potential

donors of organs, tissues and hematopoietic cells.



13.2 the Periodicity and time limits for the transmission of data:



Data shall be transmitted after the assessment of the medical fitness of potential donors, and

immediately after the identification of the potential donors of organs, tissues and

hematopoietic cells.



14. National Register of people waiting for a transplant



14.1 range of providers and other persons that is forwarding the data:



Each provider of health services, which seems to indicate the trainee to
